DESCRIPTION:
  • Restless leg syndrome (RLS) and periodic limb movement disorder (PLMS) are separate and distinct but related entities classified as intrinsic disorders of sleep.
  • Restless leg syndrome is a neurological movement disorder due to disagreeable leg sensations that are more troublesome during inactivity with a peak in sensitivity occurring between about 2:00 PM and 2:00 AM. Therefore, they may interfere with enjoyment of passive activities involving confinement of the lower limbs or with the ability to fall asleep. Symptoms are most intense at rest.
  • Periodic limb movements are brief rhythmic and repetitive brief dystonic limb movements of 0.5-5 seconds duration occurring in clusters during the sleep period. They are most likely to occur in the first half of the night and with a predominant frequency of 5-90 seconds. Dorsiflexion at the ankle, or flexion at the knee or hip may be seen in severe cases. It can occur in the upper extremities. PLMS occur in more than 80% of cases of RLS, but can occur independently from RLS.
  • If symptom onset is before age 45, there may be many more affected relatives; the disease usually progresses slowly. If symptoms onset is after age 45 it occurs less commonly in families but the disease usually progresses rapidly. It may be associated with a low serum ferritin level.

Both PLMD and RLS are also called (nocturnal) myoclonus, which describes frequent or involuntary muscle spasms. Periodic limb movement was formally described first in the 1950s, and, by the 1970s, it was listed as a potential cause of insomnia. In addition to producing similar symptoms, PLMD and RLS are treated similarly. Periodic Limb Movement Disorder Periodic limb movement disorder affects people only during sleep. The condition is characterized by behavior ranging from shallow, continual movement of the ankle or toes, to wild and strenuous kicking and flailing of the legs and arms. Furthermore, abdominal, oral, and nasal movement sometimes accompanies PLMD. Movement of the legs is more typical than movement of the arms in cases of PLMD. Movements typically occur for 0.5 to 10 seconds, in intervals separated by five to 90 seconds. In 1979, the Association of Sleep Disorder Centers (ASDC) set the parameters for determining the presence of PLMD:

Restless Legs Syndrome
Uninterrupted sleep is taken for granted by millions of Americans, but a significant number of people - those with restless legs syndrome - face a disorder that prevents them from getting the rest they need each night. Restless legs syndrome wreaks havoc on nighttime sleep patterns and a quality lifestyle. Individuals with restless legs syndrome experience uncomfortable sensations in the lower extremities accompanied by an uncontrollable urge to move their legs during rest. This occurs most commonly at night and can interfere with their ability to fall asleep. While it's estimated 20 million people have restless legs syndrome, the number may be much higher. Some people don't recognize the specific symptoms as abnormal, yet can't figure out why they are constantly tired during the day. Others are very aware of their condition, but don’t seek medical care. Due to these reasons and a general lack of awareness, restless legs syndrome is under-diagnosed. People who have it are understandably exhausted and struggle to make it through each day. Many people with restless legs syndrome complain of leg discomfort often described as a "creeping" or "crawling" sensation and an irresistible urge to move or kick their legs. The symptoms are often relieved by movement but tend to reappear again with rest. The cause of the disorder is not known, but several medications and conditions are associated with restless legs syndrome. Some of these conditions include peripheral neuropathy, renal failure, pregnancy and low serum (and brain) iron levels. Excessive caffeine use and nicotine are also known to aggravate restless legs syndrome.

Restless leg syndrome
Every night before I fall asleep I develop an unpleasant wriggly sort of sensation in my legs. Massaging my legs sometimes helps, but otherwise I have to get up and walk around. It can go on for ages and is now regularly disturbing my sleep so I feel tired all day. What's causing this and what can I do about it? Mary

You're describing what's known as restless legs syndrome, a common condition that affects up to one in 12 of the population. For most people, it's mild and improves with simple measures, although some need more intensive medical therapy. There are four characteristic features in restless legs syndrome. These include:
  • An irritating, non-painful sensation in your legs that gives you an overwhelming urge to move them. Occasionally, this sensation also occurs in the arms. Symptoms occur (or are worse) when you're resting. They improve when you move your legs around. Symptoms are worse from evening onwards, especially when you lie down, but you don't have to be in bed or going to sleep.

No one knows exactly why restless leg syndrome occurs. People who suffer from restless leg syndrome have uncontrollable urges to move their legs. A feeling of discomfort in the leg prompts the need for movement. For some people, these sensations may be the result of nerve damage. The sensations may also be caused by decreased blood flow through some vessels in the legs. These leg movements often occur after a person has gone to bed. This can make sleeping difficult.     • a "crawling" sensation
    • a "creeping" inside the calves
    • aches and pains in the legs
    • an urge to constantly move the legs
    What Relieves the Symptoms? The only thing that seems to relieve the discomfort is to move the legs. streching them or getting up and walking around on them. Unfortunately, this interrupts the sleep cycle, so people with RLS are often very tired from lack of sleep. What Can Be Done to Help RLS Sufferers Sleep? First of all, a sleep study (polysomnogram) is done to determine the cause of sleepiness. Once RLS has been diagnosed, mediction can be prescribed to relieve the symptoms. RLS SOURCES Restless Legs Syndrome Foundation is a terrific source of information and referrals. The RLS Foundation, Inc., is a non-profit agency dedicated to supporting as many as 12 million people who suffer from RLS and to educate the medical community that this disorder is physical and treatable.

    Restless legs syndrome more common than thought BY MITCH LESLIE Imagine that a platoon of furry spiders is slinking up your legs and you'll get a sense of what a person with restless legs syndrome endures every night as she tries to sleep. This sleep-robbing disorder may be much more common than previously recognized, affecting nearly 30 percent of the population, Stanford researchers report. The hallmark of restless legs syndrome (RLS) is an "itching, creeping, crawling feeling in the legs that is relieved by leg movements," said Clete Kushida, MD, PhD, director of the Stanford Center for Human Sleep Research and co-author of the study. Sometimes patients notice a burning or painful sensation, while some feel nothing but still have a powerful urge to move their legs. RLS is more than an annoyance. It steals sleep. The frequent leg movements needed to relieve discomfort prevent sufferers from dozing off, Kushida said. To make matters worse, people with RLS often have a related problem called periodic limb movement disorder, characterized by violent contractions of the limbs during sleep. These episodes can be extreme enough to wake the sleeper. Kushida said that because sleep is "chopped up," periodic limb movement disorder contributes to daytime fatigue.
