Intravenous regional sympathetic blockade (IRSB) for reflex sympathetic dystrophy (Complex Regional Pain Syndrome)
Clinical bottom line: There is no evidence from existing trials that guanethidine used in intravenous regional sympathetic blockade reduces the pain associated with reflex sympathetic dystrophy. Based on a small number of patients, there is weak evidence that ketanserin and bretylium may provide some relief. Reflex sympathetic dystrophy or Complex Regional Pain Syndrome (CPRS) describes the constellation of chronic pain conditions associated with hyperactivity of the sympathetic nervous system. Pain is usually constant, severe and unresponsive to conventional analgesics. Intravenous regional sympathetic blockade (IRSB) is an intervention where a drug known to block the sympathetic nervous system is given in high local concentration in the painful limb isolated with a tourniquet.

    Reflex sympathetic dystrophy (RSD) Reflex sympathetic dystrophies are clinically much more important than causalgia because they are frequent causes of pain and disability, especially in orthopedic and industrial practices, and also because prompt recognition and effective treatment result in rapid remission of symptoms and complete recovery. For many years they were frequently misdiagnosed and improperly treated, or both, and in patients sustained prolonged or permanent disability. Fortunately, in recent years there has been an increased awareness of these disorders, and effective therapy is being carried out in more and more patients.
    Etiology
    Reflex sympathetic dystrophy can be produced by: accidental injury, surgical or other iatrogenic injury, and micro- or macrotrauma associated with certain occupations; it is also associated with certain diseases such as myocardial infarction and necrologic disorders. Trauma Iatrogenic Injury A number of cases of RSD have developed from iatrogenic complication of surgical or medical therapy including amputation of the hand or fingers, excision of small ganglia or tumors of the wrist, forceful manipulation, tight casts, and myelography (57, 86). A significant number of cases of reflex sympathetic dystrophy have been reported that developed after accidental insertion of needles into the median nerve in the course of starting an infusion or into the sciatic nerve in the course of giving an intramuscular injection of analgesics (57, 87, 88). The syndrome can also develop after injection into or near major nerves of such irritating agents as thiopental sodium and alcohol.

The syndrome usually develops in an injured limb, such as a broken leg, or following surgery. However, many cases of RSD involve only a minor injury, such as a sprain. And in some cases, no precipitating event can be identified. Pain may begin in one area or limb and then spread to other limbs. RSD/CRPS is characterized by various degrees of burning pain, excessive sweating, swelling, and sensitivity to touch. Symptoms of RSD/CRPS may recede for years and then reappear with a new injury. Types Two types of RSD/CRPS have been defined:
    • Type 1 - without nerve injury
    • Type 2 (formerly called causalgia) - with nerve injury
    Both types share the same signs and symptoms.

    Reflex sympathetic dystrophy syndrome (RSDS), also known as complex regional pain syndrome, is a rare disorder of the sympathetic nervous system that is characterized by chronic, severe pain. The sympathetic nervous system is that part of the autonomic nervous system that regulates involuntary functions of the body such as increasing heart rate, constricting blood vessels, and increasing blood pressure. Excessive or abnormal responses of portions of the sympathetic nervous system are thought to be responsible for the pain associated with reflex sympathetic dystrophy syndrome.
    The symptoms of reflex sympathetic dystrophy syndrome typically begin with burning pain, especially in an arm, finger(s), palm of the hand(s), and/or shoulder(s). In some individuals, RSDS may occur in one or both legs or it may be localized to one knee or hip. Frequently, RSDS may be misdiagnosed as a painful nerve injury. The skin over the affected area(s) may become swollen (edema) and inflamed. Affected skin may be extremely sensitive to touch and to hot or cold temperatures (cutaneous hypersensitivity). The affected limb(s) may perspire excessively and be warm to the touch (vasomotor instability). The exact cause of RSDS is not fully understood, although it may be associated with injury to the nerves, trauma, surgery, atherosclerotic cardiovascular disease, infection, or radiation therapy.

    yes no Reflex Sympathetic Dystrophy: Complex Regional Pain syndrome Reflex Sympathetic Dystrophy is a painful and confusing condition that occurs after trivial injury often involving soft tissue around joints. The pain is extremely severe and far outweighs that normally produced by tissue damage. It is thought to be due to chemical changes in the nervous system, but the exact mechanism is not known. Treatment is aimed at maintaining function. The risk is that the pain which usually involves a limb means that the child will not use the limb and this can lead to wasting and permanent loss of function. Ordinary painkillers are often unhelpful and drugs which are often not thought of as painkillers are used to reduce the overactivity in the nervous system. Inheritance patterns
    As far as we can ascertain there is no inherited predisposition to this syndrome, although it may be associated with Ehlers-Danlos syndrome which is an inherited condition.
