Reflex sympathetic dystrophy (RSD)—recently renamed complex regional pain syndrome types I and II—is a documented condition described in the literature as a pain syndrome occurring as a result of trauma to a body part. Repetitive strain injuries have been described as injuries that at least partially involve microtrauma to soft tissue. In a number of cases patients with RSI have slowly developed RSD, which has been neglected in the literature. I have recently been introduced to the possibility that many RSI patients have "mild" or "early" RSD, and I want to alert other clinicians as well as patients. Traditionally, physicians have diagnosed RSD when certain documented medical criteria have been met. In order for the diagnosis of "probable RSD" to be made, the patient would need to exhibit five or more of the following criteria as indicated in the literature by Colton and Fallat (1966): 1. Allodynia (perception of pain from a stimulus that is normally not noxious) or hyperpathia (an exaggerated response to painful stimuli)

    Reflex sympathetic dystrophy
    Three years ago I had a terrible accident, catching my arm in a garden waste shredder. I've experienced severe pain and some sensitivity to my arm ever since. After years not being able to get any help, a new GP has diagnosed reflex sympathetic dystrophy and is sending me to see a specialist. What is this condition and how will they treat it? Alan

    Reflex sympathetic dystrophy (RSD) is a nerve problem that can follow as many as one in 20 injuries, although in some cases there's no history of trauma at all. The condition can also occur after surgery. The cause is unknown, however, it's not a psychological problem - there are very real physiological changes in the affected tissues. RSD typically produces severe burning pain, a swelling of the tissues, extreme sensitivity to touch, excessive sweating, difficulty starting movement, increased tone and reflexes, tremor, muscle spasms, weakness, fatigue, skin rashes and frequent infections. There are changes to the skin and underlying bone. The skin may fluctuate between being red, warm and shiny, and blue and cold. Bone changes include osteoporosis, which is irreversible.

I have found it very difficult to find any information about my condition. I have heard that it is a rare rheumatic disease. I have also heard that it is a form of neuropathic pain. Can you please explain what these mean? Also I have had RSD for three years now, and I have noticed that the pain has become more widespread in this time. Will the pain continue to spread and will it get worse?

    Various forms of injection, surgery or drug treatment have been tried for this condition. These can be quite successful for some patients. Once nerve damage has been present for some time it is unlikely to ever recover and the best treatment one can expect is to reduce the symptoms. The condition is unlikely to spread significantly but as with all chronic pain states, there is sometimes a hypersensitivity of the adjacent nerves which lowers the pain threshold around the affected area and seems to cause spreading of the pain.

    General Discussion Reflex sympathetic dystrophy syndrome (RSDS), also known as complex regional pain syndrome, is a rare disorder of the sympathetic nervous system that is characterized by chronic, severe pain. The sympathetic nervous system is that part of the autonomic nervous system that regulates involuntary functions of the body such as increasing heart rate, constricting blood vessels, and increasing blood pressure. Excessive or abnormal responses of portions of the sympathetic nervous system are thought to be responsible for the pain associated with reflex sympathetic dystrophy syndrome. The symptoms of reflex sympathetic dystrophy syndrome typically begin with burning pain, especially in an arm, finger(s), palm of the hand(s), and/or shoulder(s). In some individuals, RSDS may occur in one or both legs or it may be localized to one knee or hip. Frequently, RSDS may be misdiagnosed as a painful nerve injury. The skin over the affected area(s) may become swollen (edema) and inflamed. Affected skin may be extremely sensitive to touch and to hot or cold temperatures (cutaneous hypersensitivity). The affected limb(s) may perspire excessively and be warm to the touch (vasomotor instability). The exact cause of RSDS is not fully understood, although it may be associated with injury to the nerves, trauma, surgery, atherosclerotic cardiovascular disease, infection, or radiation therapy.

    What is reflex sympathetic dystrophy?
    Reflex sympathetic dystrophy (RSD) is a condition of localized severe pain. We have known about this syndrome since the 19th century when it was called causalgia. Over the years this syndrome has changed names. RSD is also known as Complex Regional Pain Syndrome Type 1 or reflex neurovascular dystrophy. In RSD a person experiences a stressful event such as trauma, infection, sprain, emotional stress, surgery or similar event which is demanding to the body. The body's nervous system then reacts by causing the pain signals to continuously send signals to the brain. Your brain then interprets these signals as ongoing pain (pain cycle). Most people have their arms or legs which suffer from the pain, but almost any body part can be affected.

    The National Institute of Neurological Disorders and Stroke defines reflex sympathetic dystrophy syndrome (RSDS) as "a chronic condition characterized by severe burning pain, pathological changes in bone and skin, excessive sweating, tissue swelling, and extreme sensitivity to touch. The syndrome, which is a variant of a condition known as causalgia, is a nerve disorder that occurs at the site of an injury (most often to the arms or legs).
