Case Presentation:
A 58 year-old white male was referred for evaluation of right foot and ankle pain of four months duration associated with swelling. There was no history of trauma, surgery or infection. He had been placed in a plaster cast for several weeks for pain of unclear etiology. Erythrocyte sedimentation rate was 29. Alkaline phosphatase was normal.
Findings:
The bone scintigram showed diffusely increased deposition of Tc-99m MDP in the ankle, subtalar, tarsal and metatarso-phalangeal joints of the right foot. Slightly increased radiotracer was deposited in the region of the tarso-matatarsal joints of the left foot. An X-ray of the right foot showed peri-articular osteopenia. A diagnosis of reflex sympathetic dystrophy syndrome was made. The patient markedly improved on conservative treatment.
Discussion:
Clinical:
RSDS often follows trauma or surgery. In 506 patients selected by Sleinert from a large post-operative series, 25% followed operations for crush injuries, 25% followed elective operations in the upper extremities., e.g., carpal tunnel release, palmar fasciotomies, and 50% followed sprains, contusions and other operations. Symptoms begin immediately after injury in 30% of patients; in the remainder they usually start during the first week. The Should Hand Syndrome is a variant of RSDS often related to myocardial infarction, cervical disk herniation and degenerative disease of the cervical spine.

What Is It? Reflex sympathetic (REE-flecks SIM-pah-theh-tick) dystrophy syndrome, or RSDS for short, involves a disturbance in the sympathetic nervous system which is the network of nerves located alongside the spinal cord and controls certain bodily functions, such as opening and closing blood vessels or sweat glands. It primarily affects the hands and feet. What Are the Symptoms?
  • First Stage
    • Affected area is painful and swollen. Changes in temperature and color of skin Rapid nail and hair growth Stiffness
    The second stage occurs after weeks or months, and includes the following symptoms:
    • Burning pain Cool skin Brittle nails Swelling Muscles spasms
    The third stage may result in permanent changes such as:
    • Severe pain Skin may become drawn.

This booklet has been produced for anyone interested in finding out more about reflex sympathetic dystrophy (RSD). Many people now prefer the term 'complex regional pain syndrome (CRPS)' to 'RSD'. There are also a number of other names which have been used to describe the condition in different situations, including 'Sudek's atrophy' and 'algodystrophy'. However, the term RSD is still in widespread use, probably because it is much easier to remember and say. So RSD is the term used in this booklet. We want to explain as much as possible about the condition; how it is diagnosed and how it can be treated. However we cannot hope to answer all your questions. Everyone is different and this booklet is not intended to replace an individual consultation with a doctor.

Preface to the Third Edition

The first "International Update on RSD / CRPS" was held at the University of South Florida on February 1 and 2, 2002. Thanks to the organizational efforts of Dr. Srinivasa Raja at John Hopkins University, the symposium presented an outstanding faculty of international experts on RSD / CRPS. The symposium formed a foundation for writing the third edition of the Clinical Practice Guidelines. The Clinical Practice Guidelines have become a standard for managing RSD / CRPS. As with prior editions, my intention has been to focus on providing health care providers and patients unbiased information that will enable patients to make informed choices about their treatment. As noted in a recent issue of the JOURNAL OF THE AMERICAN MEDICAL ASSOCIATION , the information had to be carefully selected and organized. * One of the major reasons why physicians do not follow clinical practice guidelines is that most are not concise enough for both physicians and patients to read, understand and follow. Guidelines need to be presented in a concise manner saying in effect, "These measures are recognized as essential, these others should be considered, and this group has been found ineffective or contraindicated."

What is RSD and how is HELPRSD involved? Reflex Sympathetic Dystrophy , or CRPS (Chronic Regional Pain Syndrome), is a condition that results from the dysfunction of the central and peripheral nervous systems thought to be caused by injuries that can range from light, repetitive motion injuries such as Tendonitis and Carpal Tunnel, up to major injuries such as crushing and high velocity impacts, although in some cases RSD can occur with no apparent injury.
It is a condition that has the ability to spread, which it does in a majority of cases, and can affect the entire body and can cause muscle, bone, and skin atrophy. RSD was once thought to progress in definitive stages, and is still thought to do so by most physicians, but as more knowledge is gained as to the progession of RSD the lines of those stages begin to blur and in many cases the stages seem not to apply.
RSD may last a few months or a lifetime and, although there are many treatments for RSD, there is no definitive cure. The symptoms also vary from person to person, some victims of RSD may have only a few and some may have many. RSD is a condition of maybes, could bes, probablys, and what ifs. The greatest ally to RSD is ignorance. As it is now, the majority of physicians are ignorant as to the flexibility of RSD in it's symptoms and causes, no two physicians have the same opinions and facts, and that does not bode well for victims of RSD. What is reflex sympathetic dystrophy?
Reflex sympathetic dystrophy syndrome (RSDS) is a condition that features a group of typical symptoms, including pain (often "burning" type), tenderness, and swelling of an extremity associated with varying degrees of sweating, warmth and/or coolness, flushing, discoloration, and shiny skin. What causes reflex sympathetic dystrophy?
RSDS is also referred to as "the shoulder-hand syndrome," "causalgia," and "Sudeck's atrophy." The exact mechanism of how RSDS develops is poorly understood. The theories include irritation and abnormal excitation of nervous tissue, leading to abnormal impulses along nerves that affect blood vessels and skin. A variety of events can trigger the condition, including trauma, surgery, heart disease, degenerative arthritis of the neck, stroke or other brain diseases, nerve irritation by entrapment (such as carpal tunnel syndrome) or shingles, shoulder problems, breast cancer, and drugs for tuberculosis and barbiturates. There is no associated event in one third of patients.

Description
The Reflex Sympathetic Dystrophy Syndrome Association (RSDSA) is a nonprofit professional and consumer organization founded in 1984 to support research into the cause, treatment, and cure of reflex sympathetic dystrophy syndrome, also known as Complex Reginal Pain Syndrome (CRPS), a multi-symptom syndrome usually affecting one or more of the extremities, although any part of the body may be affected. The only common symptom in all patients is pain. RSDSA funds research into the cause and cure of this syndrome, helps organize support groups, promote awareness among health professionals, and develop educational forums and conventions.
