Reactive Arthritis
Reactive Arthritis is one of more than one hundred different types of arthritis and mostly affects young men 16-35 years old. It is one of the most common forms of arthritis affecting young men, but it may also affect young women and children. It is called Reactive Arthritis because an infection in one part of the body causes a reaction in another part of the body without the infection being found there. Reactive Arthritis causes inflammation (redness, swelling, pain) in different parts of the body at the same time or in different parts of the body at different times. Arthritis is the most important symptom in a chain of events which is the attack of reactive arthritis. In a person at risk the attack is usually triggered by an infection. (A blood test may identify a protein on your white blood cells that indicates that you are a person at risk. This protein is called HLA B27. Up to 25% of patients with Reactive Arthritis do not have HLA B27). Often it is a bowel infection causing loose, frequent bowel motions (diarrhoea) or it may be an infection transmitted during sexual intercourse. Occasionally there does not seem to be a triggering infection. In sexually acquired reactive arthritis the first symptom noticed is usually a pain when passing urine or discharge of pus from the inside of the penis (urethritis). In women a discharge from the neck of the womb (cervix) may not be noticed. More...

What is reactive arthritis?
Reactive arthritis, also known as Reiter's syndrome, is a type of arthritis that occurs as a reaction to an infection somewhere in the body. Most infections that cause the disease originate in the genitourinary tract (the bladder, urethra, penis, or vagina) and are spread through sexual intercourse, a form of the disease called genitourinary Reiter's syndrome, or urogenital Reiter's syndrome. Other infections that can cause reactive arthritis include gastrointestinal infections due to eating contaminated food or handling contaminated substances, a form of the disease called gastrointestinal Reiter's syndrome, or enteric Reiter's syndrome.
Who is affected by reactive arthritis?
Reactive arthritis is characterized by inflamed joints and affects mostly young men, between the ages of 20 and 40. Although researchers are not sure why some people develop reactive arthritis in response to certain infections, a genetic factor (presence of the HLA-B27 gene) seems to increase the risk.

Reactive arthritis is one of the more common forms of arthritis. As well as the joints it can affect tendons and their tissues. It affects young adults in particular, but sometimes children and older people too. What is essentially one disease may go under different names. These include Reiter's syndrome and Reiter's disease. It is quite distinct from rheumatoid arthritis and osteoarthritis, and is one of a group of illnesses often known as 'sero-negative arthritis' or 'spondarthritis'.
What is reactive arthritis?
As the name implies, reactive arthritis is a reaction to an infection. Sexually-transmitted infection, acute infectious diarrhoea or food poisoning are the usual causes. As many as one or two out of every hundred individuals with these infections may develop an attack of arthritis.
Both gonorrhoea and non-gonococcal urethritis can provoke reactive arthritis, and in women cervicitis (inflammation of the cervix of the womb) may also be a cause. These infections usually produce some pain when urine is passed or there may be a discharge from the penis or vagina. The infections themselves can be very mild and may not show any symptoms. Therefore thorough clinical tests, usually carried out by a specialist genito-urinary physician, are necessary to detect or exclude infection. Reactive arthritis
(Reiter’s syndrome) Reactive arthritis (ReA) refers to acute nonpurulent arthritis complicating an infection elsewhere in the body. The most common bacterial infections known to cause reactive arthritis are Salmonella, Y. enterocolitica, C. jejuni, and C. trachomatis.
Clinical manifestations
The majority of patients will present with a history of previous infection (1-4 weeks earlier). Urinary tract symptoms (urethritis) as well as occular symptoms (conjunctivitis) may occur later in the course of the disease.
Diagnosis
Lab tests show an increase in acute phase reactants in the blood. In early or mild disease, radiographic changes may be absent or confined to juxtaarticular osteoporosis. With long-standing persistent disease, marginal erosions and loss of joint space can be seen in affected joints. Periostitis with reactive new bone formation is characteristic of the disease, as it is with all the spondyloarthropathies. Spurs at the insertion of the plantar fascia are common. Reactive arthritis is a clinical diagnosis and should be entertained with any case of asymmetric inflammatory arthritis or tendinitis.

The patient presented to her general practitioner with fever, rash and pain in the lower limbs. Swelling of the joints of the hands and left knee was found, with a widespread rash that covered the trunk, limbs and face. The rash comprised lesions of varying types, including maculopapular, vesicular and petechial lesions (Box). Rash in a child with Ross River virus disease The general practitioner transferred the patient to the state tertiary paediatric service. No antibiotics were given before transfer. On arrival at Princess Margaret Hospital for Children, Perth, the patient was unwell, with a fever of 38 C. A provisional diagnosis was made of septicaemia (probably meningococcal), and treatment was begun with intravenous ceftriaxone. Extensive investigations were performed, but results of all initial serological, polymerase chain reaction and culture investigations were negative. Rheumatology review was requested because of the prominent arthritic component of the illness. This revealed widespread polyarthritis, and the illness was felt to be a reactive or post-infectious process.
