A patient information handout on psoriasis, provided by an AAFP staff patient education writer, is presented on page 964. Psoriasis is a common dermatosis, affecting from 1 to 3 percent of the population. Until recently, the mainstays of topical therapy have been corticosteroids, tars, anthralins and keratolytics. Recently, however, vitamin D analogs, a new anthralin preparation and topical retinoids have expanded physicians' therapeutic armamentarium. These new topical therapies offer increased hope and convenience to the large patient population with psoriasis. P soriasis, one of the most common dermatoses, occurs in 1 to 3 percent of the population. Although it is rarely life-threatening, psoriasis can cause significant morbidity, social embarrassment, financial cost and disruption in patients' lives. While patients with extensive and severe disease may require potent oral therapy, less severe psoriasis is typically treated with topical medications. This article reviews both the power and the limitations of topical therapies in the treatment of psoriasis.

A patient information handout on psoriasis, written by the authors of this article, is provided on page 736. Psoriasis is characterized by red, thickened plaques with a silvery scale. The lesions vary in size and degree of inflammation. Psoriasis is categorized as localized or generalized, based on the severity of the disease and its overall impact on the patient's quality of life and well-being. Patient education about the disease and the treatment options is important. Medical treatment for localized psoriasis begins with a combination of topical corticosteroids and coal tar or calcipotriene. For lesions that are difficult to control with initial therapy, anthralin or tazarotene may be tried. The primary goal of therapy is to maintain control of the lesions. Cure is seldom achieved. If control becomes difficult or if psoriasis is generalized, the patient may benefit from phototherapy, systemic therapy and referral to a physician who specializes in the treatment of psoriasis. (Am Fam Physician 2000;61:725-33,736.) P soriasis affects about 2 percent of the U.S. population. In 1990, psoriasis was the reason for more than 1 million visits to physicians.

Psoriasis I. Definition: Psoriasis is a chronic skin condition which tends to run in families. Psoriasis affects between 1-3% of the population. It varies in severity from mild to severe. Psoriasis can affect the nails, scalp, skin and joints. Psoriasis can start at any age. When psoriasis involves the nails it usually causes pits in the nails as shown below. II. Causes:
  • The cause of psoriasis is unknown. In psoriasis, areas of the skin grow much faster than normal and form red, scaling patches. The scalp, elbows, and knees are the most common sites for psoriasis, however any part of the skin may become involved. Psoriasis is a problem only because it itches and is unsightly. It is not contagious. If the skin is traumatized, psoriasis can form in the affected area.
III. Treatment:
  • Treatment is temporarily effective, and may need to be continued for quite a while. There is no magic cure for psoriasis. If you have psoriasis of the scalp , it helps to wash your hair often. Moderate sunlight exposure of the scalp is often helpful. Do not get sunburned, since psoriasis may settle in areas of injured skin. Sunlight is an effective treatment for psoriasis, but sunlight causes aging of the skin and skin cancer, so don't overdo it.

What is psoriasis?
Psoriasis (say "sor-eye-ah-sus") is a condition that affects your skin and causes thick red marks that look like scales to form. The thick scaling is probably due to an increase in the number of skin cells. Sometimes pus-filled blisters form. Most of the time, the skin on the elbows and knees is affected, but psoriasis can occur anywhere on the body, including the scalp, fingernails and mouth, and even the skin over the joints. Doctors aren't sure what causes psoriasis. It seems to get worse in many people when they are under stress or they have an infection. Some medicines may make psoriasis worse, too. Return to top
