Figure 1. Pseudoxanthoma Elasticum. Characteristic skin lesions can be seen in the axilla and anterior to the axilla in a patient with pseudoxanthoma elasticum, an inherited disorder of connective tissue. The lesions are yellow, xanthoma-like papules that have become confluent to form plaques, and there are redundant folds of skin in the axilla. In general, the skin lesions of pseudoxanthoma elasticum occur in flexural sites such as the neck, axillae (as in this patient), antecubital and popliteal fossae, and groin, and are said to resemble "plucked chicken skin." Franz Wirz Description: A rare hereditary disorder with degenerated elastic fibres present in the skin, blood vessels and heart, due to calcification of elastic fibres. The skin abnormalities present with raised, soft yellowish papules, especially around the mouth, neck, axillae, elbows, groin, and periumbilical areas, sometimes involving most parts of the skin. The skin has been likened to the appearance of the skin of a plucked chicken. Ocular features usually include angioid streaks, retinal haemorrhage, loss of visual acuity, and, sometimes, macular degeneration and chorioretinitis. Cardiovascular complications generally consist of absence of the pulse in the arms and legs, intermittent claudication, hypertension, angina pectoris, peripheral periarterial calcification, and stenosis of the celiac artery.

Visceral angiographic findings in pseudoxanthoma elasticum
A Belli and S Cawthorne
Department of Radiology, St George's Hospital, London. Two cases of pseudoxanthoma elasticum are described in brothers who both presented with gastric haemorrhage. The angiographic findings demonstrate the spectrum of vascular abnormalities seen in this condition. Although rare, the combination of angiomatous malformations, aneurysmal dilatation, narrowing and/or occlusion of visceral or peripheral arteries in a relatively young patient with abdominal pain or gastrointestinal bleeding, should alert the radiologist to the diagnosis. Examination will usually reveal the characteristic xanthomatous papular rash and retinal angioid streaks which confirm the diagnosis.

Patients with this condition may also have angioid streaks of the optic fundus and angiomatous malformations of the blood vessels (which can bleed into the gastrointestinal tract). A similar condition may also occur with long-term and high-dose treatment with D-penicillamine which produces similar cutaneous features.
