Last Revision Date: 2003 Treatment of raised intracranial pressure is both medical and surgical. It is aimed mainly at lowering of intracranial pressure and secondarily at treating symptoms directly (e.g. headache). Unfortunately, all reports to date are anecdotal as there have not been any controlled clinical treatment trials for idiopathic intracranial hypertension. Weight Loss
Weight loss has been used to treat IIH for many years. Newborg in 1974 reported remission of papilledema in all nine patients placed on a strict diet. She used a low calorie adaptation of Kempner's rice diet. The patient's intake was 400-1000 calories per day by fruits, rice, vegetables and occasionally 1-2 oz of meat. Fluids were limited to 750-1250 ml/day and sodium to less than 100 mg/day. All patients had reversal of their papilledema. Unfortunately, there was no mention of the patients' visual testing. The beneficial effects of weight loss have also been reported more recently. Kupersmith and colleagues retrospectively reviewed the charts of 250 IIH medically treated IIH patients from two centers and tabulated results on 56 patients that had at least 6 months of follow-up and otherwise met entry criteria.

PTC causes symptoms and signs of isolated elevated blood pressure in the cranium, or covering of the brain. The disorder can lead to significant, negative effects on the visual system. Increased pressure of the cerebrospinal fluid, that is, fluid around the brain, is a factor, but the cause of the disorder is not clear. There has been documentation of clustering of PTC within families. It suggests that potential genetic polymorphisms-abilities to take on different forms-may become evident after exposure to conditions known to trigger PTC. Thrombosis comes about by interactions between genetic and environmental or acquired factors, or both, resulting in a blood clot at a specific time and location. Because the disease occurs in episodes, the interaction of the genetic and nongenetic risk factors is important. Cystinosis is a recessive disorder caused by deposits of cystine within the lysosomes of cells-that is, sac-like cell parts that contain various enzymes. Involvement of the kidneys remains the primary characteristic, eventually leading to renal failure. Of all of the risk factors that make it easier for blood clotting, a high level of a substance called homocysteine is of particular interest. Too much homocysteine in blood plasma is a common finding in patients with kidney failure, and it has been recently identified as an independent risk factor for diseases of the blood vessels.

General Discussion
Intracranial Hypertension (Pseudotumor cerebri) is characterized by increased pressure inside the skull. Intracranial means inside the skull and hypertension means high fluid pressure. Basically, the pressure of the fluid that surrounds the brain (cerebrospinal fluid or CSF) is too high. Elevated CSF pressure produces severe headache and often visual difficulties, which, if left untreated can result in loss of vision or blindness.
Pseudotumor Cerebri and Benign Intracranial Hypertension are both former names for Intracranial Hypertension (IH) which are now considered inaccurate. These names do not adequately describe the disorder and downplay the seriousness of IH. There are two categories of IH: Primary Intracranial Hypertension and Secondary Intracranial Hypertension.

The pressures of diving would have no effect on the intracranial pressure. The increased intracranial pressure causes symptoms that can be confused with those of decompression sickness - headache, tinnitus, nausea and vomiting. It also causes loss of vision.
Treatment Related
Treatment is by osmotic diuresis, diuretics, spinal taps and the placement of a drainage tube from the lower spinal canal into the abdomen.
Diver Related
Loss of vision occurs as a major effect of the condition . Nausea and vomiting are an underwater risk. Headache, ringing in the ears mimic decompression sickness. Diving Risk Assessment Risk from the Condition Diving would not adversely affect the condition. Risks from the Treatment Diuretics can cause dehydration , a known increased risk for decompression sickness.

Thank goodness there are people caring enough to devote so much time and energy to help better the lives of others. Thank you for this website. I am a 31 year old mother of one and wife. My family has suffered for years due to untreated PCOS. Now I have found, in my area, a spectacular doctor to treat me and am seeing results and can now see the "light at the end of the tunnel". I am, however, interested in learning more about a link with PCOS and pseudotumor cerebri. I was also diagnosed with PTC and now have a lumbar-peritoneal shunt. This is the beginning of recovery for me and my family and we are hearing links between the two and are interested in learning more. Can someone steer me in the right direction? Thank you, Stacie S.
