    Mechanisms of Proteinuria in Kidney

    Our work on kidney ultrafiltration over the last few years has demonstrated that albumin is processed by being filtered on the basis of its size and that the relatively large amount of albumin that is transported across the filter is returned to the blood supply, undegraded, by a rapid and specific high capacity pathway associated with intracellular trafficking in proximal tubular cells. The high capacity of this pathway makes it the likely focus for the etiology of albuminuria in most renal diseases. This means that discussion of the molecular mechanisms of albuminuria must recognise that glomerular negative charge and the status of glomerular heparan sulfate, and alterations in the size selectivity of the glomerular capillary wall, are not the major parameters governing the etiology of albuminuria. This represents a totally new understanding of the renal processing of albumin and now presents exciting new opportunities to explore new and old aspects of glomerular ultrafiltration, particularly in disease.

Larson reviewed the evaluation of proteinuria and ways to distinguish between benign and pathologic conditions. Causes of proteinuria include dysproteinemias, medications, infections of the urinary tract and tubular and glomerular diseases. The urine dipstick is a crude measure of protein excretion. Since false-positive and false-negative results may occur, quantitative 24-hour urine specimens must be obtained for accurate evaluation. False-positive test results may occur in cases of highly alkaline urine, highly concentrated urine or gross hematuria, and cases in which urine has been contaminated with certain antiseptics. False-negative results can occur in cases of dilute urine or in cases of nonalbumin proteinuria.

Diagnosis/Definition
  • Greater than 150 mg of proteinuria per day.
Initial Diagnosis and Management
  • Diagnosis is usually suspected on the basis of an abnormal dipstick urinalysis. The suspicion should be confirmed by either a 24-hour urine for protein or a spot urine protein to creatinine ratio of greater than 0.15 (equivalent to or greater than 150 mg of urinary protein daily). Be sure to use correct units when calculating ratio. Proteinuria is a marker of intrinsic renal disease, and causes of this need to be assessed. Perform a history and physical examination to evaluate for evidence of a systemic disorder that may cause glomerulonephritis, i.e., signs/symptoms to suggest collagen vascular disease such as SLE, cancer, chronic infections, or diabetes. Evaluate for evidence of nephrotic syndrome: serum albumin (low), cholesterol (high), the presence of edema, and greater than 3 gm of proteinuria daily.

Proteinuria: Causes
Glomerular permeability is affected by molecular weight, size, shape and electrical charge of molecules. It is also influenced by renal hemodynamics. In general, the renal threshold is 68,000 daltons, therefore the glomerular filtrate contains electrolytes (in a similar concentration to plasma) and low molecular weight proteins. These filtered proteins are normally taken up and catabolized by renal tubular epithelial cells, therefore altered renal tubular function can result in these proteins being retained in the urine (tubular proteinuria).
Alterations in the glomerular barrier from altered renal blood flow (e.g. passive venous congestion in congestive heart failure) may result in mild proteinuria, whereas glomerular disease will result in a significant and severe proteinuria, consisting mostly of albumin (glomerular proteinuria). Increased filtration of low molecular weight proteins that are in high concentration in blood (hemoglobin, myoglobin, Bence-Jones proteins) will result in a significant proteinuria as the high concentration of these proteins in the filtrate overwhelm the renal tubular resorptive capacity (prerenal proteinuria). Urinary tract inflammation and hemorrhage will also result in proteinuria because of the contribution of plasma proteins.
Note that there are also cases in which physiologic proteinuria occurs. These are usually due to alterations in the glomerular barrier and are typically transient in nature. Physiologic proteinuria may occur with exercise and fever. The exact mechanism is unknown, but may be due to alterations in renal hemodynamics.
