    We read a great deal about prostate cancer and enlarged prostates but what is prostatitis? Richard

    Prostatitis simply means inflammation of the prostate gland. It's the most common urological problem diagnosed in men under the age of 50. It can be acute (acute prostatitis), persistent or relapsing (chronic prostatitis). The term chronic pelvic pain syndrome is used more than chronic prostatitis. In acute prostatitis the symptoms come on quite quickly and men feel very unwell. Pain is often felt in the lower back, the lower abdomen above the penis, and in between the legs. Fever, aching muscles, shivers and problems passing water also usually occur. Although men of all ages may be affected, it's slightly more common in younger men. Acute prostatitis is relatively uncommon and is usually associated with infection spreading from the bladder. It may also develop as a complication of sexually transmitted infections such as chlamydia and gonorrhoea. Chronic prostatitis is far more common than acute prostatitis. Although the problems with pain and passing water are the same as with acute prostatitis, in general these symptoms don't come on so suddenly although flare-ups do occur. Men with chronic prostatitis often experience very poor quality of life because of their relapsing or persistent symptoms. It can be caused by bacteria but this is uncommon and often no explanation for it can be found.

    Prostatitis: Disorders of the Prostate
    Prostatitis may account for up to 25 percent of all office visits by young and middle-age men for complaints involving the genital and urinary systems. The term prostatitis actually encompasses four disorders:
    • Acute bacterial prostatitis is the least common of the four types but also the easiest to diagnose and treat effectively. Men with this disease often have chills, fever, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract, as evidenced by white blood cells and bacteria in the urine. It is treated with an appropriate antibiotic. Which antibiotics are used is determined by your test results. Treatment will last for between two weeks and a month. If the infection is severe, it may be necessary to go into hospital for antibiotic injections. If you have bacterial prostatitis, your doctor can look through a microscope and find bacteria in a sample of your urine.
      Chronic bacterial prostatitis is also relatively uncommon. It is acute prostatitis associated with an underlying defect in the prostate, a focal point for bacterial persistence in the urinary tract. Effective treatment usually requires identifying and removing the defect and then treating the infection with antibiotics. However, antibiotics often do not cure it.

    What Is It? Prostatitis is an acute or chronic inflammation of the prostate, a walnut-size gland located just below the bladder in men, which produces about 30 percent of the fluid portion of semen. Inflammation may be caused by an infection. Acute prostatitis is characterized by the sudden onset of symptoms; chronic prostatitis causes persistent, long-term symptoms that are less severe. Chronic prostatitis may affect as many as 35 percent of men over age 50. An inflamed prostate may press upon the urethra and completely block the flow of urine. Additionally, if left untreated, acute prostatitis may lead to a prostate abscess, an infected, pus-filled cavity. Treatment for the acute and chronic forms of prostatitis may differ considerably.

    By John D. Perry, PhD How tragic it is that the various professions seldom speak to each other, nor read each other's literature. While urologists discuss "prostatitis", physical therapists talk about "Levator Ani Syndrome". Are these just two different perspectives on the same disease? Or are we dealing with two distinctly different diseases? And, if different, is there any overlap between "prostatitis" and "levator ani"? Is this another classic "chicken vs. egg" question? And, why should men's diseases be any different from women's? These questions are provoked by a recent article by four urologists [Zermann et al, 1999] which asks "whether chronic pelvic pain syndrome may be a consequence of functional disease within the pelvis". They studied clinic patients with "chronic abacterial prostatitis"; It turns out that for 88 percent of 103 patients, "myofascial tenderness" in the rectal area was "always associated with the inability to relax the pelvic floor efficiently"; "rarely did these patients learn to relax and control pelvic muscles." Measured with urodynamic instruments, they found 92.2% of patients had "dysfunction of the pelvic floor muscles."

Cochrane reviews are regularly checked and updated if necessary. Background: Chronic prostatitis is a condition that causes men substantial morbidity through the associated constellation of urinary symptoms, sexual dysfunction, and pelvic pain. The etiology of chronic prostatitis is unknown, and the many and varied treatments for chronic prostatitis reflect in part this knowledge gap. One novel etiologic theory is that the reflux of urine into prostatic ducts causes prostatic inflammation via high concentrations of purine and pyrimidine base-containing metabolites in prostatic secretions. This theory has led to the use of allopurinol for treatment of chronic prostatitis in hopes of lowering prostatic levels of uric acid and improving symptoms. Objectives: To determine the effects of allopurinol in the treatment of chronic prostatitis Search strategy: Trials were searched in computerized general and specialized databases (MEDLINE, Cochrane Library, Cochrane Prostate Group database), bibliographies of obtained articles, and direct contact with authors.

If a man has an episode of prostatitis, he has a 50 percent chance of having a recurrence in his lifetime.
