Prostatitis: The Basics What is it? Prostatitis is an acute or chronic inflammation of the prostate, a walnut-size gland located just below the bladder in men, which produces about 30 percent of the fluid portion of semen. Inflammation may be caused by an infection. Acute prostatitis is characterized by the sudden onset of symptoms; chronic prostatitis causes persistent, long-term symptoms that are less severe.
Chronic prostatitis may affect as many as 35 percent of men over age 50. An inflamed prostate may press upon the urethra and completely block the flow of urine. Additionally, if left untreated, acute prostatitis may lead to a prostate abscess, an infected, pus-filled cavity. Treatment for the acute and chronic forms of prostatitis may differ considerably. It accounts for as many as 25 per cent of all visits to the doctor for problems in the genital and urinary systems by the young and middle aged. So what is it? Definition Prostatitis is the name given to inflammatory conditions of the prostate gland. It can be (very) painful during urination or ejaculation and you need to see your doctor to ascertain the cause and get appropriate treatment. If you do not get it treated it can result in serious complications. There are four types of prostatitis: Acute bacterial prostatitis : Is the least common form of prostatitis but it demands prompt treatment so that complications do not occur. Bacteria such as Klebsiella or Escherichia cause the symptoms which include fever, general pains and, more specifically, pain in the lower back and genital region, the testicles, penis and the area between the scrotum and anus. Frequent trips to the rest room may be required, with pain during urination. You may feel you need to go urgently. Painful ejaculation may also be a symptom. The doctor will be able to tell if the prostate is tender and swollen.

Abstract
BACKGROUND: Chronic prostatitis is a common diagnosis but the etiology of this disease is unknown. We wished to catalog symptoms and etiologic factors for chronic prostatitis to gather epidemiological data about the disease. We also wished to determine the feasibility of doing this using the internet. METHODS: A 54 question survey was designed and posted to several internet newsgroups dealing with chronic prostatitis. The questions dealt with symptoms, predisposing factors, demographic data and sexual practices. Responses were collected by electronic mail and tabulated at a central site. RESULTS: We received 163 completed surveys over a 2 month period. Nine percent of responses came from individuals outside of the U.S. or Canada. Only 9.8% of completed surveys contained one or more unreadable answers. The responding population consisted of relatively young men (mean 43 years) with a chronic, relapsing set of symptoms mostly characterized by pain at various locations in the pelvis, irritative voiding symptoms and pain associated with ejaculation. Most respondents were potent and had a single sexual partner.

This reflects primarily only our inability to demonstrate the causative bacteria, not their absence.

What is prostate drainage?
Prostate drainage is an essential part of Dr. AE Feliciano's treatment for prostatitis. This drainage must be done every two days or the whole treatment may fail. The rest of the treatment consists of several cultures of seminal fluid to determine the cause(s) of the infection and to determine the best antibiotic or antifungal to take, followed by a course of the appropriate antibiotic or antifungal.
Prostate drainage is an advanced version of the digital rectal exam (DRE). The doctor inserts his gloved finger into your rectum and presses on portions of the prostate in an attempt to drain all of the trapped fluid.

Prostatitis is a common urologic condition that many clinicians find difficult to treat effectively. It has been estimated that up to half of all men suffer from symptoms of prostatitis at some time in their lives. The prostate is a walnut sized gland that is located beneath the urinary bladder (see picture below). The prostate surrounds the urethra (urine channel) like a donut. When the prostate becomes inflammed it can impinge on urine flow and cause symptoms. The function of the prostate is to produce fluid and enzymes that contribute to the semen and are needed for male fertility. Prostatitis Prostatitis is often described as an infection of the prostate, but it can also be an inflammation with no sign of infection. Just 5-10% of cases are caused by bacterial infection. Prostatitis can affect men of all ages. An estimated 50% of all men experience prostatitis-like symptoms at some point during their lifetime. In fact, chronic prostatitis is the number one reason men under the age of 50 visit a urologist. In some cases, chronic prostatitis follows an attack of acute prostatitis. Chronic prostatitis may also be associated with other urinary tract infections. The primary symptom of chronic infectious prostatitis is usually repeated bladder infections. Prostatitis is considered chronic if it lasts more than 3 months. Types of prostatitis include: Acute bacterial prostatitis. A sudden bacterial infection characterized by inflammation of the prostate. This is the least common form of prostatitis but the symptoms are usually severe. Patients with this condition have an acute urinary tract infection with increased urinary frequency and urgency, need to urinate a lot at night, and have pain in the pelvis and genital area. They often have fever, chills, nausea, vomiting, and burning when urinating. Acute bacterial prostatitis requires prompt treatment, as the condition can lead to bladder infections, abscesses in the prostate or, in extreme cases, completely blocked urine flow. Left untreated, the condition can cause confusion, low blood pressure, and may be fatal. The condition is usually treated in the hospital with intravenous antibiotics, pain relievers, and fluids.

What is bacterial prostatitis? Bacterial prostatitis means you have an infection of your prostate gland. There are two types of bacterial prostatitis - acute (sudden onset) and chronic (persistent). (Another leaflet deals with non-infective prostatitis which is also called chronic pelvic pain syndrome.) What is the prostate gland? Only men have a prostate gland. It lies just beneath the bladder. It is normally about the size of a chestnut. The urethra (the tube that passes urine from the bladder) runs through the middle of the prostate. The prostate helps to make semen, but most semen is made by another gland nearby (the seminal vesicle). What is the difference between acute and chronic bacterial prostatitis? ACUTE BACTERIAL PROSTATITIS
This is when symptoms develop quickly. It usually occurs at the same time as a urine infection. So, you may also have a bladder infection (cystitis). Bacteria (bugs) are found in your urine if a urine sample is taken. Symptoms include:
  • Pain from your prostate, which may be severe. You feel this mainly at the base of your penis, around your anus, and lower back. Pain may spread to your penis and testes. Passing stools (faeces) can be painful.

  • Prostatitis PROSTATITIS To put it simply, prostatitis is inflammation of the prostate gland. There are three main classifications of prostatitis Acute bacterial prostatitis acute infection of the prostate gland, usually occurs in men aged 40 to 60 years. Chronic bacterial prostatitis recurrent infection of the prostate, occurs in men aged 50 to 80 years. It is associated with chronic urinary infection and, commonly, calcifications in the prostate (prostatic stones). In the cases of bacterial prostatitis, the culprit usually is a bacteria known as Escherichia coli. The cause of bacterial prostatitis is not welldefined, but one theory suggests a reflux of urine from the bladder into prostatic ducts. This reflux of urine may lead to prostatitis.
