Prostatitis: Disorders of the Prostate
Prostatitis may account for up to 25 percent of all office visits by young and middle-aged men for complaints involving the genital and urinary systems. The term prostatitis actually encompasses four disorders:
  • Acute bacterial prostatitis is the least common of the four types but also the easiest to diagnose and treat effectively. Men with this disease often have chills, fever, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract as evidenced by white blood cells and bacteria in the urine. The treatment is an appropriate antibiotic. Chronic bacterial prostatitis , also relatively uncommon, is acute prostatitis associated with an underlying defect in the prostate, which becomes a focal point for bacterial persistence in the urinary tract. Effective treatment usually requires identifying and removing the defect and then treating the infection with antibiotics. However, antibiotics often do not cure this condition. Chronic prostatitis/chronic pelvic pain syndrome is the most common but least understood form of prostatitis. It is found in men of any age, its symptoms go away and then return without warning, and it may be inflammatory or noninflammatory. In the inflammatory form, urine, semen, and other fluids from the prostate show no evidence of a known infecting organism but do contain the kinds of cells the body usually produces to fight infection. In the noninflammatory form, no evidence of inflammation, including infection-fighting cells, is present.

A patient education handout on prostatitis, written by the authors of this article, is provided on page 3025. P rostatitis is inflammation of the prostate gland. In clinical practice, the term prostatitis encompasses multiple diverse disorders that cause symptoms related to the prostate gland. One author has described prostatitis as "a wastebasket of clinical ignorance" because so many poorly characterized syndromes are diagnosed as prostatitis. The spectrum of prostatitis ranges from straightforward acute bacterial prostatitis to complex conditions that may not even involve prostatic inflammation. These conditions can often be frustrating for the patient and the clinician. Every year, approximately 2 million physician visits include the diagnosis of prostatitis. Prostatitis is a common condition. In a survey of National Guard members (20 to 49 years of age) using a self-reported diagnosis of prostatitis, a 5 percent lifetime prevalence was noted. A population-based study of men (40 to 79 years of age) in Olmstead County, Minn., suggests a lifetime prevalence close to 9 percent. The latter study used a medical record review to confirm physician diagnosis of prostatitis. Patients with a previous episode of prostatitis were at significantly increased risk for subsequent episodes.

Common side effects include restlessness, nausea, diarrhea, vomiting and abdominal pain. Less common are unpleasant taste, decreased appetite, dry mouth, rash, tremors, confusion, sleep disorders, nervousness, drowsiness, hallucinations, dizziness, and photophobia, which is a strong reaction to bright light. Seizures can be fatal if not treated promptly. Dosage
Administration with antacids, iron salts, sucralfate, or zinc salts decreases absorption. Ofloxacin is given in pill form, 200-400 mg every 12 hours on an empty stomach at least 1 hour before or 2 hours after meals with a full glass of water. Do not take with food. How long it may take to work
Symptoms should improve within a few days. Length of time for complete resolution depends on the type and site of infection.

What is prostatitis?
Prostatitis is common and affects many men at some time. Prostatitis is an inflammation of the prostate gland. When part of your body is inflamed, it is red, hot and sore. Prostatitis can cause many symptoms. It can make urinating difficult or painful. It can make you have to urinate more often. It can also give you a fever, low-back pain or pain in your groin (the area where the legs meet your body). It may make you less interested in having sex or unable to get an erection or keep it. What is the prostate gland?
The prostate is a gland that lies just below a man's urinary bladder. It surrounds the urethra like a donut and is in front of the rectum. The urethra is the tube that carries urine out of the bladder, through the penis and out of the body. Your doctor may check your prostate by putting a finger into your rectum to feel the back of your prostate gland. The prostate gland makes a fluid that provides nutrients for sperm. This fluid makes up most of the ejaculate fluid. We do not yet know all of the ways the prostate gland works.

In meinem Fall verursachte sie SCHMERZEN, die im Extremfall (über die Samenleiter) bis in die Hoden ausstrahlten. Das gefährliche daran war, daß die Konzentration auf den Straßenverkehr nachläßt und damit die Unfallgefahr steigt. Mit der Einfachsten Lösung: das Motorrad fahren aufzugeben kann und will ich mich (noch) nicht abfinden!!! Zumal ich auch ohne Motorrad zu fahren nicht schmerzfrei bin, wie das Winterhalbjahr 1998/1999 gezeigt hat. Im April 1999 begann ich, diese Seite zu gestalten. Mittlerweile habe ich einen Gesundheitszustand erreicht, mit dem ich sehr zufrieden bin. Auf dieser Seite können Betroffene nachlesen, wie ich mit der Krankheit umgehe und klarkomme, um vielleicht Anregungen für sich selbst abzuleiten.

This certainly is very frustrating for most doctors and patients, but can be a unique opportunity for those who want to dig into some large, yet still unexplored field in medicine (there are not too many left). This has been a challenge also to me, and I learned that a whole deal of very interesting knowledge can be gained by dedicating some positive interest to this matter. I would like to share my experience on this internet news group, which I consider a perfect platform for discussing this difficult field. In fact, I am convinced that scientific journals like the Journal of Urology are not suited for this purpose and I'll explain why: such periodicals accept articles only if they are based on methods which fulfill strict scientific criteria.
