Background
Finasteride decreases the conversion of testosterone to dihydrotestosterone and can reduce the volume of the prostate in men with prostatic hypertrophy. This study was designed to look at the efficacy of finasteride in improving symptoms and reducing the incidence of urinary retention and prostate surgery in men with symptomatic, enlarged prostate glands.
Methods
Patients Patients were enrolled between 1990 and 1992. Inclusion criteria: Men with a serum PSA between 4.0 and 9.9 were required to have a negative biopsy for cancer. Exclusion criteria: Use of alpha-blockers, anti-androgens, chronic prostatitis, prostate or bladder cancer or surgery.

Most men over the age of 50 develop benign prostatic hypertrophy (BPH), an enlargement of the gland that may be due to accumulation of testosterone in it. This condition can be asymptomatic or it can cause a number of urinary problems: increased frequency, decreased force of urination, and nighttime awakening to empty the bladder. If these symptoms become severe, doctors will urge surgery to remove all or part of the gland. Useful links Benign Prostatic Hypertrophy (BPH) A cross section through the prostate showing its relationship to neighbouring structures Benign enlargement of the prostate, otherwise known as benign prostatic hypertrophy transurethral prostatectomy , or TURP. In this operation a tube is inserted through the penis along the urethra to clear away the physical obstruction. Because the enlargement of the prostate is a process that takes place over many years, medical treatment of BPH is generally less effective than surgery. Medical treatments for benign enlargement will take a considerable period of time to improve urinary symptoms. It is estimated that over a period of approximately one year, 30% of patients will have a 30% improvement of their urinary symptoms with medical treatments. A TURP will lead to an improvement in nearly all patients. Medical treatment of prostatic enlargement is generally reserved for those patients who are not fit for surgery. Where there is a very long waiting list for surgery, your GP may give you medical treatment to ease the condition during the wait for definitive surgery. Medical treatments are also reserved for patients who do not want to have surgery but would like some treatment for their condition. Many of these treatments are hormonal and include the LHRH agonists and finasteride. Dietary supplements such as Saw Palmetto have some activity. Although we are not clear how this works, it does have an effect.

    In Brief: In men with BPH, oral administration of Serenoa repens 160mg twice daily for 1 to 3 months was generally superior to placebo in improving subjective and objective symptoms of BPH. The drug appears to have similar efficacy to finasteride as evidenced by data from over 1000 men. Few significant differences were demonstrated between Serenoa repens and a -adrenoceptor blockers in small comparative studies. Both alpha-adrenergic blockers and finasteride have demonstrated efficacy in reducing symptoms related to BPH. One-year trials combining an alpha blocker and finasteride did not show any benefit superior to single-drug therapy in relief of urinary symptoms. McConnell and coinvestigators report on data from a four-year trial of combination therapy for BPH.

    Enlarged Prostate Benign Prostatic Hypertrophy
    by Dr. Rita Louise The prostate is a walnut sized gland that sits just below the bladder in men and is an integral part of the male reproductive system. Made up of two lobes and enclosed by a layer of tissue, the prostate goes through two main periods of growth. The first occurs early in puberty, when the prostate doubles in size. At around the age of 25, the gland begins to grow again. This second growth phase often results in what is identified as an enlarged prostate. As the prostate gets larger, the layer of tissue surrounding it stops it from expanding, causing the gland to press against the urethra. While the data varies, it is believed that most men over the age of 45 experience some amount of prostate enlargement, but may live symptom free. This enlargement is usually harmless, but it often results in problems urinating later on in life. By 60, it is believed that 80% of all men experience some sort of urinary interference due to prostate enlargement.
