Benign prostatic hyperplasia (BPH)
Overview Symptoms Treatment Prevention Definition: BPH is a condition where benign (non-cancerous) nodules enlarge the prostate gland (the gland that produces the liquid in which sperm are expelled from the penis).

Causes, incidence, and risk factors: The actual cause of benign prostatic hyperplasia (BPH) is unknown. However, men who have had their testicles removed do not develop BPH and, after castration, BPH has been observed to regress. In other words, the presence of normally functioning testicles appears to be necessary for the development of BPH. Abnormally growing prostate tissue may use male hormones differently than normal prostate tissue. Although this tissue growth is non-cancerous, as the tumor grows larger it can obstruct the urethra and interfere with the normal flow of urine.
The incidence of BPH increases with advancing age. BPH is so common that it has been said, "All men will have benign prostatic hyperplasia if they live long enough!" A small amount of BPH is present in 80% of men over 40 years old and over 95% of men 80 years old. No risk factors have been identified other than having normally functioning testicles.

There were 375,000 U.S. hospitalizations for this condition in 1993. Prostate Gland Anatomy The prostate is a walnut sized gland located between the base of the bladder and the base of the penis. The urethra drains the bladder and passes through the center of the prostate and then into the penis. The prostate is made of firm glandular tissue surrounded by a fibrous capsule. It contains muscular tissue which squeezes prostatic fluid into the urethra as part of an ejaculate. It also is surrounded by nerves that are critical to men for achieving and maintaining an erection. Your doctor usually examines your prostate for potentially cancerous nodules, orders a PSA blood test and checks your urine for infection. If your doctor finds a nodule, he usually orders an ultrasound test and then cuts out small pieces of the nodules which are then analyzed for cancer. If he finds no cancer, he must check you for infection or benign prostate enlargement. It is so unusual to find bacteria in a man's urine, that it requires further tests to check your entire urinary system. PSA or Prostate Specific Antigen is made by prostate cells and released into the bloodstream. The larger your prostate, the more cells you have to release PSA into your bloodstream. Therefore, PSA is a measure of the size of the prostate gland, rather than just prostate cancer. A sudden rise in PSA means that the gland is enlarging rapidly which can be a sign of cancer, but it also can mean infection or benign enlargement. If you have an elevated PSA, burning on urination, feeling that you have to urinate all the time and terrible discomfort when your bladder is full, the odds are overwhelming that you have an infection, that can be very difficult to cure.

    What is it? Benign prostatic hypertrophy is also called "BPH." It is when the prostate gland slowly grows until it is larger than normal, or "enlarged." BPH is a very common problem in aging men. It is more common in men over 45 to 50 years of age. The prostate is a male sex gland that makes fluid called semen. It sits below the bladder and wraps around the urethra like a donut. The urethra is a tube that carries urine from the bladder into the penis. The urine is then passed out of the body. As the prostate grows, it can squeeze the urethra making it harder to pass urine. Causes: It is not known what causes BPH. It may be just part of getting older. Some care givers believe it is caused by the continual and cumulative exposure to testosterone (male sex hormone) as men age. Signs and Symptoms: Most men with BPH feel like they need to pass urine often. Some men may pass little or no urine. You may dribble or leak urine. You may pass urine while you sleep. Or you may need to wake up at night to urinate . You may have to push to start your urine stream. You may feel that you need to pass more urine even if you just did. You may also think your urine stream is weak. Your urine may look pink or red. This may mean there is blood in it. Wellness Recommendations: If you have prostatic enlargement you should avoid stimulants or decongestants (often found in over-the-counter cold and allergy medications) as these can contract the muscle fibers in the prostate and make the symptoms much worse.

    Treatment options for BPH include surgery and medications to reduce the amount of tissue and increase the flow of urine. If BPH is left untreated, urine can back up into the kidneys, leading to kidney damage and failure, and weakening of the bladder muscle so that all the urine is not expelled. In addition, BPH may lead to reduced sexual ability, painful orgasm, and impotence.
    Regular screening for prostate cancer, including the Prostate-Specific Antigen blood test (PSA) and/or Digital Rectal Examination (DRE), will also aid in the early detection of BPH.
