Nonoperative therapy, such as drug therapy or lifestyle modification, is aimed primarily at reducing bothersome lower urinary tract symptoms and preventing serious morbidity such as urinary retention and renal function impairment. As more men reach advanced age, it becomes crucial for the primary care physician to be aware of the epidemiology, pathophysiology, natural history, clinical presentation, and therapeutic modalities available for the treatment of clinically significant cases. B enign prostatic hypertrophy (BPH) is the nonmalignant enlargement of the prostate gland from an increase in cellular growth. BPH is the most common benign neoplasm among men, and its incidence increases steadily with age. Clinical BPH is well defined and quite prevalent: Up to 20% of men between 40 and 64and 40% of men older than 65are afflicted. Histologic BPH is found in 50% of men older than 60 and up to 88% of men older than 80.

Various factors affecting BPH are age, genetic makeup, testosterone levels, and environment. It can start in men's 20's but usually does not cause problems until middle age. It is not related to either Prostate Cancer, (PCa), or prostatitis. What is BPH The prostate gland lies below the bladder and surrounds the urethra, or urine tube. The prostate has little function in humans other than supporting the urethra and some of the sperm ducts. What are the symptoms of BPH? As the prostate enlarges it compresses the urethra causing obstruction to urine flow. The urinary stream becomes smaller, there may be difficulty in initiating the stream, (hesitancy), dribbling, intermittent flow, frequency of urination, getting up at night to void, (nocturia), inability to empty, or even inability to void at all, (retention). BPH can also be the cause of urinary tract infections. How is BPH Evaluated?

  • Signs and symptoms include decreased force and caliber of urinary stream, hesitancy, retention, postmicturition dribbling, double voiding (patient voids and is able to void again in 5 to 10 minutes), and overflow urinary incontinence (on straining or coughing). Irritative symptoms such as dysuria, frequency, nocturia, urgency, hematuria, and incontinence occur frequently. Flank pain during micturition, suprapubic pain, and azotemic symptoms occur less commonly. Exam . The bladder may be distended, and the prostate is enlarged, smooth, and symmetric. The prostate gland may be soft or firm and possibly nodular. However, the nodules lack the stony-hard consistency associated with carcinoma. Laboratory findings . UA may reveal signs of infection. If the obstruction has been severe enough to impair renal function, BUN and creatinine may be elevated. PSA may be elevated.

    The prostate is a small gland present in men, located just below the bladder. The prostate encloses the urine passage (urethra) and is usually about the size of a walnut. Although men have a prostate all their lives it usually begins to enlarge at mid-life. As men age, the prostate continues to grow and may push in on the urine passage. A prostate gland that is larger than normal is said to be hypertrophic, and the condition is called benign prostatic hypertrophy (BPH). An enlarged prostate does not indicate prostate cancer. The enlargement caused by aging is inevitable, but suffering from symptoms of an enlarged prostate is not. The Department of Urology at Wayne State University has extensive experience treating men with BPH. We have been a leader in urologic research and innovative surgical treatment. Most of the diagnostic procedures for BPH can be completed in office.

    Transurethral resection of the prostate is a common surgery for BPH often nicknamed "roto-rooter" surgery. In a TURP an instrument is passed through the urine passage to the point where the prostate is blocking the flow of urine. The excess tissue is then cut away. There is no incision on the outside of the body and usually a hospital stay of only a few days is required. TURP has been used for many years and most patients notice a rapid improvement in symptoms which lasts for many years.

As the prostate enlarges, which happens to almost all men as they get older, it can press on the urethra (the tube that empties urine from the bladder to the outside) causing problems with urination. Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer.

    GENERAL INFORMATION: What is it?
    • Benign (buh-nine) prostatic (prah-stah-tik) hypertrophy (hi-per-tro-fee) is when the prostate (prah-state) slows grows larger. It is also called "BPH." The prostate is a male sex gland that helps makes semen. It sits below the bladder and wraps around the urethra like a donut. The urethra (u-ree-thruh) is the tube that carries urine from the bladder into the penis. The urine is then passed out of the body. As the prostate grows, it can squeeze the urethra (u-ree-thruh) making it harder to urinate (pass urine). This can block urine flow and cause urinary problems. BPH is very common in men over 45 years of age but rarely causes problems before age 60. Tests will be done to learn about your prostate. You may need drug treatment or prostate surgery if your urinary problems get worse.
    Causes: It is not known what causes BPH. It may be just part of getting older. Some caregivers believe it is caused by a change in hormones as men age.

    Initial Diagnosis and Management
    • Patients with obstructive and irritative voiding symptoms should complete the International Prostate Symptom Scoring (IPSS) Sheet. Contact Urology to obtain a copy of this. Urinalysis to rule out infection or to discover microhematuria. PSA and digital rectal exam to evaluate for prostate cancer. IVP is not indicated unless patient has hematuria.
