December 2002, revised June 2004 An enlarged prostate – known as benign prostatic hyperplasia or BPH – is caused by an overgrowth of prostate cells. This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder.
The prostate gland
The prostate is a gland about the size of a walnut that is only present in men. It is located just below the bladder and surrounds the urethra, the tube through which urine flows from the bladder and out through the penis. Please see the diagram, overleaf. One of the main functions of the prostate is to produce an important liquefying component of semen, which allows the sperm to move freely. The gland is divided into three zones, peripheral, transitional and central. With BPH, it is the central part where overgrowth of cells takes place.

What is benign prostatic hypertrophy?
The prostate is an internal gland in men between the bottom of the bladder and the top of the penis. The urethra, the tube your urine passes down from the bladder when you pass water, runs through the middle of it. Benign prostatic hypertrophy is swelling of this gland. It is not cancerous. Because the prostate surrounds the urethra, when it swells it can block the flow of water.
What causes it?
There is no special cause for this condition. It just sometimes happens as you get older.

What is the prostate gland?
Men have prostate glands, women don't. The prostate gland sits under the bladder, and is coloured red in this diagram. The tube through which urine passes out is called the urethra, and travels right through the middle of the prostate gland.
The job of the prostate gland is to secrete a fluid, called prostatic fluid. When you ejaculate, the prostatic fluid is released, and mingles with the sperm. This fluid contains nourishment and protection for the sperm.
How common are symptoms?
As you get older, your prostate gland grows larger in size. This process is called benign prostatic hypertrophy, or BPH. As a result of this enlargement, your bladder has to work harder to force urine past the large prostate gland.

An extract of the fruit from the American saw palmetto plant has been used by native American Indians for hundreds of years to treat a variety of men's problems, and prostate gland swelling since the 1800s. Does it work? Systematic reviews [1,2] say that it does.
Search
The search for randomised controlled studies of Saw palmetto (using all those peculiar botanical names that plants have) against placebo or other treatments was thorough. It found 24 studies, 18 of which met the inclusion criteria. The mean age of men involved was 65 years and the mean duration of the studies was nine weeks.
Results
Compared with placebo, Saw palmetto improved peak urinary flow rate by an average of 1.9 mL/second and mean urinary flow rate by 2.2 mL/second (Figure 1). Saw palmetto did not decrease prostate size.

Patient rating of prostate-related symptoms showed significant benefits for Saw palmetto over placebo. In six studies (Figure 2) 242/329 (74%) men had symptom improvement with Saw palmetto compared with 168/330 (51%) with placebo. This means that for every 4.4 men treated with Saw palmetto (95% CI 3.4 to 6.5) one will have symptoms improved who would not have done with placebo.

As the prostate enlarges, which happens to almost all men as they get older, it can press on the urethra (the tube that empties urine from the bladder). This causes problems with urination. Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer. Causes, incidence, and risk factors Return to top The actual cause of prostate enlargement is unknown. In addition to age, overgrowth of the prostate seems to require normal function of the testicles, which produce testosterone. This is known because men who have had their testicles removed at a young age (for example, for testicular cancer) do not develop BPH. Similarly, if the testicles are removed after a man develops BPH, the prostate begins to shrink in size. The likelihood of developing an enlarged prostate increases with age. In fact, BPH is so common that it has been said, "All men will have an enlarged prostate if they live long enough!" A small amount of prostate enlargement is present in many men over age 40 and more than 90% of men over age 80. No risk factors have been identified other than having normally functioning testicles.
