As prostate cancer grows, it may spread to the interior of the gland, to tissues near the prostate, to sac-like structures attached to the prostate (seminal vesicles), and to distant parts of the body (e.g., bones, liver, lungs). Prostate cancer confined to the gland often is treated successfully. Anatomy The prostate gland is located in the pelvis, below the bladder, above the urethral sphincter and the penis, and in front of the rectum in men. It is made up of glandular tissue and muscle fibers that surround a portion of the urethra. The gland is covered by a membrane (called the prostate capsule) that produces prostate-specific antigen.

ATLANTA, Georgia (CNN) A study in the Journal of the American Medical Association indicates that prostate cancer screening can benefit men younger than age 50, the typical age screening begins. The American Cancer Society plans to advise men who are at high risk for the disease to begin yearly screenings at age 45. Those at high risk include men with a family history of the disease and African-American men. Dr. Harmon Eyre is chief medical officer for the ACS, and Dr. Robert Smith is director of cancer screening for the organization.

Prostate Cancer Overview
The prostate is a glandular organ present only in males. Only men develop prostate cancer. The prostate is normally about 3 cm long (slightly more than 1 inch) and lies at the neck of the bladder and in front of the rectum.
  • The prostate surrounds the urethra, which is a tubular structure that carries sperm and urine out of the penis.
    It produces a thin, milky fluid that is added to the sperm at the time of ejaculation.
    Older men often have an enlarged prostate, which is a noncancerous condition called benign prostatic hypertrophy (BPH) that causes urinary symptoms.
Cancer occurs when normal cells undergo a transformation in which they grow and multiply without normal controls.
  • As the cells multiply, they form a mass called a tumor.
    Tumors are cancerous only if they are malignant. This means that they encroach on and invade neighboring tissues because of their uncontrolled growth.
    They may also travel to remote organs via the bloodstream.

The risk of prostate cancer increases faster with age than any other form of cancer. The increasing life span of our aging population places a greater number of men at risk for this disease. One such man was my father. With little or no warning, Robert Benjamin Ablin went into acute urinary retention and was diagnosed with metastatic cancer of the prostate. This was in 1978, in an era of limited ability to diagnose prostate cancer. He was diagnosed in a late stage of his disease; he died one year later, in August 1979. Founded in his memory, the Robert Benjamin Ablin Foundation for Cancer Research is directed toward development and improvement of methods of early detection, diagnosis, and treatment of prostate cancer. What is the prostate gland?
The prostate gland is an organ that is located at the base or outlet (neck) of the urinary bladder. (See the diagram.) The gland surrounds the first part of the urethra. The urethra is the passage through which urine drains from the bladder to exit from the penis. One function of the prostate gland is to help control urination by pressing directly against the part of the urethra that it surrounds. Another function of the prostate gland is to produce some of the substances that are found in normal semen, such as minerals and sugar. Semen is the fluid that transports the sperm. A man can manage quite well, however, without his prostate gland. (See the section on surgical treatment for prostate cancer In a young man, the normal prostate gland is the size of a walnut. During normal aging, however, the gland usually grows larger. This enlargement with aging is called benign prostatic hypertrophy (BPH), but this condition is not associated with prostate cancer. Both BPH and prostate cancer, however, can cause similar problems in older men. For example, an enlarged prostate gland can squeeze or impinge on the outlet of the bladder or the urethra, leading to difficulty with urination. The resulting symptoms commonly include slowing of the urinary stream and urinating more frequently, particularly at night.
