2003 Summer School in Primary Immune Deficiency Disorders
Coconut Grove, FL
October 22-26, 2003 CIS received applications from North, Central and South America also Europe and the Middle East. A number of excellent applications were submitted making the selection process difficult. Thirty fellows were selected to attend the five-day meeting; twenty (20) from the United States, one (1) from Canada, one (1) from Kuwait and nine (9) from Central and South America. The international make up of the participants provided a unique learning experience. In preparation for the course, the faculty, also international in scope; seven (7) from the United States, three (3) from Europe, and one (1) from Costa Rica, provided each student with guidance on their case presentation and slides. The intensive five-day course covered topics including: SCID/CIS and transplant, WAS/XLT, DiGeorge, unknown defects, Hyper IgM, Hyper IgE, Mycobacterial Infections, XLA, AT and antibody defects. Student case presentations were followed by faculty lectures in each topic. The question and answer sessions that resulted from the presentations were invaluable. The participants did not leave the course empty handed. In addition to knowledge gained from the presentations, participants received a syllabus made up of teaching slides, articles of relevance, lab references and case studies. Attendees were encouraged to share this information with others in their labs.

Dr. Geha finds TACI gene
Its official, Dr. Geha and his team of Immunologists at Children's Hospital Boston found the genetic roots of another immune deficiency. The gene is called TACI and its related to IgA deficiency. Children with TACI have low levels of immunoglobulin A (IgA), immunoglobulin G (IgG) or both. Dr. Geha is Andy's doctor. TACI/IgA deficiency affects 1 in 600 people. For reference NEMO affects 1 in 250,000 people. One of the articles I found includes the following words from Dr. Geha,
"A test for TACI would allow for diagnosis of more children and their relatives," says Geha, senior author of the study and a professor of pediatrics at Harvard Medical School. For generally healthy people, its easy to take for granted our bodies ability to fight disease. But when theres a problem with ones immune system, life can be downright miserable until the source of the problem is discovered and addressed. Weve all heard of AIDSAcquired Immune Deficiency Syndromewhere the immune system is compromised by a virus. What you may not be familiar with are Primary Immune Deficiency Diseases, which result when the bodys immune system is missing a key component or simply doesnt work right. Often times, people are simply born this way. One in 20,000-50,000 people will suffer from one of nearly 100 disorders that fall under this general category. Print Preview of Section As with the humoral immune response, the cell-mediated immune response involves a complex series of events after antigens enter the body. Helper T cells are required, so some of the antigen must be taken up by macrophages and presented to helper T cells. The helper T cells bind the presented antigen and thereby become activated to divide and secrete interleukins. The interleukins in turn activate any killer T cells that have already bound antigen attached to class I MHC molecules on infected cells. The activated killer T cells can then kill any cells displaying antigen attached to class I MHC molecules, effectively eliminating any cells infected with the antigen.

It is an outcome of a 2001 NIH advisory panel's recommendations to accelerate basic and clinical research in primary immunodeficiency diseases and to attract new investigators to the field. "Until now, a relative scarcity of study subjects has limited a researcher's options for investigating these lesser known diseases," says Anthony S. Fauci, M.D., director of the NIAID. "The consortium will help advance our understanding by focusing research priorities, uniting established researchers with new investigators, funding small-scale clinical and preclinical projects, and creating or expanding research resources, including a cell line repository and Web-based patient registry to which all researchers will have access."

If the immune system fails, children get sick faster and for longer periods of time from usual childhood infections. These children are also susceptible to infection with ordinarily harmless organisms. Conditions in which a child is born with a defective or absent immune system are called congenital or primary immunodeficiency disorders (PID). The immune system is made up of several different types of white blood cells. Each white blood cell type is specialized to deal with different categories of infection. For instance, neutrophils generally fight bacteria, while lymphocytes generally fight viruses. Cell types also use different strategies to fight infection. For instance, T lymphocytes can kill other cells that are infected with a virus. B lymphocytes on the other hand work by making special substances in the blood called antibodies, which are proteins that fight infection.

Goals of the Fellowship Program:
The primary goals are to: 1. Provide state-of-the-art clinical training in allergy and immunology as it pertains to the evaluation and management of associated medical disorders. Emphasis will be placed on atopic disorders, such as asthma, rhinitis, sinusitis, and dermatitis. This is balanced with experiences in immune deficiency and autoimmune disease. 2. Develop a solid foundation in the principles of basic immunology fundamental to understanding and managing clinical disorders. 3. Initiate a solid experience in the fundamentals of research, either basic or clinical. This is intended to provide the base for career development in academic medicine and subspecialty clinical practice.
