The types of patients seen in our unit may include those with:
  • maternal/family history of cardiac disease
  • a known or newly diagnosed history of hypertension (high blood pressure)
  • preeclampsia (toxemia)
  • maternal metabolic diseases, including diabetes (both pregestational and gestational types)
  • infectious diseases (to include parvovirus, toxoplasmosis, hepatitis, HIV, and AIDS)
  • maternal/family history of renal, gastrointestional disease, and cystic fibrosis
  • an abnormal AFP (alpha fetoprotein) blood test (including the suspicion of a neural tube defect)
  • an abnormal antenatal serum screening (triple screen/quad scren) with an increased risk/suspicion of a chromosomal abnormality such as Trisomy 21 [Down syndrome], Trisomy 13 or Trisomy 18

Teenage girls are not only less ready than older women EMOTIONALLY to have a baby, they are less ready PHYSICALLY to have a health baby. Pregnant teens are at much higher risk of having serious medical complications such as toxemia pregnancy-induced hypertension , significant anemia , premature delivery, and/or placenta previa . The maternal death rate for mothers age 15 or younger is 60% greater than that of women in their 20s. Infants born to teens are 2 to 6 times more likely to have low birth weight than those born to mothers age 20 or older.

If you have diabetes, with some advance planning you can generally have a successful pregnancy and a healthy baby. The most important step you can take is to keep your blood glucose tightly controlled and your A1c on target for several months before the baby is even conceived
Risks to the Fetus
Low blood glucose generally does not cause health problems for your baby. However, even an occasional episode of high blood glucose can affect your baby. The first trimester is a critical time for your developing baby's health.
During the first three months of pregnancy, there is a higher risk of miscarriage. If the mother has high blood glucose, the fetus may develop breathing problems at birth or deformities of the spine, skeleton, kidneys, heart and circulatory system. In months four through nine, the major risks are an overly large baby (called Macrosomia) or a stillbirth. About one third of babies born to mothers with diabetes have Macrosomia. Aside from their large size, these babies are otherwise generally healthy.

Homocysteine May Lead to Pregnancy Complications and Birth Defects
High levels of homocysteine in pregnant women may be a marker of risk for preeclampsia and umbilical placental vascular disease, according to a new study.
  • Researchers studied plasma homocysteine levels in the mother and fetus during the third trimester of 26 normal pregnancies and 60 pregnancies with evidence of preeclampsia and placental vascular disease. Maternal plasma homocysteine levels were significantly higher than normals in women with either of these problems. Fetal homocysteine levels were also higher, particularly for the preeclampsia group. Infants in the affected groups were born earlier than those in the normal group, and they weighed significantly less at birth.
