Polymyositis is a type of inflammatory myopathy , related to Dermatomyositis and Inclusion body myositis . Polymyositis means 'many muscle inflammation'. Polymyositis tends to become evident in adulthood, presenting with bilateral proximal muscle weakness, often noted in the upper legs due to early fatigue while walking. The weakness is generally progressive, accompanied by lymphocytic inflammation (mainly cytotoxic T8 lymphocytes ). The cause is unknown, but seems to be related to autoimmune factors, genetics , and perhaps viruses . In rare cases, the cause is known to be infectious, associated with the pathogens that cause Lyme disease Toxoplasmosis , and others. Polymyositis, like Dermatomyositis , strikes females with greater frequency than males. The skin involvement of Dermatomyositis is absent in Polymyositis. Polymyositis and dermatomyositis (PM-DM) are autoimmune neuromuscular diseases referred to as connective tissue diseases. In the family of myositis diseases, polymyositis and dermatomyositis can occur in all ages and both sexes, although they are more common in women. Also, the childhood disease is differentiated from the adult type. A recent study found that polymyositis affects approximately 120 women per million. How do polymyositis and dermatomyositis develop?
Polymyositis and dermatomyositis are autoimmune diseases. In both conditions the immune system malfunctions and produces antibodies that attack healthy tissue in different parts of the body. Polymyositis and dermatomyositis are classified as connective tissue diseases—disorders featuring abnormalities involving the collagen and elastin. Polymyositis and dermatomyositis are characterized by painful inflammation and degeneration of the muscles. The weakness usually occurs in the shoulders and hips but can affect muscles symmetrically throughout the body. A skin rash on the face and upper chest area is present in dermatomyositis, distinguishing it from polymyositis. Researchers are linking cancer as a possible trigger to the disease, primarily in men.

Dermatomyositis and polymyositis
Dermatomyositis and polymyositis are a mixture of various inflammatory diseases of the muscles with or without inflammation of skin too. Both polymyositis and dermatomyositis have an autoimmune basis (and respond to immunosuppression), but the basic mechanisms for the conditions are different and they have specific clinical and histological features. They may be associated with other autoimmune diseases such as myasthenia gravis Hashimoto's thyroiditis systemic sclerosis and Waldenstrom's Macroglobulinaemia. Damage to capillaries would seem to be an underlying pathology in dermatomyositis. It is possible to divide the conditions into 5 basic subgroups according to aetiology:
  • Primary idiopathic polymyositis in adults
  • Idiopathic dermatomyositis in adults
  • Childhood dermatomyositis or myositis with necrotizing vasculitis
  • Polymyositis associated with connective tissue diseases
  • Polymyositis or dermatomyositis associated with malignancy
Epidemiology: The disease occurs in between 5 and 10 people of every 100,000. Juvenile dermatomyositis has an incidence of 2 or 3 per million per year.

They are a group of conditions in which there is inflammation of the muscles. Resulting disability is very variable: sometimes weakness is mild but at the other extreme there may be major disability. Generally speaking, polymyositis and dermatomyositis respond to treatment.

Polymyositis is the inflammation of several muscle groups. The muscles in the shoulder area and pelvis are the most often affected. In this disease, patients usually have the following symptoms:
* weakness in the hips and shoulders,
* swelling and redness in the joints of the hands and ankles,
* chronic shortness of breath, even at rest, and
* problems swallowing. If you have the above symptoms, but also have a rash, you may have dermatomyositis. Dermatomyositis is basically the same disease, but also with a red rash that can be anywhere on your body. Patients who get this disease are usually young women, between the ages of 23 and 60. Occasionally, children, ages 5 to 15 years of age are diagnosed with polymyositis. Healthcare providers may suspect this disease if you report these symptoms and are in the right age group. Your provider may do the following tests to see if you have this disease:
