Alternate Names : Polycystic Ovarian Syndrome (PCOS), Polycystic Ovaries, Sclerocystic Ovarian Disease Definition Stein-Leventhal syndrome is an accumulation of many incompletely developed follicles in the ovaries. This condition is characterized by irregular menstrual cycles, scanty or absent menses , multiple small cysts on the ovaries (polycystic ovaries), mild hirsutism (excessive hair), and infertility . Many women who have this condition also have diabetes with insulin resistance.

A disturbing array of symptoms indicate PCOS, including acne, obesity and excess hair Not a lot is known about the causes of polycystic ovarian syndrome (PCOS), but its symptoms are fairly telltale and include obesity, hirsutism (the medical name for excess face or body hair) acne and infertility. It's estimated that PCOS affects over five per cent of women in the UK and it has recently been associated with long-term risks of diabetes, heart disease and endometrial cancer. As PCOS's name suggests, the syndrome is often (but not always) accompanied by ovaries enlarged with advertisement
multiple small cysts. During the normal ovulation process, a hormone called follicle stimulating hormone (FSH) causes the release of an egg from an ovarian follicle, which then bursts from the follicle in the middle of the woman's menstrual cycle in response to another hormone, luteinising hormone (LH). It is thought that in polycystic ovarian syndrome, male hormones or androgens somehow interfere with the levels of FSH and LH. This causes lots of follicles to form and no egg to be released from these follicles - whose appearance via an ultrasound is sometimes likened to a string of pearls - which forms the cysts observed in PCOS.

General Discussion Polycystic ovary syndrome (PCOS) affects women and is a complex of symptoms that are not necessarily all present in all cases. Some, but not all, affected women have multiple cysts on the ovaries (polycystic ovaries). Other characteristics include the absence of menstruation (amenorrhea) or irregular menstruation, failure of the ovary to release eggs (anovulation), elevated levels of the male hormones known as androgens (hyperandrogenism), excessive amounts of body hair (hirsutism), a high rate of miscarriage, and infertility. Three criteria often used for a diagnosis are menstrual irregularity, hyperandrogenism, and exclusion of other disease. There is some evidence that PCOS is an inherited condition.

Polycystic ovarian syndrome (PCOS) is one of the main causes of infertility in women. Polycystic ovary syndrome
Polycystic ovary syndrome (PCOS) is characterised by menstrual irregularities/abnormalities, hirsutism (male-like hairiness in women) and obesity, resulting from abnormalities in the metabolism of androgens and oestrogen and in the control of androgen production. It was once thought to be mainly due to an ovarian defect as it was often associated with enlarged polycystic ovaries, hence its name polycystic ovary syndrome. However, it is now known that PCOS may occur in women with normal sized ovaries and polycystic ovarian changes can be found in women with normal menstrual cycles.
Who gets it and what is the cause?
PCOS occurs in women only and generally starts at around puberty. It is estimated to affect 3-10% of women. Although the signs and symptoms are similar in most patients, different races can be affected differently. For example, in the US, 70% of patients have hirsutism compared to 10-20% in Japan. The exact cause of PCOS is not yet known but is thought to develop when the ovaries overproduce androgen hormones (e.g. testosterone). This is caused by the overproduction of luteinizing hormone (LH), which is produced by the pituitary gland. People with PCOS also have an inability to process insulin effectively. This is known as peripheral insulin resistance. It is more of a problem in obese patients. Elevated insulin levels also have an effect on ovarian function.

General Information
Some sections may seem very technical as this is a complex disorder Polycystic ovarian syndrome (PCOS) is a set of clinical findings and not a disease itself. There are many abnormalities which may collectively produce the findings which characterize PCOS. Originally the syndrome was termed Stein-Leventhal Syndrome after the investigators who published papers on the clinical findings in 1935. With the advent and availability of ultrasound, the term polycystic ovaries became more common. This name refers to the characteristic ultrasound appearance of the ovaries. Due to abnormal follicular development, the ovaries have multiple small follicles usually measuring <5mm dispersed around the periphery of the ovary in a pattern often referred to as a "string of pearls".

Polycystic Ovary Syndrome (PCOS) is a hormone or endocrine syndrome or group of clinical symptoms and was first described by doctors in the 1930s.
In PCOS characteristically there is an excess of androgens (male-like hormones) and lack of regular ovulation (release of an egg from the ovary). It is sometimes also referred to as “hyperandrogen anovulation syndrome” or “Stein Leventhal Syndrome”.
Incidence
PCOS (with clinical symptoms) affects between 5-10% of all women of childbearing age. However around 20-25% of premenopausal women have polycystic ovaries mostly with no symptoms and therefore do not have the “syndrome”.
Cause
The exact cause is unknown but there appears to be a hereditary and a lifestyle component e.g. sisters of PCOS sufferers have up to a 50% chance of having the disorder and 70% of women with PCOS are overweight and not physically active.
The ovary produces excessive androgens (eg testosterone), which may be caused by the body not producing and/or processing the hormone insulin normally.
