POEMS Syndrome and VEGF Takeshi Hayashi Key words:  POEMS syndrome, vascular permeability, VEGF VEGF, which plays a central role in new vessel formation, is also known as vascular permeability factor. Its permeability increasing effect is much stronger than that of histamine, and it is believed to be implicated in edema formation associated with tumor or infarction (1, 2). In experimental studies, blocking this growth factor successfully prevents brain edema formation (3). As Matsui et al described in this article (4), this growth factor is thought to be involved in the pathogenesis of POEMS syndrome (5). See also. Some features of this disorder, such as ascites and pleural effusion, can be attributed to the increased level of VEGF. On the other hand, there are some symptoms which cannot be attributed to this growth factor overproduction; dusky skin with much hair, for example, cannot simply be explained by the increased level of VEGF. Therefore, whether this molecule actually plays an active role remains uncertain; it is possible to consider that another molecule is a real central player and the VEGF increase is merely an epiphenomenon. Ticlopidine decreased serum level of VEGF in this case. If VEGF is actually the central player in the pathogenesis of POEMS syndrome, ticlopidine should improve the clinical findings of this disorder. Strict following up of this patient, as well as administrating this drug to other POEMS patients, may help us understand more precise pathophysiology of this disorder.

In the current health care environment, delivering high quality and high value patient care is paramount. The goal of quality assurance plan established in electrodiagnostic lab is to improve quality service using ongoing monitoring and evaluation strategies.
The Electrodiagnostic laboratory of Bombay Hospital is one of the pioneering centres in India, well equipped with pentium based state of art electromyograph machines. The department has a good infrastructure and a very good reputation to its credit. The continuous quality management is thus required to cater the quality services.
Electrodiagnostic studies performed in EDX lab are the diagnostic tests; however, without adequate history and physical examination the test has very little value. Furthermore, adequate testing of various nerves and muscles is a prerequisite for correct interpretation and diagnosis.
Common diagnoses encountered in a clinical EDX laboratory include Carpal tunnel syndrome, peripheral neuropathy, radiculopathy, plexus lesions and myopathy. Depending upon the types of patients seen in a given laboratory, certain aspects of care could be monitored.

RESUMEN:
ABSTRACT: Being a multisystemic disorder, POEMS syndrome has different clinical features.and comprises multiple organs. The referred acronym: polyneuropathy, organomegaly, endocrinopathy, monoclonal protein M and skin changes, results insufficient when this syndrome is described, because of the constellation of multiple signs and symptoms. Neurologically a chronic demyelinating peripheral neuropathy is the common clinical presentation, which is associated with an osteosclerotic myeloma in 50% of cases. Involvement of central nervous system includes papilledema and intracraneal hypertension, less frequently. We report a patient with POEMS syndrome with 11 years follow-up, who has recently developed intracraneal hypertension features. We discuss clinical findings and review literature.

Summary
POEMS syndrome A case of POEMS syndrome is presented. The patient suffered from disabling polyneuropathy, organomegaly, endocrine disturbances, an M component and a skin disorder. In addition, the patient had severe anasarca, pulmonary hypertension and lymphadenopathy. Bone lesions (usually sclerotic myelomas), which are common in POEMS syndrome, were not present.
