Department of Pathology, 'La Princesa' University Hospital, Madrid, Spain. J Cutan Pathol. 2005 Jul;32(6):449-452. Abstract quote Background: The term 'glomeruloid haemangioma' was coined by Chan et al. for a histologically distinctive cutaneous haemangioma, which they considered a specific cutaneous marker for POEMS syndrome. Glomeruloid haemangiomas appear to be specific to POEMS syndrome, because they have not been reported in patients without this syndrome. Methods: We report on an 86-year-old man without POEMS syndrome and with a solitary red papule on the face.

General Discussion POEMS syndrome is an extremely rare multisystem disorder. POEMS is an acronym that stands for (P)olyneuropathy, disease affecting many nerves; (O)rganomegaly, abnormal enlargement of an organ; (E)ndocrinoapthy, disease affecting certain hormone-producing glands that help to regulate the rate of growth, sexual development, and certain metabolic functions (endocrine system); (M)onoclonal gammopathy or M proteins; and (S)kin defects. Common symptoms include progressive weakness of the nerves in the arms and legs, an abnormally enlarged liver and/or spleen (hepatosplenomegaly), abnormally darkening of the skin (hyperpigmentation) and excessive hair growth (hypertrichosis). Endocrine abnormalities such as failure of the ovaries and testes (gonads) to function properly (primary gonadal failure) and diabetes mellitus type I may be present. Specific endocrine abnormalities associated with POEMS syndrome vary from case to case.

POEMS Syndrome Diagnosis
POEMS syndrome, a rare multi systemic disease, is more prevalent among men than women. POEMS syndrome is considered rare but note that the presence of this syndrome could go unrecognized in many cases. POEMS syndrome usually affects people in their fifth or sixth decades. It is reported that the median survival period for patients with POEMS syndrome is 8 years. Causes of POEMS Syndrome Plasma cell disorder is considered to be the main cause of POEMS syndrome. It is still unclear as to why plasma cells in some dyscrasias produce factors that cause the POEMS syndrome. This syndrome is associated with MGUS, osteosclerotic myeloma and Waldenstrom macroglobulinemia. Symptoms of POEMS Syndrome The POEMS syndrome is the acronym for the most common symptoms of the syndrome: P: Peripheral neuropathy, which includes tingling, numbness and the weakening of hands and feet.
O: Organomegaly that includes large organs like the lymph nodes, liver and spleen.
E: Endocrinopathy that causes abnormality in hormone levels.

Characteristic feature is a chronic inflammatory demyelinating neuropathy causing mainly motor disability. Hepatomegaly found in approximately half of patients. Hyperpigmentation and hypertrichosis are common. May be angiomatous lesions of trunk. Gynaecomastia and atrophic testes. Investigations Management If osteosclerotic lesions in limited area - radiotherapy
If osteosclerotic lesions widespread - chemotherapy or autologous stem cell transplantation. References Used

This syndrome is of unknown cause and takes its name from the typical features: polyneuropathy, organomegaly, endocrinopathy, M proteins and skin changes. Numerous other characteristics can be noted. However, patients do not have an increased prevalence of amyloidosis and rarely reveal Bence Jones proteinuria. Bone proliferation occurs at entheses (sites of tendon and ligament attachment to bone). Particularly characteristic are irregular bony excrescences on the posterior elements of the spine and about the sacroiliac and costovertebral joints ( Fig.1
