WHO issues a global alert about cases of atypical pneumonia Cases Of Severe Respiratory Illness May Spread To Hospital Staff Since mid February, WHO has been actively working to confirm reports of outbreaks of a severe form of pneumonia in Viet Nam, Hong Kong Special Administrative Region (SAR), China, and Guangdong province in China. In Viet Nam the outbreak began with a single initial case who was hospitalized for treatment of severe, acute respiratory syndrome of unknown origin. He felt unwell during his journey and fell ill shortly after arrival in Hanoi from Shanghai and Hong Kong SAR, China. Following his admission to the hospital, approximately 20 hospital staff became sick with similar symptoms. The signs and symptoms of the disease in Hanoi include initial flu-like illness (rapid onset of high fever followed by muscle aches, headache and sore throat). These are the most common symptoms. Early laboratory findings may include thrombocytopenia (low platelet count) and leucopenia (low white blood cell count). In some, but not all cases, this is followed by bilateral pneumonia, in some cases progressing to acute respiratory distress requiring assisted breathing on a respirator. Some patients are recovering but some patients remain critically ill. Today, the Department of Health Hong Kong SAR has reported on an outbreak of respiratory illness in one of its public hospitals. As of midnight 11 March, 50 health care workers had been screened and 23 of them were found to have febrile illness. They were admitted to the hospital for observation as a precautionary measure. In this group, eight have developed early chest x-ray signs of pneumonia. Their conditions are stable. Three other health care workers self-presented to hospitals with febrile illness and two of them have chest x-ray signs of pneumonia.

The above comments from an RFI report in ProMed are causes for concern. Although the patients are said to be suffering from bacterial pneumonia, the creation of an isolation ward for a cluster of pneumonia cases near the bird flu outbreak in geese in Tacheng Xinjiang is curious. Fever clinics were also set up in the vicinity of Qinghai Lake after an unprecedented die-off of migratory birds infected with H5N1.
Abundant news had reported 121 human deaths linked to the Qinghai Lake outbreak. Although reports of human case have been denied, the 121 deaths and 79 additional infections were never specifically addressed. Those reports, if true, would have signaled phase 6 of the 2005 flu pandemic and further spread such as the pneumonia cluster would have been expected.
WHO has requested an onsite visit to Qinghai Lake to observe how that outbreak was handled. Clearly an expanded trip to include sites in Xinjiang as well as sites in India, where the bar headed geese

Pneumonia Prevention: It's Worth a Shot Pneumococcal (pronounced new-mo-KOK-al) disease is an infection caused by bacteria. These bacteria can attack different parts of the body. When they invade the lungs, they cause the most common kind of bacterial pneumonia. When the same bacteria enter the blood, they cause an infection called bacteremia (bak-ter-E-me-ah). In the brain, they cause meningitis. Pneumococcal pneumonia is a serious illness that kills thousands of older people in the United States each year. Can Pneumonia Be Prevented? For some causes of pneumonia, yes. The pneumococcal vaccine is safe, it works, and one shot lasts most people up to 10 years. People who get the vaccine are protected against almost all of the bacteria that cause pneumococcal pneumonia and other pneumococcal diseases as well. The shot, which is covered by Medicare, can be a lifesaver.
Some experts say it may be best to get the shot before age 65anytime after age 50since the younger you are, the better the results. They also say people should have this shot even if they have had pneumonia before. There are many different kinds of pneumonia, and having one kind does not protect against the others. The vaccine, however, does protect against 88 percent of the pneumococcal bacteria that cause pneumonia. It does not guarantee that you will never get pneumonia. It does not protect against viral pneumonia. Most people need to get the shot only once. However some older people may need a booster; check with your doctor to find out if this is necessary.

A Toronto nurse takes precautions The deadly pneumonia, which is spreading across the world, could be more contagious than previously thought, an expert has warned. Hong Kong, which has been hit hard, has established quarantine camps in an effort to try to contain the spread of the disease. And at Singapore airport, nurses monitored passengers for signs of the illness. Seven were sent to hospital because they showed symptoms of the pneumonia. The illness, dubbed Severe Acute Respiratory Syndrome (SARS), has so far killed 62 people worldwide, and infected 1,800. Initially it was thought that the illness could only be spread by close contact. But Dr Hitoshi Oshitani, a World Health Organization (WHO) expert, told the BBC that this was not necessarily the case. He said: "In most cases infection occurs by close person-to-person contact, but there are several cases now that we cannot explain by this model of transmission." He stressed that SARS was more contagious than the deadly Ebola virus.

B efore the epidemic of acquired immunodeficiency syndrome (AIDS) in the early 1980s, Pneumocystis carinii pneumonia (PCP) was a rare infection that occurred in immunosuppressed patients with protein malnutrition or acute lymphocytic leukemia, or in patients receiving corticosteroid therapy. This opportunistic infection is now most commonly associated with advanced human immunodeficiency virus (HIV) infection. The drug of choice for both treatment and prophylaxis of PCP is trimethoprim-sulfamethoxazole. The appearance of PCP in previously healthy gay men was one of the initial signs of the emergence of AIDS. Before the widespread use of prophylaxis, PCP was the AIDS-defining illness in 60 percent of cases and eventually affected 80 percent of patients with AIDS.

Anxiety, headache, bronchospasm, cough, and chills are some side effects you may experience from treatment with aerosol pentamidine. IV pentamidine can cause leukopenia, thrombocytopenia, and anemia. You must discontinue ddC until you are stable on pentamidine.

Mycoplasmal Pneumonia
Mycoplasmal pneumonia (in the U.S.) or porcine enzootic pneumonia (England and other countries) is caused by Mycoplasma hyopneumoniae . Another synonym for this disease is swine enzootic pneumonia (SEP). Fifteen years ago this disease was called virus pneumonia of pigs (VPP); since that time it has been discovered that a mycoplasma, not a virus, is the cause. Mycoplasma are very small and readily pass through ordinary bacterial filters. In addition, mycoplasma are difficult to grow in the laboratory. For these reasons, it was originally assumed that mycoplasmal pneumonia was a virus. Mycoplasmal pneumonia affects pigs of all ages, starting with those as young as 7-10 days of age. It is estimated that 90% or more of the swine herds in the Midwestern United States are infected with mycoplasmal pneumonia.

Two different terms may be used to describe pneumonia depending on where it affects the lungs. In lobar pneumonia, the infection or inflammation involves a section (or lobe) of a lung. In bronchial pneumonia, the infection or inflammation is not confined to a single anatomic location.. In either case, the disorder typically causes a cough (with or without mucus), chest pain, rapid breathing, and shortness of breath. Fever, sweats, a headache, and weakness are usually present. The medical history is extremely important in making a diagnosis of pneumonia, and often also in differentiating the types of infections involved. Bacterial pneumonias typically have rather sudden onset of symptoms with a relatively high fever. The cough may be either dry or contain large amounts of sputum. Chest x-rays reveal shadows where the pneumonia is present, and blood counts show elevated white blood counts that are more typical of bacterial infections than either viruses or fungi.

Pneumocystis carinii pneumonia, commonly referred to as PCP, can affect the lungs as well as other parts of the body, including the skin and internal organs. People who are at the greatest risk of contracting this type of pneumonia are those who have less than 200 T4 cells. Symptoms of PCP include shortness of breath, dry cough, and fever. PCP seems to occur very frequently in women as a first or second AIDS-defining illness. Diagnosis in women is often delayed and the illness may be severe by the time it is detected. Studies are ongoing to determine the usefulness and safety of combined treatment with primaquine and clindamycin. Side effects
The combination of primaquine and clindamycin is generally well tolerated. Nausea, vomiting and stomach cramps occasionally occur. Other side effects can be anemia, headache, visual disturbance and intense itching. Dosage
Combination therapy with oral primaquine at 15 or 30 mg daily with clindamycin given intravenously or orally at 1.8-3.6 g in 3 or 4 divided doses daily is effective in treating PCP.

Modern drug researchers look for genetic weaknesses in bugs like S. pneumoniae , and having the whole genetic sequence available should make the job easier. They say they have shown for the first time that alcohol impairs an important defence barrier to lung infections, and that smoking intensifies this. Rats exposed to alcohol and smoke lost movement of cilia - tiny hairs lining the airways that beat rhythmically to waft bugs up and out of the lungs. The authors' paper in Alcoholism said the same was probably true in humans. Defences down Researcher Professor Martha Gentry-Nielsen, of Creighton University School of Medicine, said there were several reasons why alcoholics were more susceptible to an infection called streptococcus pneumoniae, which can cause pneumonia. "They have a decreased gag reflex," she said. This means it is easier for mucus from the nose and fluid from the gut, such as vomit, to go down into the lung, particularly when they lose consciousness, she said. "Host defences within the lungs of alcoholics are also compromised, so they are not well equipped to handle the infection once the organisms reach the lungs."

Contagious Bovine pleuro pneumonia
Contagious Bovine Pleuro-pneumonia (CBPP) is a contagious disease affecting the lungs and pleura of cattle and other closely related ruminants, and is caused by an infective organism - Mycoplasma mycoides.
History and spread of the disease
Evidently the slaughter policy met with success. Pleuro-pneumonia had been eradicated from Great Britain by 1898 and has not subsequently occurred here. Nevertheless, it survives in other parts of the world. It is included in the OIE listings as a Class A disease, making it one of the more economically serious diseases of livestock.
Symptoms
The first signs of the disease often escape notice. The temperature rises and a dry husky cough is present, especially when the animal first gets on its feet or when made to run. In marked cases its breathing is rapid, and the movement of the flanks is increased. If pressure is applied to the ribs, the animal may grunt or show signs of pain. Some of the affected animals may become greatly emaciated and die but others, especially those that have passed the acute stages, may appear to recover.
