Signs and symptoms of anterior lobe pituitary disorder: A slowly growing pituitary adenoma, which is essentially a benign tumor of the anterior pituitary gland, will often first affect the gonadotropin function, followed by growth hormone deficiency, and finally low TSH and low ACTH. Due to the pressure of the adenoma on the optic chiasm that is in very close proximity to the pituitary gland, there is a characteristic visual field loss of the corresponding outside halves of the visual field on each side (black area in this link corresponds with blind fields). The eye specialist calls this a bitemporal hemianopsia.

Dr. Ray's Health Newsletter / table of contents The latest Health Newsletter Pituitary Disorders Introduction: In the introduction to this chapter on hormones I pointed out how important feedback systems are to the regulation of hormone production. Here I am describing pituitary gland disorders. The pituitary gland is one of the central regulating hormone glands that responds to mostly stimulatory hormones from the hypothalamus gland. But it also responds to inhibitory stimuli from the hormone glands in the body. The pituitary gland is intimately related to adrenal gland function (via cortisol hormones), function of the thyroid gland (via T3 and T4 hormones) and to the function of the male hormones and female hormones (testosterone and estrogen). The release of prolactin is inhibited by dopamine (from the hypothalamus). This hormone is only released in a lactating woman and is regulated through the dopamine flow from the hypothalamus. This is diminished after the delivery of a baby leading to release of prolactin from the pituitary gland and lactation of the breasts.

(For those of you who are not Zoe ... feel free to pitch in with your own suggestions since i think we are both pretty much out of ideas right now. We have both posted all over these boards so you might have seen us before. We both have a bunch of severe but nonspecific symptoms that have totally taken over our lives, including GI problems, skin problems, swelling, unexplainable weight changes, dizziness/fainting, weakness, changes in appearance (face shape, skin color), etc.) I have become a research junkie and my current theory is that I have a pituitary disorder, so here are sites I have found helpful.

Pituitary Tumors
Large pituitary tumors that press on the optic nerve are traditionally approached through the skull with standard, open transeptal and transphenoidal surgery. Since the mid-1990's, Upstate neurosurgeons - working side by side with ear, nose and throat surgeons - have approached smaller pituitary tumors through the nose and sinuses, using endoscopes and specially designed instruments that are much less invasive. This less traumatic approach provides excellent visualization of the pituitary gland and surrounding structures, involves less perioperative discomfort and shortens hospital stays.

Endoscopic tools tailored to pituitary surgery have been developed by Gerard Rodziewicz, MD, who published the first paper on debulking pituitary tumors by combining endosopic instrumentation with a transnasal transphenoidal approach. Pituitary and Neuroendocrine Center menu The University of Michigan Pituitary and Neuroendocrine Center is a nationally and internationally recognized resource available to provide a full service multidisciplinary approach to the diagnosis and treatment of pituitary and hypothalamic disorders. Patients will be evaluated at the initial visit by an endocrinologist and a neurosurgeon, both of whom specialize in the treatment of pituitary problems. Often the problem will be completely addressed in one visit, but if further endocrine or imaging studies are needed these will be scheduled and a return visit to the clinic arranged. We also have neuroophthalmologists, neuropathologists, neuroradiologists and radiation oncologists readily available to evaluate a situation on the same day as the initial visit. Treatment options include surgery, conventional and focused radiation and the complete spectrum of medical treatments. The faculty provides an extensive experience in treating pituitary tumors (including prolactinomas, Cushing's disease, acromegaly and non-functioning tumors) as well as craniopharyngiomas, meningiomas and other disorders of the pituitary-hypothalamic region.

The prevalence of acromegaly is approximately 60 cases per million population, and the incidence is 3.3 new cases per million per year. The name "acromegaly" comes from the Greek words for "extremities" (acro) and "great" (megaly), because one of the most common symptoms of this condition is abnormal growth of the hands and feet. One out of five adults worldwide may have a non-cancerous tumor of their pituitary gland, and at least one-third of these pituitary tumors may be clinically active and causing significant health problems, according to results of a new study presented today at the 10th Annual Meeting and Clinical Congress of the American Association of Clinical Endocrinologists (AACE). The findings are based on an analysis of 12 published studies on the prevalence of pituitary tumors, conducted by clinicians experienced in the management of pituitary disorders.

Pituitary Apoplexy PITUITARY TUMOR PROGRAM PITUITARY DIAGNOSES INDEX Pituitary Apoplexy is a condition that develops over hours to several days, typically resulting from hemorrhage and/or infarction of a pituitary macroadenoma. Symptoms often include headache, nausea, visual loss, double vision and altered mental status. Most patients also have undiagnosed hormone insufficiency prior to the apoplectic event. In patients with such symptoms, the diagnosis of pituitary apoplexy is best confirmed with an MRI of the brain with special attention to the pituitary. A CT scan of the pituitary gland will also show an abnormality in the majority of cases. Other conditions to consider when trying to establish the diagnosis of pituitary apoplexy include ruptured intracranial aneurysm, meningitis, brain stem stroke, cavernous sinus thrombosis, intracerebral hemorrhage, temporal arteritis and ophthalmoplegic migraine headache. (Left)
MRI of patient with two days of worsening headaches and loss of peripheral vision and new onset of double vision. The enlarged size of the pituitary and the bright signal in the pituitary seen on MRI are highly suggestive of pituitary apoplexy. Pathophysiology Pituitary apoplexy is thought to occur when a tumor spontaneously hemorrhages or when it outgrows its blood supply. This loss of blood supply can result in tumor cell death, bleeding and acute tumor swelling. The relatively sudden change in tumor volume causes compression of the normal pituitary gland, the optic nerves and the nerves coursing through the cavernous sinus that control eye movements. If bleeding extends into the cerebrospinal fluid spaces, severe headache, neck stiffness and fever may occur.

The pituitary gland is no larger than a pea, located at the base of the brain. The gland is attached to the hypothalamus (a part of the brain that affects the pituitary gland) by nerve fibers. The pituitary gland itself consists of three sections, which each produce certain hormones:
  • Anterior (front) lobe - produces growth hormone, prolactin (to stimulate milk production after giving birth), ACTH (adrenocorticotropic hormone), TSH (thyroid-stimulating hormone), FSH (follicle-stimulating hormone), and LH (luteinizing hormone). Intermediate lobe - produces melanocyte-stimulating hormone (controls skin pigmentation). Posterior (back) lobe - produces ADH (antidiuretic hormone) and oxytocin (to contract the uterus during childbirth and stimulate milk production).
