Using a modified nasopharyngeal airway in Pierre Robin syndrome
E DITOR E DITOR Masters et al describe how a modified endotracheal tube can be used as a nasopharyngeal airway in infants with Pierre Robin syndrome. We describe further modifications made to overcome some problems which we encountered in using the technique in one of our patients. In the original description the protruding part of the airway is cut into four strands and the upper one cut off. We found the remaining strands rather thick, so cut them to half the width.

Division of Ophthalmology, Duke University Medical Center, Durham, North Carolina. The Pierre Robin syndrome is characterized by three defects: micrognathia, cleft palate and glossoptosis. It is a distinct clinical entity possessing a history that is almost monotonous in regularity, and with physical findings so striking as to not easily be overlooked. As early as 1911, Shukowsky observed that the physical complex of micrognathia, cleft palate, and glossoptosis constituted a cause for dyspnea and cyanosis a newborn infant and a serious threat to life. Due to the further interest and writings on this subject by Pierre Robin in 1923, the syndrome received much attention and its name. Infants must be kept prone (face down), which allows gravity to pull the tongue forward and keep the airway open. These problems abate over the first few years as the lower jaw grows and assumes a more normal size. In moderate cases, the patient requires placement of a nasopharyngeal airway (a tube placed through the nose and into the airway) to avoid airway blockage. In severe cases, surgery is indicated for recurrent upper airway obstruction. A tracheostomy is sometimes required. Feeding must be done very carefully to avoid choking and aspiration of liquids into the airways.

Outlook (Prognosis)
Choking and feeding problems may go away spontaneously as the jaw grows. There is a significant risk of problems if the airway is not protected against obstruction.

This condition is often apparent at birth. Call if choking episodes or breathing problems increase in frequency. Airway blockage may be indicated by a high-pitched, crowing noise when inhaling (
