Obesity hypoventilation syndrome (OHS)
Definition: Obesity hypoventilation syndrome (OHS) is a condition related to (but can occur separately from) obstructive sleep apnea . In OHS, a very obese person does not breathe a sufficient amount of oxygen during sleep or while awake.

Causes, incidence, and risk factors: The cause of OHS in unknown, but it likely involves a combination of a disorder in the brain's control over breathing and the effects of obesity on the chest wall. With the excess weight of massive obesity, the muscles of the chest wall can have difficulty expanding the chest enough to exchange air efficiently. This results in a decreased ability to oxygenate the blood, and the retention of carbon dioxide (see respiratory acidosis ). Pickwickian syndrome is a disorder that was named after Joe, the fat, red faced boy in Charles Dickens' The Pickwick Papers. This syndrome is a combination of obstructive apnea in moderately to severely obese people, most of whom attempt to sleep face up.
Extreme obesity along with shallow breathing, sleep apnea, excessive sleepiness, and heart failure.

Background: Sleep apnea occurs during sleep when a cessation of airflow occurs for at least 10 seconds (usually 20-30 s but rarely >2 min). The apneas (absences of breath) are accompanied by snoring, sleep arousals, and hypoxia. The term sleep apnea describes 2 major sleep-related clinical problems: (1) obstructive sleep apnea (OSA) and (2) central sleep apnea. OSA is caused by upper airway (UA) obstruction at the level of the pharynx and is the most common form of sleep apnea. Central apnea is the result of impairment in respiratory control of breathing. Sleep apnea syndrome encompasses a spectrum of sleep-related breathing disorders ranging from simple snoring to severe sleep apnea with associated neurocognitive deficits, residual daytime sleepiness, pauses in breathing, and cardiovascular consequences such as hypertension and stroke. UA resistance syndrome is a condition within this spectrum that is intermediate in severity to these 2 extremes. A patient with chronic heart failure and chronic respiratory failure (CRF) underwent ambulatory polysomnography at home. She was found dead on the morning after the recording. The tracings confirmed severe sleep apnea syndrome. After 8 h of incessant Cheyne-Stokes respiration during sleep, respiratory arrest occurred, followed 7 min later by asystole. This report illustrates a case of respiratory drive failure during sleep as the mode of death in a patient with heart failure, sleep apnea syndrome, and CRF.
