(Redirected from Vegetative state A persistent vegetative state (PVS) , also known as cortical death , is a condition of patients with severe brain damage in whom coma has progressed to a state of wakefulness without detectable awareness . The term was coined in by Scottish neurosurgeon Bryan Jennett and American neurologist Fred Plum to describe a syndrome that seemed to have been made possible by medicine 's increased capacities to keep patients' bodies alive. A persistent vegetative state is not the same as coma Patients in a persistent vegetative state are usually considered to be unconscious and unaware. They may experience sleep -wake cycles, or be in a state of chronic wakefulness. They may exhibit some behaviors that can be construed as arising from partial consciousness, such as grinding their teeth, swallowing, smiling, shedding tears, grunting, moaning, or screaming without any apparent external stimulus . They are unresponsive to external stimuli, except, possibly, pain stimuli. Few people have been reported to recover from PVS. Some authorities hold that PVS is, in fact, irreversible, and that the reportedly recovered patients were not suffering from true PVS. Nonetheless, some dispute still remains over the reliability of PVS diagnosis, particularly when a limited number of physicians (or physicians without experience in the area of PVS) make the diagnosis. Bishop Moore coach Rob Graham, 33, collapsed of a heart attack in February during a championship game. When a police officer tried to resuscitate him with a portable defibrillator, the device failed and it took another 20 minutes for an ambulance to arrive. Doctors say they had slim hopes he'd survive. But, on Good Friday, he suddenly showed signs of recovery. "Nurses were coming from all over, and they started getting him to follow thumbs up and sticking his tongue out and then he turned to the picture of his girls and smiled, and I'm crying with him because I can't believe it.     Preamble Present requirements of health reporting fails to provide an accurate estimate of the incidence and prevalence of worldwide individuals in a persistent vegetative state (PVS). Ten years ago, a prevalence of 2 to 3 per 100,000 was estimated for Japan. It seems likely that the absolute number of such cases has risen appreciably as a consequence of current practices in critical medicine, cardiorespiratory support, parenteral feeding, and control of infections in severely brain damaged patients. How to deal with this emotionally painful, financially costly, and generally unwanted outcome of modern medical treatment is an increasing problem.

    Once qualified clinicians have determined that a person is awake but unaware, the permanence of the vegetative state depends on the nature of the brain injury, the duration of the period of unawareness, and the estimated prognosis. Some persons less than 35 years old with coma after head trauma, as well as an occasional patient with coma after intracranial hemorrhage, may recover very slowly; thus, what appears to be a PVS at one to three months after an event causing coma may in rare cases evolve into a lesser degree of impairment by six months. On the other hand, the chances of regaining independence after being vegetative for three months are vanishingly small. Rare exceptions are claimed, but some of these may have represented patients who entered an unrecognized locked-in state shortly after reawakening from a coma-causing injury. Ultimately, all have been severely disabled.

    Persistent Vegetative State Stories
    Orlando Sentinel is just the latest Florida paper to tell the stories of PVS, a condition that with the Schiavo case has become a lightning rod for Florida politics. Everyone has a strong opinion - and notions of compassion riddle the stories no matter which side is telling. The PVS patient is very much alive in the Florida narratives. Alive both in the eyes of parents and loved ones and in a new kind of story - the story of what it "is like to be in PVS." It seems a little bit odd to me, almost macabre. This story focuses in particular on how the lives of PVS patients are animated by the incredible hopes of their families, who hope against all factual evidence that their children will snap out of a condition from which there is literally no chance of recovery: MIAMI GARDENS, Fla. - (KRT) - Today is a lavender day. According to the National Institute for Neurological Disorders and Stroke people in PVS "have lost their thinking abilities and awareness of their surroundings, but retain non-cognitive function and normal sleep patterns. Even though those in a persistent vegetative state lose their higher brain functions, other key functions such as breathing and circulation remain relatively intact. Spontaneous movements may occur, and the eyes may open in response to external stimuli. They may even occasionally grimace, cry, or laugh. Although individuals in a persistent vegetative state may appear somewhat normal, they do not speak and they are unable to respond to commands." People suffering from PVS can generally be distinguished from afflicted but cognitively intact patients who suffer from " locked-in syndrome " by the fact that "locked in" patients can track visual stimuli and use eye blinks for communication.

      Schiavo Drama: Florida Justice in a Persistent Vegetative State
      Business and family affairs converge this weekend near Tampa, Florida, where I’m listening and watching local and national coverage of the Terri Schiavo case. Here are some impressions and thoughts: Pinellas County Circuit Judge George W. Greer has chosen to disregard a Congressional subpoena for Terri Schiavo and ordered the feeding tube removed. What power, Congress? Greer has decided that the Congressional motion is trumped by his own court. How is this not contempt of Congress? Couldn’t federal marshals enforce the order that there be no tampering with a federal witness? Or perhaps the marshals could apprehend the state judge. Why are the liberals and Democrats against allowing this horribly damaged life being allowed to continue? How does the culture of death align with their political agenda? Activists have gathered at the hospice where Terri lives to express their views as the vigil becomes a death watch, and the legal and legislative wrangling becomes more desperate.

The target of the Judge George Greer George Felos , and the Woodside Hospice crowd in Pinellas County, Florida is someone who is not dying " quickly enough ," those whom ethicist Daniel Callahan unkindly termed "biologically tenacious." I am biologically tenacious, aren't you? Knowing what I know now, I am extremely grateful that I have never lived under Judge George Greer's jurisdiction. I have chronic progressive multiple sclerosis, have an electric wheelchair, and have been through acute rehabilitation at the University of Utah Medical Center four times . In two of those extended stays in the hospital I was given the beautiful opportunity to learn how to swallow, speak, eat, and to continue with my life. I was treated by experts, a multidisciplinary medical team that had the experience to evaluate and rehabilitate me. All of these opportunities have been denied Terri Schiavo. In previous orders by Judge Greer to remove Terri's feeding tube he based the orders on the testimonies of doctors who say Terri is in a persistent vegetative state(none of which were qualified medical rehabilitation experts). But doctors employed by the Schindlers to assess her condition conclude that with therapy, she could learn to eat and drink on her own and perhaps learn to talk.

    Permanent or Persistent Vegetative State (PVS) The first important thing to acknowledge about a diagnosis of "permanent or persistent vegetative state (PVS)," is that it is a harsh term that seems to imply what is not, or should not, be intended. That is, it does not mean that the patient has ceased to be a human person with the dignity of every other human person. This human dignity cannot be diminished by disease or disability. Nevertheless, PVS is a profoundly serious condition. Though there are many questions about the continuation of medical treatment for these patients (because of the gravity of their circumstances) basic care should continue. The term "permanent vegetative state" was coined by neurosurgeons Bryan Jennett and Fred Plum in 1972 to describe a condition resulting from severe brain damage. Using their words, it is described as coma characterized by "wakefulness without awareness." In 1983, the President's Commission for the Study of Ethical Problems in Medicine and Biomedical and Behavioral Research accepted the definition of persistent vegetative state as the "inability to experience the environment." It is one type of permanent unconsciousness.
