Treatment decisions for patients in persistent vegetative state

Defining pvs

The persistent vegetative state (pvs) presents particular medical, ethical and legal dilemmas because of the extreme nature of the condition, the difficulties associated with diagnosing it accurately and the risks of premature diagnosis. It results from severe damage to the cerebral cortex, resulting in irreparable destruction of tissue in the thinking, feeling part of the brain. Patients appear awake but show no psychologically meaningful responses to stimuli. It is widely accepted that pvs patients are unconscious and incapable of suffering mental distress or physical pain although many reflex responses remain. Research studies have shown that the level of metabolic functioning of the cerebral cortex of pvs patients is the level associated with deep surgical anaesthesia. [ Go to reference 1

A persistent vegetative state (or PVS) is a condition of patients with severe brain damage in whom coma has progressed to a state of "wakefulness without awareness". The term was introduced by two doctors in 1972 to describe a syndrome that seemed to have been made possible by medicine 's increased capacities to keep patients' bodies alive. A persistent vegetative state is not the same as coma , the major distinction being that coma sufferers cannot breathe on their own. Patients in a persistent vegetative state are usually considered to be unconscious and unaware, but exhibit sleep -wake cycles and some behaviors that can be construed as arising from partial consciousness, such as grinding their teeth, swallowing, smiling, shedding tears, grunting, moaning, or screaming without any apparent external stimulus . Their heads and eyes can track moving objects or turn towards a sound Few people ever recover from PVS, but in occasional cases, family members who visit the patient will detect evidence of awareness when doctors with limited patient contact will deny it. Eye tracking is often the earliest symptom of recovery.

The reason why these patients experience periods of wakeful unawareness is readily explainable by the underlying characteristic neuropathological changes present in most patients. Whatever the primary cause of the vegetative state, such as hypoxic-ischemic encephalopathy secondary to a cardiac or pulmonary arrest, or shearing injuries in brain trauma, there is extensive damage to the higher centers of the brain, with relative preservation of the brain stem structures. Hence, the cognitive functions of the cerebral cortex are absent, while the arousal system (the reticular activating system) in the brain stem is intact. Making the PVS Diagnosis

None of the guidelines in the literature precisely defines these terms, but when the patients do develop sustained visual pursuit, it is usually readily apparent to anyone seeing the patient, families and health care professionals alike, and it is so consistent and reproducible that it is present almost 100 percent of the time during the periods of wakefulness.     Coma
    Coma is not a diagnosis but rather a description of a dramatic symptom of profound brain injury. Using symptoms in place of a proper diagnosis often leads to the attempt to treat those symptoms. This does not work. To be successful one must treat the brain, where the injury actually exists. A proper diagnosis describes where the injury exists in the brain, the degree of the injury to the brain, and the extent of the injury to the brain. The dictionary defines coma as "a state of unconsciousness from which the patient cannot be aroused." The Institutes defines coma as "a state of unconsciousness from which the patient has not yet been aroused ." Coma due to brain injury is not an isolated phenomenon, but is instead an integral part of the unbroken continuum that extends from death and profound coma at one end of the spectrum to very mild brain injury and normality at the other.

Persistent Vegetative States can and should be use to advance many causes
This Craigslist poster has the right idea: donate your persistent vegetative state in advance to be cynically manipulated for the cause of your choice. If I am rendered comatose and determined to be in a persistent vegetative state (PVS) for a period longer than one month and if no imminent cure is forthcoming, I do not wish to be kept alive by artificial means including but not limited to nourishment, hydration, etc. However.... If, due to the absurd political state of affairs in this country, my persistent vegetative state and impending unplugging can be parlayed into some sort of political leverage, I wholly endorse using my predicament in whatever way possible for the purposes of passing legislation favorable to my general political and ethical outlook. Here is a list of top-tier causes I support and will continue to support, both while in my PVS and after my eventual death. * Debt Relief to Impoverished Nations: I will agree to stay in a PVS for an indeterminate amount of time if the United States aggressively pursues a policy of debt relief and debt forgiveness to developing and impoverished nations.

Fox News is in a Persistent Vegetative State
The only topic covered today (April 1, 2005) on Your World w/Neil Cavuto was the impending death of Pope John Paul II. Linda Vester, the host of Dayside w/Linda Vester was the substitute host. Comment: It's incredible how much news has been unreported on Fox and the other cable "news" networks during the last several weeks. Now that poor Terri Schiavo has died and we've moved off wall-to-wall coverage of that, it looks like we're sliding directly into another long period of wall-to-wall coverage, this time of the ceremonies surrounding the funeral of Pope John Paul II. So much is happening in the world that the average "news" viewer knows nothing about. Whatever happened to the resolve the networks showed after 9/11 to devote more time and attention to world affairs? I have no doubt that if and when there's another terrorist attack, we'll again hear that naive cry: "Why do they hate us"? At any rate, in case anyone's interested, here's the economic news that Your World watchers didn't get today (I'm sure the Bush administration is thrilled).

Misdiagnosing the persistent vegetative state An apparently high rate of misdiagnosis demands critical review and action Diagnosing the persistent vegetative state is important and sometimes difficult. Patients who retain some degree of awareness must be recognised if their quality of life is to be maximised and inappropriate withdrawal of tube feeding prevented. On p 13 Keith Andrews and colleagues present a retrospective study of the clinical records of 40 patients admitted over three years to a single unit specialising in the rehabilitation of patients with profound brain damage. The authors conclude that 17 of these patients were misdiagnosed. Patients in the persistent vegetative state seem to be awake with their eyes open but show no evidence of awareness, but the 17 misdiagnosed patients were able to communicate consistently using eye pointing or a touch sensitive buzzer. Such an apparently high rate of misdiagnosis raises important concerns about the accuracy of the diagnosis of the vegetative state and related syndromes of severe brain damage.

Terry Schiavo Awakens From Persistent Vegetative State, No Explanation
*****EXCLUSIVE*****MUST CITE JAWA REPORT**** Nearly 10 years after a brain injury left a firefighter virtually mute, he suddenly started talking to his wife and sons last weekend. A couple of years ago, it happened with a severely injured car accident victim who'd spent 19 years in silence. And before that, a paralyzed policeman whose brain had been damaged in a shooting suddenly regained his speech after eight years. And now, Terri Schiavo has suddently sprung back to vibrancy. Experts had claimed that the probability of Schiavo's recovery from a constant vegetative state were about as low as her chances of recovering from her state induced death.
Normally, brain-injured patients who get better do so within the first five years, especially in the first two years, and usually the change is gradual. So what's the explanation for these reports of long-delayed, sudden improvement?
