    Top Is there any treatment? Therapy for peripheral neuropathy differs depending on the cause. For example, therapy for peripheral neuropathy caused by diabetes involves control of the diabetes. In cases where a tumor or ruptured disc is the cause, therapy may involve surgery to remove the tumor or to repair the ruptured disc. In entrapment or compression neuropathy treatment may consist of splinting or surgical decompression of the ulnar or median nerves. Peroneal and radial compression neuropathies may require avoidance of pressure. Physical therapy and/or splints may be useful in preventing contractures (a condition in which shortened muscles around joints cause abnormal and sometimes painful positioning of the joints).     Recovery from peripheral neuropathy is usually slow. Depending on the type of peripheral neuropathy, the patient may fully recover without residual effects or may partially recover and have sensory, motor, and vasomotor (blood vessel) deficits. Peripheral Neuropathy is a generic phrase denoting functional disturbances and/or pathological changes in the peripheral nervous system. If the involvement is in one nerve it is commonly referred to as mononeuropathy , in several nerves, mononeuritis multiplex and if diffuse and bilateral, polyneuropathy The treatment of Peripheral Neuropathy is dependent on diagnosing the underlying cause; therefore early recognition and intervention is paramount. If the neuropathy is diagnosed quickly then there is likely to be less damage to the nerves and obviously an increased chance that the neuropathy can be slowed down, halted or reversed.     One of the most common known causes of Peripheral Neuropathy is diabetes mellitus. It is estimated that approximately 10 per cent of diabetics develop neuropathic symptoms.     We users of statin drugs have experienced some of the symptoms listed below [1] that characterize peripheral neuropathy (damage to nerves outside the brain). Medical research published in peer-reviewed journals has shown that statins are able to cause peripheral neuropathy or a syndrome that is very similar to it.
    We petition the pharmaceutical manufacturers of statins [2] to:
    1. Notify patients (past, current, and prospective users of statins) and healthcare professionals (physicians, pharmacists, nurses, physicians' assistants) of the risk associated with statin use and what to do once the first signs and symptoms of neuropathy have appeared.
    2. Sponsor and perform research on how statins cause neuropathy.
    3. Sponsor and perform clinical research on how to cure and reverse the neuropathy caused by statins.
    4. Perform clinical research and recommend the best drug treatments to mitigate the pain and make other symptoms of statin-induced neuropathy more tolerable.

Polydefkis, M.D. Peripheral nerve damage is one of the most common neurological complications of HIV infection and its treatment. Of these, the distal sensory neuropathies, which occur in the advanced stages of HIV disease, are the most common, affecting approximately 30% of AIDS patients. It is important, however, to recognize that other forms of peripheral nerve disease occur in HIV infection. Many are caused by other infectious agents and are therefore potentially treatable. This article will briefly review less common forms of peripheral nerve injury in HIV and will then focus on the HIV associated sensory neuropathies.

    Peripheral Neuropathies other than HIV Associated Sensory Neuropathies CIDP (Chronic inflammatory demyelin-ating polyneuropathy) may also be the presenting illness for HIV disease and generally occurs with CD4 counts between 200-500 cells/mm . This condition can be thought of as a chronic form of GBS. Patients typically have absent or reduced reflexes as well as patchy numbness and weakness. CSF examination often shows a mild mononuclear pleocytosis in addition to an elevated protein. Treatment centers on immunomodulation and is potentially problematic in the setting of HIV disease given the pre-existing immuno-suppression. Corticosteroids, plasmapheresis, IVIG and cyclosporin have all been used successfully. No clear guidelines for anti-retroviral therapy exist, though it seems prudent to avoid potentially neurotoxic agents.

    Why Does Peripheral Neuropathy Cause Pain?

Kelley Associate Professor of Neurology The University of Pennsylvania Medical Center Philadelphia, PA 19104-6077 Peripheral nerves are a collection of nerve fibers that originate from many different kinds of neurons. Motor fibers originate from motor neurons that are located in the spinal cord. Sensory axons originate from neurons that are located outside the spinal cord in large clusters called ganglia. The ganglia that contain the sensory neurons for the leg are located in the low back region (called the lumbar and sacral levels); those for the arm are located in the neck (called the cervical region). Each of these ganglia contains many thousands of sensory neurons. Every sensory neuron has two ends. One end is connected to a tissue in the body (a piece of skin, muscle, bone, etc.), and the other end is connected to the spinal cord. Under normal circumstances, sensations are generated only upon stimulation of the end of the nerve fiber that is in the body. Then sensory nerve fibers relay this information to the spinal cord, and cells in the spinal cord, in turn, relay this information to the brain. There are many kinds of sensory neurons. This is why we can perceive so many different sensations. All of us can appreciate many of these sensations, such as heat, cold, light touch, pin prick, vibration, and movements of the hairs on our skin. Other sensations are less obvious, such as our ability to determine movements of our arms and legs. Each kind of sensation, including pain, is conveyed to the spinal cord by certain kinds of sensory neurons.
