An Algorithm for the Evaluation of Peripheral Neuropathy

The diagnosis of peripheral neuropathies can be frustrating, time consuming and costly. Careful clinical and electrodiagnostic assessment, with attention to the pattern of involvement and the types of nerve fibers most affected, narrows the differential diagnosis and helps to focus the laboratory evaluation. An algorithmic approach to the evaluation and differential diagnosis of a patient with peripheral neuropathy is presented, based on important elements of the clinical history and physical examination, the use of electromyography and nerve conduction studies, autonomic testing, cerebrospinal fluid analysis and nerve biopsy findings. The underlying cause of axonal neuropathies can frequently be treated; demyelinating neuropathies are generally managed with the assistance of a neurologist. T he incidence of peripheral neuropathy is not known, but it is a common feature of many systemic diseases. Diabetes and alcoholism are the most common etiologies of peripheral neuropathy in adults living in developed countries. The primary worldwide cause of treatable neuropathy is leprosy.

What is a peripheral neuropathy?
The term peripheral neuropathy describes a problem with the functioning of the nerves outside of the spinal cord. The symptoms of a neuropathy may include numbness, weakness, burning pain (especially at night), and loss of reflexes. The pain may be severe and disabling. What causes a peripheral neuropathy?
There are many possible causes of peripheral neuropathy, including:
  • Some of the most common causes include repetitive activities such as typing or working on an assembly line. In this case, the neuropathy may be isolated to the upper extremities, such as with carpal tunnel syndrome. Pressure on a nerve can cause a peripheral neuropathy. For example, pressure on a nerve that comes out from the groin to the skin in front of the upper thigh (the lateral femoral cutaneous nerve) can cause burning and tingling in this location. This particular problem is called meralgia paresthetica and can be caused by wearing a tight belt or other restrictive clothing. Additionally, it can result from being overweight or pregnant.

    • Diabetics are prone to foot ulcerations due to both neurologic and vascular complications. Peripheral neuropathy can cause altered or complete loss of sensation in the foot and /or leg. Similar to the feeling of a "fat lip" after a dentist's anesthetic injection, the diabetic with advanced neuropathy looses all sharp-dull discrimination. Any cuts or trauma to the foot can go completely unnoticed for days or weeks in a patient with neuropathy. It's not uncommon to have a patient with neuropathy tell you that the ulcer "just appeared" when, in fact, the ulcer has been present for quite some time. There is no known cure for neuropathy, but strict glucose control has been shown to slow the progression of the neuropathy. Charcot foot deformity occurs as a result of decreased sensation. People with "normal" feeling in their feet automatically determine when too much pressure is being placed on an area of the foot. Once identified, our bodies instinctively shift position to relieve this stress. A patient with advanced neuropathy looses this important mechanism. As a result, tissue ischemia and necrosis may occur leading to plantar ulcerations. Microfractures in the bones of the foot go unnoticed and untreated, resulting in disfigurement, chronic swelling and additional bony prominences.

    Peripheral neuropathy (PN) affects two million people in the U.S., typically middle-aged and elderly individuals. It is a neurological disorder that affects the sensory, motor and/or autonomic nerves, and is caused by abnormal function of these nerves due to various etiologies. These disorders can originate from numerous causes, such as diabetes, alcoholism, HIV, toxin exposure, metabolic abnormalities, vitamin deficiency, or adverse effects of certain drugs. However, after evaluations for the etiologies of these PNs are performed, 32-70% of all peripheral neuropathies remain idiopathic. With the development of autoimumme and genetic tests, these idiopathic peripheral neuropathies can often be diagnosed. Autoimmune Peripheral Neuropathy
    Symptoms of an autoimmune peripheral neuropathy may include weakness, cramping, decreased tendon reflexes, numbness, tingling, and pain affecting the arms and/or legs. Clinically, peripheral neuropathies are classified according to various characteristics: symmetric or asymmetric, proximal or distal, acute or chronic, slowly progressive or rapid onset, affecting one (mononeuropathy) or many nerves (polyneuropathy). Upon electrophysiological examination, assessment of a neuropathy can be further classified, depending on which part of the peripheral nervous system is affected, such as the axon or myelin sheath. Peripheral neuropathy can be demyelinating, axonal, or both, as determined by electromyography (EMG) and nerve conduction studies (NCS).

    WHAT IS PERIPHERAL NEUROPATHY? Peripheral neuropathy (PN) is a disease of the nerves. The peripheral nerves are all the nerves except for the brain and spinal cord.
    About 30% of people with HIV develop PN. Some PN is a breakdown of the nerve endings (axons.) They send sensations to the brain. Sometimes, PN is damage to the coating of nerve fibers (myelin). This affects the transmission of pain signals to the brain. PN can be a minor nuisance or a disabling weakness. It usually shows up as a feeling of pins and needles, burning, stiffness, or numbness in the feet and toes. It can also be tickling sensations, pain when there's no reason for it, or sensations that seem more intense than normal. PN symptoms can come and go. Serious PN can cause difficulty walking or standing. WHAT CAUSES NEUROPATHY? PN can be caused by HIV infection of nerve cells, by cytomegalovirus (CMV) infection , and as a side effect of drugs taken to fight HIV or opportunistic infections . Higher viral load is associated with more PN. It also occurs in people with diabetes.
    No laboratory testing is needed to diagnose PN: the signs and symptoms are enough. Special tests may be needed to find the cause of PN. These tests measure tiny electrical currents in nerves and muscles. The amount or speed of these electric signals drops with different types of PN. However, many patients with PN are not diagnosed correctly.

    Peripheral Neuropathy Treatment of neuropathic pain such as is encountered in neuropathy is notoriously difficult. Minimizing neurotoxic drugs, optimizing diet, assuring that there are no contributing vitamin deficiencies (especially B and thiamine) are important first steps. Alcohol is often a neurotoxin, and continued heavy alcohol use may worsen symptoms. Routine analgesics such as aspirin and ibuprofen generally provide little relief. Even narcotics may not fully relieve this kind of pain. The most commonly helpful therapy is use of low doses of tricycle antidepressant drugs such as amitriptyline or imipramine, not because they can relieve depression, but because they change the pain processing in the brain, thus providing symptomatic relief for the patient. Newer antidepressants (Prozac and similar drugs) are generally less helpful than the older tricyclic compounds for this indication. Treatment of painful neuropathy is a major current focus for the Neurologic AIDS Research Consortium and of the AIDS Clinical Trial Group.     More than 2 million Americans have peripheral neuropathy. This painful condition results from damage to the nerves that carry signals from the hands and feet. These nerves are long and fragile and therefore easily damaged. It is one of the Accepted Symptoms of Agent Orange , however the VA has managed to make it Very Difficult to approve a claim. It has to be reported within one year of leaving Vietnam. For those of us who were young and didn’t mention any numbness or loss of feeling, We loose, which is not an uncommon loss for the Vietnam Veteran!
