Diagnosis: Pericarditis
Full history:
84 year-old woman with a history of congestive heart failure who presents with worsening dyspnea on exertion and lower extremity edema. Initial work-up revealed bilateral pleural effusions and a hemorrhagic pericardial effusion. PET is requested to evaluate for possible underlying malignancy.
Radiopharmaceutical:
14.9 mCi F-18 Fluorodeoxyglucose intravenously
Findings:
Projection (main image), coronal and axial (second image) FDG-PET images demonstrate marked hypermetabolism surrounding the heart in the region of the pericardium. A nodular area of increased uptake is also seen in the expected position of the lower pole of the right lobe of the thyroid gland. Axial computed tomographic images (third image) reveal a large pericardial effusion with some thickening and mild enhancement of the pericardium inferiorly.

Definition Pericarditis is a disorder caused by inflammation of the pericardium, which is the sac-like covering of the heart. Causes And Risk Pericarditis is usually a complication of viral infections caused by adenovirus and coxsackie virus. Less frequently, it can be caused by the polio influenza or rubella viruses. Infections with bacteria can lead to bacterial pericarditis (also called purulent pericarditis). Some fungal infections can also produce pericarditis. In addition, pericarditis can be associated with systemic diseases such as autoimmune disorders rheumatic fever tuberculosis , cancer, leukemia, kidney failure , HIV infections, AIDS and hypothyroidism. Heart disorders which can cause pericarditis include heart attack (see post-MI pericarditis ) and myocarditis . Radiation therapy to the chest and medications that suppress the immune system can also sometimes induce this condition. Pericarditis can also result from injury (including surgery) or trauma to the chest, esophagus or heart.
