What is pericarditis?
Pericarditis is inflammation of the pericardium, the thin sac (membrane) that surrounds the heart and the roots of the great vessels. There is a small amount of fluid between the inner and outer layers of the pericardium. When the pericardium becomes inflamed, the amount of fluid between its two layers increases, compressing the heart and interfering with its ability to function properly. Symptoms of pericarditis:
  • chest pain that:
    • can especially be felt behind the breastbone, sometimes spreading to the neck and left shoulder
    • is a sharp, piercing pain over the center or left side of the chest that increases if the person takes a deep breath a low-grade fever
    • pain when swallowing
    What causes pericarditis?
    Usually, the cause of pericarditis is unknown, but may include:
    • a viral, bacterial or fungal infection

Pericardium Pericarditis Definition: Pericarditis is a disorder caused by inflammation of the pericardium, which is the sac-like covering of the heart. Causes, incidence, and risk factors: Pericarditis is usually a complication of viral infections, most commonly echovirus or cocksackie viruses. Less frequently, it can be caused by influenza or HIV infection. Infections with bacteria can lead to bacterial pericarditis (also called purulent pericarditis). Some fungal infections can also produce pericarditis. In addition, pericarditis can be associated with systemic diseases such as autoimmune disorders rheumatic fever tuberculosis , cancer, leukemia, kidney failure , HIV infections, AIDS and hypothyroidism. Heart disorders that can cause pericarditis include heart attack (see post-MI pericarditis ) and myocarditis . Radiation therapy to the chest and medications that suppress the immune system can also sometimes induce this condition. Pericarditis can also result from injury (including surgery) or trauma to the chest, esophagus, or heart.

Bacterial pericarditis is an inflammation (irritation and swelling) of the sac that surrounds the heart (pericardium), caused by a bacterial infection.
Alternative Names: Purulent pericarditis
Causes, incidence, and risk factors: Bacterial infections are one cause of pericarditis . Since the advent of antibiotics, bacterial pericarditis has become relatively rare. It most often occurs in men between the ages of 20 and 50, usually after some type of respiratory infection. It can also occur after skin or oral infections that produce bacteremia (bacterial infection of the blood) and after heart surgery. The bacterial infection causes inflammation of the pericardium. Pain occurs as a result of the inflamed pericardium rubbing against the heart. Fluid may accumulate in the pericardial sac. The most common organisms which cause this condition are staphylococci, streptococci, pneumococci, hemophilus influenza (also called H.flu), and meningococci.

    Acute pericarditis has many potential etiologies and typically presents as sharp central chest pain that worsens with recumbency and is relieved by leaning forward. The pathognomonic physical finding of acute pericarditis is the pericardial friction rub, which is usually auscultated along the lower left sternal border. The electrocardiogram (ECG) is a useful, simple tool that may aid in the diagnosis of acute pericarditis. Typical ECG findings include diffuse concave-upward ST-segment elevation and, occasionally, PR-segment depression. ECG changes of both acute myocardial infarction and early repolarization can appear similar to ECG changes of acute pericarditis. However, these conditions can usually be excluded by an accurate history, physical examination and recognition of a few key features on the ECG. This article exemplifies the AAFP 1997-98 Annual Clinical Focus on prevention and management of cardiovascular disease.

    Fibrinous pericarditis is an exudative inflammation. The pericardium is infiltrated by the fibrinous exudate. This consists in fibrin strands and leukocytes. Fibrin describes an eosinophilic (pink) network, amorphous. Leukocytes (mainly, neutrophils) are found within the fibrin deposits and intrapericardic. Vascular congestion is also present.     Pericarditis is an inflammation of the lining sac (pericardium) which surrounds the heart. It can be associated with a collection of fluid (pericardial effusion) in the space between the heart and the pericardium. Pericarditis accounts for approximately 1 in 1,000 hospital admissions. It is found in about 5% of patients at autopsy. This implies that many cases of pericarditis occur without symptoms severe enough to come to medical attention. What causes pericarditis?
    In most patients with pericarditis, no specific cause can be found, and the condition is termed idiopathic pericarditis. Scientists now believe that many cases of idiopathic pericarditis are probably due to undiscovered viruses, such as the common cold virus, which infects and damages the pericardium. Specific diseases known to cause pericarditis are listed below. These diseases damage the pericardium, thereby triggering the body's immune defenses to attack the pericardium tissue and cause pericarditis. These diseases include:

    Peer Review Status: Internally Peer Reviewed (1) Introduction Acute pericarditis, caused by inflammation of the pericardium, manifests as chest pain, a pericardial friction rub, and has serial electrocardiogram (ECG) abnormalities. The incidence is 1/1000 hospital admissions. At autopsy, changes of pericarditis appear in 2 to 6 percent of cases. The majority of cases are idiopathic or postviral, but acute myocardial infarction, uremia, bacterial infection, tuberculosis, collagen-vascular disease, neoplasm, and trauma must be considered. (2) Symptoms suggestive of pericarditis Pericarditis chest pain occurs in the pericardium with a pleuritic component. Recumbency increases the pain, which often radiates to the trapezius ridge. Swallowing may exacerbate pain. Dyspnea results from pleural irritation or, on occasion, accumulation of a large pericardial effusion. Symptoms typically occur with an ongoing or preceding upper respiratory infection. Exertion may exacerbate chest pain, but the pleuritic nature and positional component persist. When pericarditis follows myocardial infarction, ischemic chest pain precedes. Coexistent symptoms of pneumonitis suggest a bacterial etiology, especially with high fever. Recurrent fevers, sweats, and weight loss suggest tuberculosis. Malignant pericarditis is usually metastatic with other clinical evidence of malignancy. Common primary tumors include lung, breast, melanoma, lymphoma, and leukemia.
