Peptic ulcer

September 2003 A peptic ulcer is an area of damage to the lining of either the stomach or the duodenum (the part of the digestive tract that immediately follows the stomach). Peptic ulcers can cause symptoms of indigestion, including pain, which may be severe. They can usually be treated with medicines but left untreated can lead to serious complications.
About peptic ulcers
An ulcer in the stomach is called a gastric or stomach ulcer. In the duodenum it's called a duodenal ulcer. Peptic ulcers are usually raw patches that are 1-2cm in diameter. When viewed with a gastroscope (a telescope that doctors use to examine the stomach) they look a little like mouth ulcers. Duodenal ulcers affect up to 1 in 10 people at some point in their lives, and tend to affect especially younger men. Stomach ulcers are less common. They are rare in people before middle age and are most common in elderly people.

What is an ulcer?
An ulcer is damage to the inner lining (the mucosa) of the stomach or the upper part of the intestine (duodenum). A bacterium, Helicobacter pylori, is the main cause of ulcers in this area.
Why do people get ulcers?
The most common cause is infection with Helicobacter pylori bacteria and this is responsible for up to 90 per cent of all cases of peptic ulceration.
The second most common cause is damage inflicted by aspirin or non-steroidal anti-inflammatory drugs (NSAIDs, eg diclofenac or naproxen ) used by many for arthritis , rheumatism, backache headaches and period pain
Ulcers can also occur in people weakened by severe disease (such as chronic respiratory disease or major trauma). This is thought to result from poor oxygenation to the lining of the stomach.

A coating of mucus and other chemicals normally shield the stomach and duodenum from digesting themselves. When these protective mechanisms are disrupted, powerful digestive acids can erode into the lining of these organs and cause peptic ulcers. CAUSES
The most common cause of peptic ulcers is infection by a bacterium called Helicobacter Pylori (or H. Pylori, for short). H. Pylori infection breaks down the protective mucous barrier, exposing the stomach and duodenum to digestive juices. Chronic inflammation develops in the duodenum (duodenitis) or stomach (gastritis); if untreated, this can evolve into a peptic ulcer. Current medical opinion suggests that anyone with ulcer symptoms should, at some point, be checked for the presence of H. Pylori. The importance of this finding is that, in many cases of H. Pylori positive ulcers, antibiotics can actually heal the ulcer. Other causes of peptic ulcers include Non-Steroidal Anti-inflammatory Drugs (NSAIDS) e.g. Aspirin, viral illnesses and certain rare tumors. Smoking, genetics, and alcohol consumption may be contributing factors. Psychological stress and diet (spicy foods, etc.), which were once thought to be causative, are no longer considered to be significant.

It's usually not lifestyle that causes that gnawing pain. Most likely it's a bacterial infection. And the infection can be treated and permanently eliminated in most people. All it usually takes is 2 weeks of treatment with a combination of antibiotics and a drug that blocks acid production in the stomach. Just a few years ago, most of us believed ulcers were caused by too much spicy food, too much coffee, too much alcohol, and too much stress. But now we know that about 90% of ulcers are caused by a tough little germ called Helicobacter pylori (H pylori) that lives on the lining of the stomach. H pylori is common everywhere in the world, although it does not always cause ulcers. Scientists in Australia were the first to recognize that many people with ulcers had strange, corkscrewlike bacteria in their stomachs. Convincing the world that infection can cause ulcers is no easy task, however. To prove his point, one of the Australian doctors infected himself with H pylori and quickly became sick with typical ulcer symptoms. His experiment led to the first major breakthrough proving ulcers could be cured.

  • it's like a dull, gnawing ache
  • it comes and goes for several days or weeks
  • it occurs 2 to 3 hours after a meal
  • it occurs in the middle of the night (when the stomach is empty)
  • it's relieved by food
Other symptoms include weight loss, loss of appetite, bloating, burping, nausea, and vomiting. Causes of peptic ulcers Contrary to some long-held beliefs, experts now say that peptic ulcers are not caused by stress or spicy foods. According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), the main cause of peptic ulcer is a bacterial infection. Researchers have discovered that a type of bacteria called Helicobacter pylori (H. pylori) is a primary cause of peptic ulcers. This bacteria is very common, but most people infected with it do not develop ulcers. We may come into contact with H. pylori through food or water. Basically, the bacteria weakens the stomach's lining, allowing acid to get through, causing irritation and ultimately letting a sore develop.

    About one in 10 Americans develop at least one ulcer during their lifetimes. Ulcers affect about 5 million people each year. More than 40,000 people each year have surgery because of persistent symptoms or problems from ulcers. An ulcer is an open sore, or lesion, usually found on the skin or mucous membrane areas of the body.
    • An ulcer in the lining of the stomach or duodenum, where hydrochloric acid and pepsin are present, is referred to as a peptic ulcer. When the ulcer is in the stomach, it is called a gastric ulcer. When the ulcer is in the duodenum, it is called a duodenal ulcer.
    What causes gastric and duodenal ulcers?
    In the past, it was believed lifestyle factors such as stress and diet caused ulcers. Later, researchers determined that stomach acids - hydrochloric acid and pepsin - contributed to ulcer formation. Today, research shows that most ulcers (80 percent of gastric ulcers and 90 percent of duodenal ulcers) develop as a result of infection with a bacterium called Helicobacter pylori (H. pylori). It is believed that, although all three of these factors - lifestyle, acid and pepsin, and H. pylori - play a role in ulcer development, H. pylori is considered to be the primary cause, in most cases.
