The term PUD generally refers to spectrum of disorders that includes gastric ulcer (GU), pyloric channel ulcer, duodenal ulcer (DU) and postoperative ulcers at or near the site of surgical anastomosis.
HISTORICAL PERSPECTIVE
Looking back at history, peptic ulcer was a rare and generally unrecognized as a cause of symptoms/complications or death until the early 19th century. Despite sporadic case reports beginning in late 18th century, Peptic Ulcer Disease did not become widely appreciated until early 20th century. The first 6 decades saw the dominance of surgery in the treatment of peptic ulcer. With the introduction of acid suppressive drugs like histamine-2 blockers in 1970s, the treatment of PUD was revolutionized. By nineteen eighties, the advent of Helicobacter pylori (HP) brought about a dramatic twist and possibly the cure.
CAUSES OF PEPTIC ULCER DISEASE
  • So what causes PUD? Numerous factors blamed in the pathogenesis of PUD. Many of these factors may be acquired during life, although some of these these may already be predetermined. Historically, stomach acid has been the most common factor blamed. No Acid No ulcer- is an old axiom which may be questionable in the nineties. As a group, patients with duodenal ulcer have high acid secretion. Increased acid secretion causes changes in the wall of duodenum (gastric metaplasia) setting the stage for invasion by HP.

NSAIDs and Peptic Ulcers
A peptic ulcer is a sore that forms in the lining of the stomach or the duodenum (the beginning of the small intestine). An ulcer can cause a gnawing, burning pain in the upper abdomen; nausea; vomiting; loss of appetite; weight loss; and fatigue. Most peptic ulcers are caused by infection with the bacterium Helicobacter pylori (H. pylori). But some peptic ulcers are caused by prolonged use of nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin, ibuprofen, and naproxen sodium. Normally the stomach has three defenses against digestive juices: mucus that coats the stomach lining and shields it from stomach acid, the chemical bicarbonate that neutralizes stomach acid, and blood circulation to the stomach lining that aids in cell renewal and repair. NSAIDs hinder all of these protective mechanisms, and with the stomach's defenses down, digestive juices can damage the sensitive stomach lining and cause ulcers. NSAID-induced ulcers usually heal once the person stops taking the medication. To help the healing process and relieve symptoms in the meantime, the doctor may recommend taking antacids to neutralize the acid and drugs called H2-blockers or proton-pump inhibitors to decrease the amount of acid the stomach produces.

What is a stomach or duodenal ulcer?
An ulcer is an open sore, or lesion, usually found on the skin or mucous membrane areas of the body. An ulcer in the lining of the stomach or duodenum, where hydrochloric acid and pepsin are present, is referred to as a peptic ulcer

What causes gastric and duodenal ulcers?
In the past it was believed lifestyle factors, such as stress and diet caused ulcers. Later, researchers determined that stomach acids hydrochloric acid and pepsin contributed to ulcer formation. Today, research shows that most ulcers (80 percent of gastric ulcers and 90 percent of duodenal ulcers) develop as a result of infection with a bacterium called Helicobacter pylori (H. pylori).

WHAT ARE THE SYMPTOMS OF PEPTIC ULCERS?
Dyspepsia and Common Symptoms of Ulcers
The most common symptoms of peptic ulcers are known collectively as dyspepsia . Peptic ulcers can occur without dyspepsia or any gastrointestinal symptoms, especially when caused by NSAIDs. Dyspepsia may be persistent or recurrent and can encompass a variety of problems in the upper abdomen, including the following:
  • Pain or discomfort. [See Ulcer Pain , below, for more detailed description.] Bloating. A feeling of fullness. (People with severe dyspepsia are unable to drink as much water or other beverages as people with mild or no dyspepsia.) Hunger and an empty feeling in the stomach, often one to three hours after a meal. Mild nausea. (Vomiting, in fact, may relieve symptoms.) Regurgitation. (The sensation of acid backing up into the throat.) Belching.
Ulcer Pain. The pain of ulcers can be either localized in one place or diffuse. The pain has been described as burning, gnawing, or aching in the upper abdomen, or as a stabbing pain penetrating through the width of the gut. The symptoms may vary depending on the location of the ulcer:
  • Duodenal ulcers often cause a gnawing pain in the upper stomach area several hours after a meal, and the pain is often relieved by eating a meal.
