Pemphigus foliaceus is an autoantibody-mediated blistering disease in which antibodies against desmoglein 1 cause loss of adhesion among keratinocytes in the superficial epidermis. Desmogleins are transmembrane glycoproteins found in desmosomes, which provide physical connections between cells. The main desmogleins expressed in the epidermis are desmoglein 1 and desmoglein 3. TREATMENT OF PEMPHIGUS VULGARIS AND PEMPHIGUS FOLIACEUS: EXPERIENCE WITH 71 PATIENTS OVER A 20 YEAR PERIOD* SUMMARY Forty one cases of pemphigus vulgaris and thirty cases of pemphigus foliaceus were investigated at Hospital Universitario Clementino Fraga Filho from 1978 to 1999. They were divided into two treatment groups: one group received up to 100 mg of oral prednisone daily and the other group received >120 mg daily. The dose up to 100 mg provided good initial control of pemphigus vulgaris and pemphigus foliaceus and did not increase the mortality rate associated to disease. The dose >120 mg induced higher morbidity. These data allowed us to establish a regimen of oral prednisone (1-2 mg/kg/daily) with maximum of 120 mg daily in the treatment of pemphigus vulgaris and pemphigus foliaceus.

General Information
Pemphigus foliaceus is a severe skin disease of dogs and cats caused by dysfunction of the immune system. Although the underlying cause is unknown, all cases involve autoantibody production in which the immune system produces antibodies that attack the animal's own body cells.
Skin changes consist of redness, crusts, oozing, hair loss, and erosions of the skin surface that usually begin on the face and ears. These changes spread to the groin and abdomen and become widespread within 6 months. The footpads become thickened, cracked, and hard and shed flakes of pad. In cats, the disease often affects the nails and nipples. Severely affected animals seem depressed, run a fever, and refuse to eat.
Important Points in Treatment
  • Pemphigus is a very serious condition and is often fatal. Treatment involves large doses of antiinflammatory and/or anticancer drugs. Side effects are common and various blood tests and regular examinations are necessary to monitor progress. Many patients require lifelong therapy.

    Wat is het?
    Pemphigus is een zeldzame blaarvormende ziekte. In Nederland wordt jaarlijks bij ongeveer 120 mensen de aandoening vastgesteld. Patienten met pemphigus zijn meestal van middelbare leeftijd. De blaren kunnen voorkomen op de huid en op de slijmvliezen, zoals het mondslijmvlies. Pemphigus gaat vaak met jeuk gepaard. Er worden 3 soorten pemphigus onderscheiden:
    • pemphigus vulgaris (komt het meeste voor) pemphigus foliaceus paraneoplastische pemphigus (een vorm die wordt gezien bij patienten met kanker)

    Hoe ontstaat het?
    Pemphigus ontstaat door een autoimmuun reactie, dat is een reactie van het lichaam tegen het eigen weefsel. Bij pemphigus is deze reactie gericht tegen bepaalde bestanddelen van de opperhuid, waardoor de opperhuid plaatselijk loslaat en blaren gaat vormen.
    Waarom de autoimmuun reactie ontstaat is nog niet bekend. In sommige gevallen wordt het veroorzaakt door geneesmiddelen (penicillamine, captopril).
    Bij pemphigus vulgaris zijn de blaren vrij slap en met vocht gevuld. De blaren gaan door stoten of krabben makkelijk stuk.
    De blaren bij pemphigus foliaceus zijn heel slap en gaan al heel snel stuk zodat er al snel oppervlakkige wondjes (erosies) ontstaan.
