Pemphigus of the skin and mucous membranes is a chronic blistering disease most commonly found in middle-aged persons. There are three major types of pemphigus:
  • Vulgaris (literally meaning common)
  • Foliaceus (featuring superficial blisters)
  • Paraneoplastic (associated with some cancers)
Pemphigus is an autoimmune disorder in which the skin cells are attacked by the patient's own antibodies (immune proteins). This causes a separation between the layers of skin in which fluid subsequently accumulates, producing blisters. The introduction of corticosteroids in recent decades has turned what often had been a fatal disease into one that can be managed quite well. Causes The etiology of pemphigus is largely unknown, although genetic predisposition is thought to be the cause. An endemic form of pemphigus foliaceus has been identified in certain rural areas of Brazil and may be transmitted by the bite of a black fly. In rare cases, a medication can induce an attack. Such cases almost always resolve after the offending drug is discontinued. In paraneoplastic pemphigus, the skin problem is caused by cancer. The underlying cancer usually is lymphoid (lymphoma or leukemia) in origin. Paraneoplastic pemphigus typically resolves after the cancer has been treated successfully. It also can be associated with benign lymph tumors.

Histologic clue: "the naked epidermis", i.e. the superficial cell layers of the stratum corneum are gone (due to superficial acantholysis) and the epidermis appears "nude". Confirmation of the diagnosis by direct immunofluorescence (cf. figure). Differential diagnosis: superficial impetigo contagiosa, which may look very similar histologically but contains an abundance of bacteria, immuno-stainable with anti-mycobacterium bovis BCG, which cross-reacts with many other organisms incl. bacteria. PEMPHIGUS (Gr. ir~u~tE, a bubble),a skin disease, in which large blebs appear, on a red base, containing a clear or yellowish fluid; the blebs occasion much irritation, and when they burst leave raw ulcerated surfaces. The disease is principally known in unhealthy or neglected children. A variety of the malady, pemp/zigus foliaceous, affects the whole body, and gradually proves fatal. Pemphigus of an acute septicaemic type occurs in butchers or those who handle hides, and a diplococcus has been isolated by William Bullock. The treatment is mainly constitutional, by means of good nourishment, warm baths, local sedatives and tonics. In chronic pemphigus, streptococci have been found in the blebs, and the opsonic index was low to streptococci. Improvement has been known to take place on the injection of a vaccine of streptococci.     Nobody knows. It is known that Pemphigus Vulgaris is not contagious. There is a research which shows that there is a genetic predisposition to Pemphigus Vulgaris, but we can only guess what aggravates the flares of the disease. to desmoglein 3, a protein found in normal healthy skin. The antibodies produce a
reaction that leads to separation of the epidermal (skin) cells. Those afflicted suffer
from burn-like lesions and blisters all over their body.
