What is pemphigus foliaceus?
The pemphigus family are rare autoimmune blistering diseases affecting skin and/or mucous membranes. Pemphigus foliaceus (PF) is a rare relatively benign form of pemphigus. In New Zealand pemphigus foliaceus is more often encountered than its more serious relative pemphigus vulgaris , although worldwide pemphigus vulgaris is more common. Pemphigus foliaceus is characterised by blistering lesions on otherwise healthy-looking skin. Blisters tend to form when the skin is rubbed (Nikolsky sign). There are currently 6 subtypes:
  • Pemphigus erythematosus Pemphigus herpetiformis Endemic PF IgA PF Paraneoplastic PF Drug-induced PF.
What causes pemphigus foliaceus?
Pemphigus foliaceus is an autoimmune disease, which basically means that an individual's immune systems starts reacting against his or her own tissue. The building block cells of the epidermis are called keratinocytes. These cells are cemented together at special sticky spots called desmosomes. In pemphigus foliaceus autoantibodies bind to a protein called desmoglein-1, which is found in desmosomes in the keratinocytes near the top of the epidermis. The result is the surface keratinocytes separate from each other, and are replaced by fluid: the blister. Because the blister is very close to the surface of the skin the blisters rupture easily. In most cases the autoantibodies are immunoglobulin type G (IgG) but in IgA PF the autoantibodies are type A (IgA).

What is paraneoplastic pemphigus?
The pemphigus family are rare autoimmune blistering diseases affecting skin and/or mucous membranes. Paraneoplastic conditions occur in association with malignancies (cancer). Paraneoplastic pemphigus is characterised by painful denuded areas of the mouth, lips, oesophagus and skin. It is the least common but most serious form of pemphigus, particularly considering the presence of the underlying cancer.
What causes paraneoplastic pemphigus?
An autoimmune disorder is a disease in which an individual's immune system starts reacting against his or her own tissue. The building block cells of the epidermis are called keratinocytes. These cells are cemented together at special sticky spots called desmosomes. In paraneoplastic pemphigus and other types of pemphigus there are autoantibodies to a component of the desmosomes. These cause the keratinocytes to separate from each other. The gap fills with fluid, causing a blister or the skin may peel off leaving raw areas.

What is autoimmune skin disease and pemphigus foliaceus?
Autoimmune skin disease is a condition in which the body's immune system targets the skin for destruction. The damage results in lesions that normally can be seen by careful examination of the skin. Pemphigus foliaceus (PF) is a type of autoimmune skin disorder. With this disease, antibodies produced by the immune system combine with the action of white blood cells to damage the "glue" that holds epidermal (skin) cells together.

INDEX: pemphigus research samples Symptoms In pemphigus, the autoimmune system is directed against the skin itself. Affected dogs have what looks like a rash at onset that develops into blisters and then pustules. Finally, the dog is left with crusty sores on the non-furred areas of skinthe pads, nose, lips, and eye rims. Occasionally, all the skin is involved, but this is less common. Because the produce pigment-production cells in the skin may be affected also, color may disappear from involved areas.
An affected dog may show signs that he doesn't feel well; he may seem tired and depressed. He may also be stiff from swollen joints. The disease shows no sex preference and is usually seen in mature dogs (over two). Diagnosis
Diagnosis is made by skin biopsy. Thyroid panels should be run also because pemphigus is affected dogs often also have thyroid disease and need supplementation. Treatment
Affected dogs are usually maintained on steroids with the dosage being adjusted with the severity of symptoms. If steroids alone are not effective, then other immunosuppressors can be used. Pemphigus involves blistering because of separation between cells within the epidermis, while bullous pemphigoid involves separation of the full thickness of epidermis from the basement membrane. Because of the more superficial location of the epidermal separation in pemphigus, the blisters or bullae in pemphigus tend to be much more fragile than those seen in bullous pemphigoid. Definition An autoimmune skin disorder characterized by blistering of the skin and mucous membrane
Pemphigus, Vulgaris on the Back
Pemphigus, Vulgaris - Lesions in the Mouth Pemphigus involves blistering of the outer (epidermal) layer of the skin and mucous membranes . It is an autoimmune disorder in which the immune system produces antibodies against specific proteins in the skin and mucous membrane. These antibodies produce a reaction that leads to a separation of epidermal cells (acantholysis). The exact cause of the development of antibodies against the body's own tissues (autoantibodies) is unknown. Sometimes pemphigus appears due to reactions to medications, although this is rare. Pemphigus may be a side effect of ACE inhibitors (such as Elanapril) or chelating agents (such as penicillamine).

Pemphigoid and pemphigus
Dr Rob Hicks Pemphigoid and pemphigus are autoimmune diseases causing the skin to blister. Neither can be cured and both are very rare. On the surface they may appear similar but they are two different diseases.

The body attacks the skin
In autoimmune diseases the body perceives part of itself as being foreign and triggers an immune reaction whereby it attacks itself. In the case of pemphigoid it's the skin cells that are attacked. In the case of pemphigus it's the skin cells and often those of the mucosal membranes (the inside of the mouth, for example) that come under attack. When this happens it interferes with the normal structure and function of the cells causing the burn-like blisters that are characteristic of these two diseases to appear.
Pemphigoid
This is the more common but less dangerous of the two and tends to affect older people who are usually over the age of 60.

Some will manifest in the oral mucosa prior to the skin lesions and some others only after the dermatologic manifestations have taken place. Still another group will have mostly mucous membranes involvement with only minor skin changes. Pemphigus is a severe dermatologic disease that is generally divided into pemphigus vulgaris (PV), pemphigus vegetans, pemphigus foliaceous, pemphigus erythematosus and benign familial chronic pemphigus. Some drugs with terminal sulfhydryl groups (i.e. penicillamine) can induce lesions which are clinically identical to those seen in PV. Malignant neoplasms are also capable of producing the so-called paraneoplastic PV which is similar to the classic PV.

Types
There are three types of pemphigus which vary in severity: pemphigus vulgaris, pemphigus foliaceus, and paraneoplastic pemphigus. The most common form of the disorder is pemphigus vulgaris. It occurs when antibodies attack Desmoglein 3 , a protein that keeps cells bound together. Thus, cells simply fall apart, causing skin to slough off. Although pemphigus vulgaris may occur at any age, it is quite rare in children, and most common in the middle aged and elderly. Sores often originate in the mouth , making eating difficult and uncomfortable. Pemphigus foliaceus is the least severe of the three varieties. Desmoglein 1 , the protein that is destroyed by the body's immune system is only found in the top dry layer of the skin, so mouth sores do not occur.
