International Notes Outbreak of Pellagra Among Mozambican Refugees - Malawi, 1990
Micronutrient deficiency disorders, including pellagra, have emerged as major problems in refugee populations that depend on international relief assistance for food supplies (1,2). This report summarizes an investigation of pellagra that occurred among Mozambican refugees in Malawi during 1990. Since 1987, approximately 900,000 Mozambicans have fled armed conflict in their homeland to seek refuge in neighboring Malawi. Approximately half are housed in refugee camps; the remainder live in villages integrated with the local Malawian population. Overall refugee relief assistance is coordinated by the Office of the United Nations High Commissioner for Refugees (UNHCR). Health care is provided by Malawian Ministry of Health personnel assisted by private voluntary organizations. During July-October 1989, 1169 cases of pellagra were detected among Mozambican refugees settled in 11 sites (including both camps and integrated villages) in southern Malawi (1). From February 1 through October 30, 1990, 17,878 cases were reported among 285,942 refugees (attack rate (AR)=6.3%) (Figure 1). During the 1989 outbreak, Medecins Sans Frontieres, Paris (MSF), the private voluntary organization that provided health care in the 11 sites, established a passive surveillance system in which persons with pellagra who presented to health facilities were recorded separately. (Previously, pellagra cases were reported as unspecified "nutritional disorders" according to the national disease surveillance system.) In July 1990, active case detection was instituted, and detailed information was collected on each patient (including age, sex, address, nationality, date of arrival in Malawi, duration of illness, and clinical signs).

Tryptophan metabolism in alcoholic pellagra patients: measurements of urinary metabolites and histochemical studies of related muscle enzymes

Biochemical and enzymatic aspects of tryptophan-niacin metabolism were studied in 15 adult alcoholic pellagra patients and in 14 controls. In addition to the clinical signs of niacin deficiency, most of the pellagra patients had other signs of malnutrition. Interaction of niacin and zinc metabolism in patients with alcoholic pellagra

The effect of zinc supplementation on the metabolism of tryptophan conversion to niacin was studied in 14 alcoholic patients with pellagra and in 7 male control subjects aged 21-45 y. The pellagrins received chemically defined diets based on crystalline amino acids through an enteral tube for 7 d. Patients were divided into two groups (A and B), both receiving a diet from which tryptophan, Zn, and niacin were excluded. Patients in group B

Pellagra no longer stalks the nation as it once did. But during the early part of the 20th-century, pellagra, a disease that results from a diet deficient in niacin killed, many poor Southerners. Dr. Joseph Goldberger, a physician in the U.S. government's Hygienic Laboratory, the predecessor of the National Institutes of Health, discovered the cause of pellagra and stepped on a number of medical toes when his research experiments showed that diet and not germs (the currently held medical theory) caused the disease. He also stepped on Southern pride when he linked the poverty of Southern sharcroppers, tenant farmres, and mill workers to the deficient diet that caused pellagra. Back To Top Photography Credits

Joseph Goldberger's theory on pellagra contradicted commonly-held medical opinions. The work of Italian investigators as well as Goldberger's own observations in mental hospitals, orphanages, and cotton mill towns, convinced him that germs did not cause the disease. In such institutions, inmates contracted the disease, but staff never did. Goldberger knew from his years of experience working on infectious diseases that germs did not distinguish between inmates and employees. Lombroso had speculated that spoiled maize caused pellagra.
Goldberger found no evidence for that hypothesis, but diet certainly seemed the crucial factor. Shipments of food which Goldberger had requested from Washington were provided to children in two Mississippi orphanages and to inmates at the Georgia State Asylum. Results were dramatic; those fed a diet of fresh meat, milk and vegetables instead of a com-based diet recovered from pellagra. Those without the disease who ate the new diet did not contract pellagra.
Experimenting on Mississippi Prisoners
Critics, many unable to part from the germ theory of pellagra, raised doubts. Goldberger hoped to squelch those reservations by demonstrating the existence of a particular substance that when removed from the diet of healthy individuals resulted in pellagra. With the cooperation of Mississippi's progressive governor, Earl Brewer, Goldberger experimented on eleven healthy volunteer prisoners at the Rankin State Prison Farm in 1915. Offered pardons in return for their participation, the volunteers ate a corn-based diet. Six of the eleven showed pellagra rashes after five months.

Pellagra Pellagra ("Raue Haut") entsteht bei Niacinmangel. Dabei kommt es in erster Linie zu Hautveränderungen. Dazu kommt Durchfall und Appetitlosigkeit, aber auch Verwirrung und Depressionen können auftreten. Eine solche Unterversorgung mit Niacin wird nur bei einer eiweißarmen Ernährung beobachtet, die zu einem Großteil aus Mais besteht, der zudem wenig Niacin enthält. In unseren Breitengraden ist ein Mangel an Niacin selten.
