SIGNS AND SYMPTOMS
Pediculosis is an eyelid infestation by either Pediculus humanus corporis (body) or Pediculus humanus capitus (head). Phthiriasis, which is actually the most common eyelid infestation, is caused by Phthirus pubis (pubic lice, sometimes referred to as crab lice). Pediculus are 2 to 4 mm long, and typically infest the hair of the patient. Infestation of the cilia is rare and only occurs in the worst cases. Phthirus are 2mm long, and have a broad-shaped, crab-like body. Its thick, clawed legs make it less mobile than the Pediculus species and lend it to infesting areas where the adjacent hairs are within its grasp (eyelashes, beard, chest, axillary region, pubic region). They rarely infest the scalp. Ocular signs and symptoms include visible organisms on the scalp, hair, eyelashes or beard; visible blue skin lesions (louse bites); reddish brown deposits (louse feces); secondary blepharitis with preauricular adenopathy; follicular conjunctivitis; and, in severe cases, marginal keratitis. The patient often complains of bilateral ocular itching and irritation. PATHOPHYSIOLOGY
The Pediculus and Phthirus organisms look similar the each other and interbreed freely. Both types lay eggs on the hair shafts and remain firmly adherent, resisting both mechanical and chemical removal. The Pediculus organism moves well and can be passed from person to person by either close contact or by contact with contaminated bedding. Conversely, Phthiriasis are slow moving, and cannot typically be passed unless cilia is brought into close proximity with infested cilia. Both species are associated with crowded conditions or poor personal hygiene.

Pediculosis and Pthiriasis Eyelid infestation by Pediculus humanus. Phthirus pubis, also known as pubic or crab louse, attached to a lid. Signs and symptoms: Pediculosis refers to eyelid infestation by Pediculus human-us corporis (body) or capitus (head). Pthiriasis refers to eyelid infestation by Pthirus pubis (pubic louse, sometimes referred to as crab louse). Eyelid infestation is almost always Pthirus pubis. Pediculus is an organism 2-4 mm long that typically infests the hair of the patient. Infestation of the cilia is rare and only occurs in the worst cases. Pthirus is 2mm long with a broad-shaped, crab-like body. Its thick, clawed legs make it less mobile than the pediculus species and lend it to infesting areas where the adjacent hairs are within its grasp (eyelashes, beard, chest, axillary region, pubic region). Rarely does Pthirus infest the scalp. Both organisms suck the blood of the host, and Pediculus humanus may serve as a vector of diseases such as typhus and trench fever. Ocular signs and symptoms include visible organisms within the scalp, hair, eyelashes or beard; visible blue skin lesions (louse bites), reddish brown deposits (louse feces), secondary blepharitis with preauricular adenopathy, follicular conjunctivitis and, in severe cases, marginal keratitis. The patient often presents with bilateral ocular itching and irritation. Superinfection of bites can lead to preauricular gland swelling.

Lice infestation (pediculosis) is a skin infestation by tiny, wingless insects (Lice) which spread easily from person to person by bodily contact, shared clothing and other personal items. Of the three types of lice, the head louse infests the scalp and hair on the scalp. SYMPTOMS
Children may barely notice head lice, or have only a mild scalp irritation. The most common symptoms include:
  • Itching Small red bumps on scalp, neck and shoulders (lice bites) Infection, accompanied by swollen glands (less common)
PROGNOSIS/TREATMENT
Non-prescription shampoos and creams containing pyrethrins and piperonyl butoxide are common. Prescription permethrin applied as a cream or liquid can also be very effective. Lindane, a virtual insecticide, should be avoided (especially with small children) as it has been found to cause neurological disorders and even death.
Nits, or lice eggs, must be removed manually after shampooing to kill lice. The female louse lays the eggs at the base of the hair close to the scalp. They are very hard to remove, so a special nit comb is recommended and should be used after each drug application (often times the nits combs are included with the drug application).

Pediculosis (Head Lice)
What are head lice?
The head louse ( Pediculous humanus capitus ) is one of three types of lice that infest people. Head lice are tiny insects (about 1/10 to 1/8 of an inch long) that live in human hair and feed on human blood. They multiply rapidly, laying little silvery-colored oval-shaped eggs (called nits) which they glue to the base of the hair, close to the scalp. Can you see head lice?
Although it's hard to see head lice, you can see the nits if you look closely. They are most often found in the hair behind the ears and at the back of the head and neck. Nits should not be confused with dandruff. Dandruff can easily be flicked off the hair; nits cannot because they are firmly attached to individual hairs. Who gets head lice and what are the symptoms?
Anyone can get head lice. They are not a sign of being dirty. Head lice are easily spread from person to person by direct contact, and are often found in school settings. One telltale sign of head lice is a persistent itching of the scalp which is sometimes accompanied by infected scratch marks or what appears to be a rash. How are head lice spread and how long is a person able to spread them?

Definition
Pubic pediculosis is due to infestation with the crab louse, Phthirus pubis
Causative Organism
P. pubi s is a greyish, slate-blue or reddish-brown louse with crab-like appendages with which it clings to hairs. Lice which become detached from the body can live for many hours so that recently contaminated toilet seats, towels, underclothing or bedding can be sources of infestation. The lice feed on blood several times a day. The female lays eggs which are firmly attached to the hairs close to the skin. The eggs hatch in 7 to 9 days.
Incubation Period
The infestation is generally transferred during sexual or close body contact or by using unwashed bedding or clothes after their contamination by an infested person. Symptoms and signs can be expected after a variable period, which may be several weeks.
Clinical Manifestations
Intense irritation is usually the first symptom after which comes scratching, secondary infection and eczematous changes.

PEDICULOSIS
GENERAL INFORMATION: What is it? Pediculosis (puh-dik-u-lo-sus) is a lice infection of the hairy areas on the scalp, genital area, or body. Eyebrows, eyelashes, chest hair, or underarm hair may also be infected. Lice live in hairy body areas and bite the skin for food. The lice may spread from an infected person by sharing hats, combs, brushes, towels, clothes, or bedding. You may also get this infection if you have sex with someone who has lice. Head lice can spread very quickly among children in school and day care centers. With medicine the lice should be gone within 1 day. Causes: Lice are insects (bugs) that bite the skin on hairy areas of the body. Lice lay white eggs, called nits, in the hair. The eggs hatch in about 7 days. Gray bugs (lice) come from the eggs. The lice are tiny, move quickly, and are hard to see. Usually only the eggs are seen. Some lice live in clothing near the skin, such as underwear.

What is pediculosis?
Pediculosis is an infestation of lice. Body lice infestations occur primarily on clothing, especially along the seams of inner surfaces and the adjacent area of the skin. Pubic/crab lice infest hairy parts of the body including the groin, eyebrows, eyelashes and facial hair. Both body lice and pubic/crab lice feed on human blood and can cause severe local itching.
Who gets pediculosis?
Anyone can become infested with body lice or pubic/crab lice, regardless of age, sex, race, or standards of personal hygiene. Body lice infestation are usually found in people living in crowded, unsanitary conditions where clothing is infrequently changed or laundered. Pubic/crab lice infestations are mainly found among sexually active individuals.
How are body lice and pubic/crab lice spread?
Body lice are spread from person-to-person through close direct contact with a lice infested area of a person or through sharing of unlaundered clothes or bedding that had recent contact with an infested person. While other means are possible, pubic/crab lice are usually spread through sexual contact. Since nits (louse eggs) must be laid by adult lice and need warm temperatures to hatch, the chances of nits being spread from person-to-person are minimal.

These lice can also infest armpit hair and eyebrows, but they are drawn to the pubic region. Causes, incidence, and risk factors Return to top Pubic lice are known scientifically as Phthirus pubis . Lice infestation is a common sexually-transmitted condition and is found mostly in adolescents. Transmission generally occurs during sexual activity, but it can occur through physical contact with contaminated objects such as toilet seats, sheets, and blankets. Some women have acquired it while trying on swimming suits at a store. The presence of pubic lice is heralded by moderate to severe itching in the area covered by pubic hair. This itch is frequently worse at night. Because the crab louse requires human blood to survive, it buries its head inside a pubic hair follicle. It excretes a substance into the skin that causes the itching.

Method of diagnosis
Early detection makes treatment and control of head lice easier. Traditional scalp inspection is a poor method of detecting lice. It can result in 30% false positive and 10% false negative findings.
Incubation period
The life cycle consists of three stages: egg, nymph and adult. The eggs are known as nits and hatch in six to seven days. There are three nymphal forms that each last one to eight days.
Head lice have been associated with humans for 10 000 years. Head lice occur worldwide. Anyone can get lice and given the opportunity head lice will move from head to head without discrimination. They are frequently associated with children. Information on the prevalence of head lice varies around the world. In 2002 the prevalence of head lice among primary school children in Victoria was found to be 13%. Females were more than twice as likely to have head lice as males.
