Abscesses in the extradural or subdural spaces and those in the temporal lobe and cerebellum can cause vestibular complaints, as can a suppurative process in the apex of the petrous bone. [1]
VASCULAR DISORDERS
Abnormalities of blood flow to the vestibular system are very common causes of vestibular symptoms and are often difficult to distinguish from well-known end organ disorders.
Migraine
Etiology
Migraine headache results from dilatation of extracranial and dural arteries, causing stretching of sensitive pain fibers in the walls of the arteries, the symptoms of the aura reflect ischemia secondary to intracerebral. Serotonin is implicated in the pathogenesis of these phenomena.[2]
In order to elicit the appropriate history for a migraine the following must be asked:
1. scotomata, or blind spots
2. teichopsia, or fortification spectra, a zigzag pattern in the visual field 3. flashing (photopsia) or colored lights 4. paresthesia.

Clinical Services The Neurotology Program and Laboratories specialize in the diagnosis and treatment of disorders of the inner ear and eye movement control. Particular attention is paid to the causes of dizziness, tinnitus, and imbalance. The program has been in the forefront of the development of computer-assisted electronystagmography (ENG) and rotational testing. These tests are used to evaluate pathologic nystagmus, vestibulo-ocular reflex function, and visual ocular control in patients with dizziness and balance problems. Numerous publications characterize the unusual causes of dizziness and imbalance diagnosed in patients referred to the Neurotology Clinic:

* Drop attacks in Meniere's Disease
* Familial periodic ataxia
* Idiopathic bilateral vestibulopathy

THE VESTIBULAR SYSTEM IN HEALTH AND DISEASE What is the Vestibular System?
The vestibular system is the primary sensory system that maintains the animal's balance or its normal orientation relative to the gravitational field of the earth. This orientation is maintained in the face of linear or rotatory acceleration and tilting of the animal. The vestibular system is responsible for maintaining the position of the eyes, trunk, and the limbs in reference to the position or movement of the head at any time.

  • Bony and membranous labyrinth
    Semi-circular canals.

    These canals are paired right and left lateral duct, right posterior and left anterior ducts, and right anterior and left posterior ducts. These behave as synergistic pairs. While one is activated, the other is inhibited (less active). The receptors do not respond to constant velocity, but acceleration and deceleration. Each canal contains a material called endolymph and each canal terminates on an area (Ampulla) on neuroendothelium and gelatinous material called the Cristae Ampillaris. Rotation of the head on any plane causes endolymph flow and activation of the neuroepithelium (hair cells and the supporting cells).
