Claimant had previously injured his right knee in 1966 or 1967, an injury that bothered claimant for a couple of years but was not causing problems prior to April 27, 1996. (Tr. 18). However, Dr. Hoppert also opined that claimant did not require medical treatment for his medial meniscus condition. (Ex. 19-28). Based on Dr. Hoppert's persuasive medical opinion, we conclude that claimant failed to prove the compensability of his torn left medical meniscus. In this regard, we note Dr. McKillop's opinion that claimant's occupational exposure on April 27, 1996 brought about his symptoms that required medical care. (Ex. 10-3). In addition, Dr. Hardiman opined that claimant's work exposure was the major contributing cause of claimant's knee difficulty. (Ex. 17). We conclude that these opinions support a finding that the April 27, 1996 injury was the major factor in claimant's need for treatment of his bilateral patellofemoral condition.

    Abstact
    A 13 year old boy who has sustained a left lateral knee contusion while playing football, followed 2 weeks later by a mild knee blow inflicted bu a school bully. X-rays revealed an osteogenic sarcoma. This case illustrated the importence of the affected extremity when the diagnosis in doubt.
    The possibilites in injury are, a lateral collateral ligament strain or tear, physeal plate fracture, or a possible ACL,PCL, or lateral maniscus tear. Other possibilities include, discoid miniscus, patellofemoral disorder in a dislocation, medial patellar retinaculum symptoms, plica syndrome, and iliotibial band friction syndrome.
    Test results, by X-ray, demonstrate a permative bone lesion on the lateral aspect of the distal femur and a Codman's triangle superiorly-a trianglular area where the posterium, elevated by the bone tumor, rejoins the cortex of a normal bone. A CT scan showed a permative lesion of the postrolateral aspect if the distal femur extending into the soft tissue and raising the cortex laterally. A bone scan was normal, with the eception of greatly increased uptaske at the distal femur.
    The differential diagnosis is a malignant tumor, or ostiogenic sarcoma. The reason for the differential diagnosis is the athletes sex and age. This usually occurs in males from the ages from 10-20. These tumors may be secondary to trama or exposure to radiation. This tumor occurs in the metaphyseal area of lomg bones- the sight of the patients problem. Upper femur and proximal tibia, and the hemerus. The patient will experiance pain, swelling, and weight loss. In this case there was none.

    Background: Patellar injury and dislocation are common injuries in both athletic and nonathletic individuals. Among athletes, men tend to present with more patellofemoral injuries than women. In the nonathletic population, women present more commonly. Patellofemoral problems are mainly diagnosed by obtaining a thorough history and performing a physical examination. Imaging studies help confirm the diagnosis. Plain radiography is not as sensitive as MRI, but it is the least expensive and most readily available modality. Patellofemoral syndromes usually are the result of biomechanical imbalances of the kinetic chain. Therefore, in rehabilitative treatment, the most critical concept the patient must achieve is the ability to perform functional activities without surpassing his or her optimal joint-loading limit. Therapy techniques are designed around this principle. Surgery is more effective in preventing recurrence of dislocation, but it has its own risks. Surgery should be considered an option after all conservative treatment is unsuccessful. Traditionally, several different systems have been used to classify patellofemoral dysfunction. Some were developed from a functional perspective while others were developed from more of an anatomic viewpoint. Of the following authors who wrote on this subject, Insall and Merchant classified patellofemoral dysfunction according to anatomy.

    Patellofemoral pain syndrome (PFPS) is a common knee disorder, which often affects the senior athlete and those involved in running and jumping sports. Overuse, a change in activity, and an altered metabolic status are frequently responsible for the development of PFPS. Structural alignment and muscular weakness or imbalance may cause the patella (kneecap) to track improperly on the femur (thigh bone) during movement, leading to pain around the kneecap.
    Other terms for patellofemoral pain are: retropatellar pain, peripatellar pain, anterior knee pain, and runner's knee.

    Patellofemoral syndrome As this is the single most common condition seen by most sports medicine practitioners we devote substantial attention to it in this book. We discuss nomenclature, etiology of the pain, functional anatomy and predisposing factors, before outlining clinical assessment and treatment in detail. Nomenclature and origin of the pain Patellofemoral syndrome is the term used to describe pain in and around the patella. From the 1930s until the 1970s, the term 'chondromalacia patellae' was synonymous with patellofemoral pain, because softening was noted on the undersurface of the patella. This term is now out of vogue and has been replaced by nonspecific terms such as patellofemoral syndrome (which we use throughout this book), patellofemoral pain syndrome, patellofemoral joint syndrome, anterior knee pain, extensor mechanism disorder. There are two schools of thought as to the aetiology of patellofemoral pain. The longer standing theory invokes malalignment patellar malalignment relative to the femoral trochlea causes abnormalities within articular cartilage, which may or may not be visible to the naked eye. Correction of the malalignment should therefore decrease the patient's symptoms and even out loading of the articular cartilage. There is, however, poor correlation between articular cartilage lesions and pain, so patellar malalignment only explain a proportion of patellofemoral problems.

    Infrared Thermography and the Sports Injury Practice
    Infrared thermal imaging can be very useful to the sports injury practice. Infrared Thermography (IRT) measures cutaneous surface temperature which is reflective of the underlying sympathetics and local chemical mediators. Previous studies have shown that skin temperature from right to left sides only differ within a few tenths of a degree for homologous parts. In the case of the anterior calves, the normal mean temperature differential is .31 degrees C. Other studies have shown that temperature differentials over .65-.80 degrees C are indicative of underlying pathology. IRT has been found to be useful by both the AMA Council on Scientific Affairs and the ACA Council on Diagnostic Imaging, which has its own American Chiropractic College of Thermology. IRT is also helpful in the following conditions:

    by Joseph Kurnik,DC There is no clear consensus in the literature concerning the terminology, aetiology and treatment for pain in the anterior part of the knee. The term 'anterior knee pain' is suggested to encompass all pain-related problems. By excluding anterior knee pain due to intra-articular pathology, peripatellar tendinitis or bursitis, plica syndromes, Sinding Larsen's disease, Osgood Schlatter's disease, neuromas and other rarely occurring pathologies, it is suggested that remaining patients with a clinical presentation of anterior knee pain could be diagnosed with patellofemoral pain syndrome (PFPS). Three major contributing factors of PFPS are discussed: (1) malalignment of the lower extremity and/or the patella: (2) muscular imbalance of the lower extremity: and (3) overactivity.     The following paper aims at outlining the most common causes of anterior knee pain of which are commonly grouped into the category of Patellofemoral Pain Syndrome. Due to the enormity of the topic at hand acute injury related to trauma will be omitted. The purpose of the literature review is to briefly outline current concepts in the understanding of conditions, and the discussion will highlight details with respect to differential diagnosis. It is assumed that the reader has previous knowledge of knee biomechanics, as these will be briefly intimated in the formulation of etiological factors. A major limitation of this paper is the lack of inclusion of treatment strategy, largely due to the brevity of the paper in relation to size of the topic. Back to index.
