: buring/numbness in an area including the lateral aspect of the left knee and inferolateral thigh, and superior to the patella. From an extended position, the area extends 10cm superiorly from the patella and spreads superolaterally. The 'pain' is rated as 8/10. The pain feels like it is in the skin - "like a rash or cut". The patient also has objective knee swelling, especially obvious above the knee, with a positive patella tap sign and has a history of lateral dislocation at age 13. The current episode came on 4 months ago for no apparent reason. : x-rays and ultrasound were NAD. : The pain now comes on at rest, and prevents the patient from walking to the shop. Every now and then the patient also feeling a stabbing, shooting pain in the same distribution as described above. : On examination, all orthopaedic tests of the knee were negative, however the patient was quite apprehensive when moving the patella and said 'it feels like it is going to pop out!' The patella is highly mobile and sits quite high on the femur. Specific tests of patella movement were equivocal, due partly to the discomfort they caused the patient. There is no doubt though that there is something going on locally, in the knee joint.

Purpose: It is not clear how the knee extension torque is distributed quantitatively among the lateral and medial vasti in patellofemoral pain (PFP) patients with patellar malalignment, which was investigated in vivo and noninvasively in ten PFP patients and eleven controls. We hypothesized that the vastus medialis oblique (VMO) and vastus medialis longus (VML) of PFP patients contribute less to knee extension than that in controls. Methods: Electrical stimulation was used to activate each vastus component selectively. The relationship between the knee extension torque generated by each individual vastus component and the corresponding compound muscle action potential (M-wave) was established over different contraction levels, which was used to calibrate the corresponding voluntary EMG signal and determine torque ratios of VMO/VL (vastus lateralis), VMO/VML, VML/VL and (VMO+VML)/VL during voluntary isometric knee extension. Results: The VMO and VML of PFP patients contributed significantly less to knee extension than their counterparts in controls. The combination of VMO and VML generated comparable amount of extension torque as the VL in the controls, while it produced significantly lower extension torque than that of the VL in the PFP patients. In addition, the VMO/VL was lower than VMO/VML and VML/VL in both PFP and control groups.

Sports Exerc., Vol. 36, No. 1, pp. 93-101, 2004. Purpose: Unbalanced actions of the quadriceps components are closely linked to patellar mal-tracking and patellofemoral pain syndrome. However, it is not clear how individual quadriceps components pull and rotate the patella three dimensionally. The purpose of this study was to investigate in vivo and noninvasively patellar tracking induced by individual quadriceps components. Methods: Individual quadriceps component was activated selectively through electrical stimulation at the muscle motor point, and the resulting patellar tracking was measured in vivo and noninvasively in 18 knees of 12 subjects. The in vivo and noninvasively patellar tracking was corroborated with in vivo fluoroscopy and in vitro cadaver measurements. Results: Vastus medialis (VM) mainly pulled the patella first in the medial and second in the proximal directions and vastus lateralis (VL) pulled first in the proximal and second in the lateral directions. The oblique portion (VMO) of the VM pulled the patella mainly medially and the longus portion (VML) more proximally. Medial tilt was the major patellar rotation induced by VMO contraction at full knee extension. With the knee at the more flexed positions, the amplitude of patellar movement induced by comparable quadriceps contractions was reduced significantly compared to that at full knee extension, and VMO changed its main action from extending to flexing the patella.

INTRODUCTION
Advances in arthroscopy and knee replacement have led to high expectations and unprecedented demand. The knee is the most commonly injured joint and can often be treated conservatively, but many doctors, including consultants, frequently get the diagnosis wrong on first seeing a patient. Accurate identification of patients who need an orthopaedic opinion is needed. The aim of this report is to clarify history-taking and examination of the painful knee and to indicate appropriate management.
THE HISTORY
In acute knee injuries the history is all important. For example 'I felt my knee pop when I landed' , is typical of rupture of the anterior cruciate ligament (ACL): examination in such instances is limited due to swelling and guarding. The average delay in diagnosis is over 18 months! In contrast, examination is more revealing in patients with chronic knee problems when the joint is nicely relaxed. Indeed, it is the history which may be misleading. For instance

Course Description
Patellofemoral pain remains one of the most common conditions seen by physical therapists, yet evaluation and treatment programs remain highly variable and are often without scientific backing. The purpose of this course is to critically assess current approaches to the management of this disorder. Course participants will receive a thorough review of the clinical biomechanics of the patellofemoral joint, as well as an overview of the entire lower kinetic chain as it relates to patellofemoral pain and dysfunction. Emphasis will be placed on current research findings in the areas of gait analysis, Kinematic MRI, EMG, and patellofemoral joint mechanics. Critical review of long held, but poorly researched, areas of evaluation and treatment will be addressed. Participants will be challenged to critically review their beliefs regarding their etiology, assessment, and treatment approaches, but will also be provided with practical information to "take back" to the clinic for immediate use through hands-on lab experience.

WHAT IS PATELLOFEMORAL PAIN SYNDROME (Chondromalacia)? Patellofemoral pain syndrome is pain behind the kneecap. It has many given names, including patellofemoral disorder, patellar malalignment, runner's knee, and chondromalacia. HOW DOES IT OCCUR? Patellofemoral pain syndrome can occur from overuse of the knee in sports and activities such as running, walking, jumping, or bicycling. The kneecap (patella) is attached to the large group of muscles in the thigh called the quadriceps. It is also attached to the shin bone by the patellar tendon. The kneecap fits into grooves in the end of the thigh bone (femur) called the femoral condyle. With repeated bending and straightening of the knee, you can irritate the inside surface of the kneecap and cause pain. Patellofemoral pain syndrome also may result from the way your hips, legs, knees, or feet are aligned. This alignment problem can be caused by your having wide hips or underdeveloped thigh muscles, being knock-kneed, or having feet with arches that collapse when walking or running (a condition called over-pronation).
