Michael J. Fox reveals he has Parkinson's disease
Web posted on:
Wednesday, November 25, 1998 2:33:52 PM EST LOS ANGELES (CNN) Actor Michael J. Fox has been battling Parkinson's disease for the past seven years, and underwent brain surgery last March to lessen the tremors caused by the illness, he confirmed Wednesday. Fox, 37, made the revelations in an interview with People magazine, published in this Friday's edition. Fox's spokesperson released a statement saying, "It is important to Mr. Fox to convey that he lives a normal, thriving and happy life, and since his diagnosis seven years ago he has been active as an executive producer and star of a very successful television series." Most cases of Parkinson's disease not inherited, study says
SUNNYVALE, California (CNN) A new study suggests most cases of Parkinson's disease are not inherited, therefore suggesting environmental causes. The study followed almost 20,000 white male twins enrolled in the National Academy of Sciences/National Research Council World War II Veteran Twins Registry. Researchers found no evidence to suggest the disease is genetically linked in most cases. However, genetics do appear to be important when the disease begins before age 50. Only about 15 percent of all Parkinson's cases are in younger people. Researchers say the study addresses a 150-year-old debate regarding whether Parkinson's disease is inherited. "This is a very important study, which has the power to answer the question of heredity in Parkinson's disease," said Dr. Jorges Juncos of Emory University School of Medicine. "Therefore, this study suggests environment plays a major role in the development of Parkinson's."

A brain disease which, like Alzheimer's is uncomfortably common as we age. It's Parkinson's. And while there are drugs to treat it they are far from ideal. So researchers have been looking at drugs which might prevent the diseases progress and even forms of brain surgery including transplantation and a technique called 'deep brain stimulation'. One of the people who's been involved with all three is Warren Olanow, who's Professor of Neurology at Mount Sinai School of Medicine in New York. Professor Olanow: Parkinson's is a disease that is due to degeneration of nerve cells in the centre of the brain called the Substantia Nigra. When these cells degenerate patients lose a chemical called dopamine. And this results in a number of signs and symptoms which include tremor, rigidity or stiffness, bradykinesia or slowness, and difficulty with walking, tendency to fall, postural instabilities etc. Norman Swan: Has it also recently been shown that it's actually more widespread than the substantia nigra, large parts of the brain are affected and it can affect thinking ability as well?

Manganese and Parkinson's disease have been linked in a number of studies done around the world. People who work with manganese often suffer from a variety of symptoms, known as manganism, which experts have classified as a Parkinson's syndrome. Symptoms of manganese-related Parkinson's include tremors, lethargy, impaired concentration, speech difficulties and awkward gait. Parkinson's from manganese exposure continues to progress clinically even years later, when traces of manganese can no longer be detected in the body. Workers in the welding industry are especially at risk, as they are often exposed to airborne manganese, and Parkinson's disease is disproportionately common among this population. In studying connections between manganese and Parkinson's, doctors found some alarming correlations. Improper work conditions exist in many industries involving manganese and Parkinson's cases among workers in the same locations are not uncommon. Disturbing links between exposure to manganese and Parkinson's emerged again after a ventilation system in a ferromanganese smelter broke. In a group of eight welders, six developed Parkinsonism (manganism) after being exposed to manganese for thirty minutes a day, seven days a week.

Med.Sci 532 Structure-Function Parkinson Disease Parkinson disease (PD) is characterized by a slowing of voluntary movements, bradykinesia, muscular rigidity and tremor at rest. These abnormalities result from a reduction of neurons that make dopamine in the pars compacta of the substantia nigra. The axons of these neurons normally release this neurotransmitter where they synapse, in parts of the basal ganglia called the caudate nucleus and putamen or, collectively, the corpus striatum. Dopamine usually works as an inhibitory neurotransmitter in the corpus striatum where it acts on cholinergic neurons. Lewy bodies, a hyaline inclusion, are seen microscopically in the cytoplasm of residual neurons in the substantia nigra of nearly all patients with Parkinsonism. The loss of neurons results in a grossly evident depigmentation of the substantia nigra because those dopaminergic neurons also contain the pigment neuromelanin in their cytoplasm. There are various causes for the loss of dopaminergic neurons in the substantia nigra and the resultant signs of PD. Some evidence exists for involvement of genetic factors in the pathogenesis of PD. For example, several large families with an autosomal, dominant inheritance pattern of PD have now been described, and the first genetic locus for PD has been identified in one of those families.

How I discovered that I had Parkinson's At first my husband noticed something strange. I walked without
swinging my right arm - it just hung down loosely at my side. I was
unaware of any pain or discomfort and was rather reluctant to go to
    the doctor.
Then we wondered whether I had a tumour on the brain and on the
recommendation of a doctor friend, we consult a Neurologist. After
a Magnetic Resonance Scan it was established that there was no tumour. By process of elimination, it was diagnosed that I had Parkinson's. Having had absolutely no record of my family having this disease, I was astounded. However, one must face reality. My symptoms Looking back now, various Parkinson's symptoms developed which were initially overlooked. I did not have much of a tremor as such, only a slight one under stress. One of my more recent symptoms was "frozen feet". This feels as though your brain is not getting through to your feet. In other words, you tell your feet to move

The Neurobiology of Parkinsons Disease, part 2:
further research and parallels to Alzheimers?
    Rani Shankar
In my previous paper, the basic pathogenesis and neurobiology of Parkinsons Disease was discussed. Parkinsons Disease is recognized as a progressive neurodegenerative condition that is believed to be caused by a combination of genetic and environmental factors. Parkinsons Disease is characterized by a deficit in dopamine, a neurotransmitter produced in the substantia nigra, that is responsible for maintaining normal motor functioning. This paper will continue the exploration of Parkinsons Disease research, also examining Lewy Bodies, genetic factors, and drawing parallels and possible links to Alzheimers Disease. Lewy Bodies, aggregated eosiphilic cytoplasmic inclusion bodies found in substantia nigra, are a hallmark characteristic of Parkinsons disease identified by pathological studies. As mentioned in my previous paper, Lewy Bodies are not able to be used to diagnose Parkinsons, as they are only observable during an autopsy, but are a fool-proof method of identifying the disease, after the fact. Lewy Bodies are present in dementia and found in some cases of Alzheimers Disease. In Parkinsons, the cells of the substantia nigra die, so that the normal dark brown pigment (hence, the name, nigra) is instead pale. The Lewy Bodies are among the dying nerve cells left in the substantia nigra

    Parkinsons disease drugs
Drugs used to treat Parkinson’s Disease consist of a combination of dopaminergic and antimuscarinic drug.
Parkinson’s disease is a progressive, incurable neurological disease. The three main symptoms are slowness of movement (bradykinesia), tremor and rigidity (stiffness). Treatment for this disease has been continually changing over the past 30 years, especially with the introduction of levodopa. Present-day medicine helps improve symptoms, delays the onset of total physical dependence and hence delays morbidity and mortality, which result due to progressive immobility. Initially the use of drug treatment should be withheld and the patient’s progress monitored. Drug treatment should begin once significant functional disability sets in and the treatment regime should be monitored regularly, especially regarding timing of doses, benefit obtained and the severity of any side-effects. Dopaminergic drugs: LEVODOPA is used with a dopa-decarboxylase inhibitor, which prevents peripheral degeneration of levodopa to dopamine, which does not cross the blood-brain barrier. It is the amino-acid precursor of dopamine, which helps replace the depleted striatal dopamine and so helps to decrease rigidity and bradykinesia. This drug has less effect on tremor. The extracerebral dopa-decarboxylase inhibitors used are benserazide (co-beneldopa) and carbidopa (co-careldopa). The reduced levels of peripheral dopamine decreases possible side effects such as nausea, vomiting and cardiovascular problems. There is also a shorter time lapse between the onset of therapeutic effects and a smoother clinical response. However, there usually is an increase in abnormal involuntary movements and psychiatric complications.
