Pancreatitis One of the functions of the pancreas is production of digestive enzymes. These are the enzymes that break down ingested foods into molecules the body can digest. These enzymes are carefully handled by the pancreas in order to prevent them from damaging the pancreas itself or surrounding tissue. When these self-protective mechanisms break down for any reason, the result is leakage of enyzmes which damage the pancreas and any surrounding tissue they reach. This breakdown is called pancreatitis. There are a number of things which can initiate or facilitate enzyme leakage, so pancreatitis can occur for a number or reasons. Often, it takes a combination of precipitating factors to cause pancreatitis to occur in a dog or cat. High fat diets, obesity and lack of exercise are the most common "life-style" contributors. Miniature schnauzers are predisposed to pancreatitis due to a tendency to have high levels of lipoproteins in their blood streams. Corticosteroids and azathioprine medications may contribute to the tendency to develop pancreatitis. Hyperadrenocorticism, a naturally occurring overproduction of corticosteriods that is fairly common in dogs may also lead to an increased susceptibility to pancreatitis. Anything that interferes with blood supply to the pancreas or release of digestive enzymes by the pancreas may lead to pancreatitis. For some reason, pancreatitis does not occur, or is not recognized, as often in cats as it does in dogs.

PANCREATITIS IN CATS Pancreatitis, or inflammation of the pancreas, is a disease seen frequently in dogs. In cats it used to be considered rare but we now believe that it is simply difficult to diagnose when it occurs and is probably a common problem, especially in older cats. The pancreas is an important organ located near the small intestine. It is responsible for the production of most of the enzymes that digest food, and it also produces insulin. When the pancreas is diseased or injured, the enzymes it produces leak in and around the pancreas, damaging it and the other organs nearby. These enzymes that are leaking out break down fat and protein locally, instead of traveling down the pancreatic duct to the intestines. In effect, the cat or dog digests its own tissues. The abdomen soon becomes inflamed, and may subsequently become infected. An animal with pancreatitis may show such symptoms as lethargy, fever, vomiting, diarrhea (which is often bloody), refusal to eat and abdominal pain. Symptoms may start suddenly or come on gradually, and can range from mild and intermittent to severe and life threatening. In cats it is often subtle, appearing as vague signs such as poor appetite or gradual weight loss, without the more dramatic signs that are usually seen in dogs. The disease may be seen in any breed at any age, but it seems to be more common in older pets. Pancreatitis occasionally occurs soon after ingestion of garbage, dead animals or a fatty meal (scraps of fatty meat, cheese, whole milk, chicken skins, etc.). In many cases, however, the disease is caused by the body

HomeVet This disorder of the digestive system is potentially life threatening (particularly in its acute form) but often responds well to treatment. It is more common in some breeds than others and is frequently seen in German Shepherd Dogs. Successful treatment of pancreatic problems requires close cooperation between owner and veterinarian, as well as a certain flexibility in working with changes in the animal's condition. The pancreas serves two separate functions, endocrine and exocrine. The endocrine gland secretes hormones, notably insulin, and the most feared pancreatic malfunction seen by endocrinologists (human and veterinary alike) is of course diabetes. The exocrine gland secretes enzymes into the small intestine, aiding digestion. Pancreatic insufficiency in dogs is not, thus, an endocrine problem like diabetes but rather an exocrine problem, referring to the organ's failure to produce digetive enzymes. The condition is therefore properly referred to as Exocrine Pancreatic Insufficiency, often shortened in the veterinary field to EPI. Some authorities note that pancreatitis (the acute form) is particularly common in spayed, overweight females in middle age, and most agree that excessive amounts of fat in the diet are likely to trigger this form. As to animals suffering from EPI that have not previously had pancreatitis, some sources designate an onset by the age of two to three years in previously healthy animals, but seldom later. The prevalence of the chronic disease in German Shepherd Dogs has led to speculation about genetic links.

The Midwest Multicenter Pancreatic Study Group (MMPSG), in cooperation with the International Pancreatitis Study Group is maintaining a registry of patients with hereditary pancreatitis. The purpose is to follow the development of complications and other problems associated with hereditary pancreatitis. The Hereditary Pancreatitis Data Collection Form should be filled out the physician caring for the family member. The family members name should not appear on the form. If there is a problem completing this form either email Dr. Whitcomb or Dr. Lowenfels or call the Pancreatic Disease Study line at 888-PITT-DNA.
