Causes, incidence, and risk factors: Otosclerosis is an inherited disorder involving the growth of abnormal spongy bone in the middle ear. This growth prevents the stapes (stirrup bone) from vibrating in response to sound waves, thus leading to progressive (worsening over time) hearing loss
Otosclerosis is the most frequent cause of middle ear hearing loss in young adults, affecting about 10% of the population of the US. Otosclerosis usually affects both ears, and is most commonly seen in women, 15 to 30 years old.
Risks include pregnancy (which may trigger onset) and a family history of hearing loss. Caucasians are more susceptible to otosclerosis than others.

Hearing Loss From Otosclerosis
Otosclerosis is a disorder of the bone that surrounds the inner ear. A more accurate term for this is otospongiosis, which means spongy bone. This disorder often occurs in the bones, between middle and the inner ear. The stapes is a tiny middle ear bone that transmits sound waves by banging on another area, like a stick on a drum creates sound. Otosclerosis can cause the stapes to become fixed, so that it cannot "beat on" the drum. Many people are unaware they have otosclerosis. When it affects the inner ear, nerve deafness and dizziness may result. One or both ears may be affected. It is almost always accompanied by ringing in the ear, which is called tinnitus. About one million people in the U.S. are affected by this hereditary ear disorder. It may occur in childhood, but hearing loss does not occur until a person is 20 to 40 years old. It is mostly seen in white women and may become active during pregnancy. It is also one of the leading causes of hearing loss in the elderly. Unlike some causes of hearing loss, a hearing loss caused by otosclerosis can be treated. Treatment includes surgery to stabilize or replace the small bones of the inner ear. Surgery can usually return hearing to a normal or near normal status. If surgery is not possible, a hearing aid may be helpful.

In otosclerosis, the stapes bone becomes fixed and does not vibrate normally. This lack of normal vibration leads to a conductive hearing loss. This hearing loss can be corrected surgically. The surgery is known as a stapedectomy . In the surgery, a platinum or stainless steel piston is used to bypass that portion of the stapes that is not working correctly. This surgery is extremely effective and is done on an outpatient basis. Hearing aids are also an option in relieving the perceived hearing loss in otosclerosis. What Is Otosclerosis?
Otosclerosis is a condition affecting the stapes bone. A normal stapes bone moves like a small piston; sounds cause the piston to vibrate. In otosclerosis, a callus of bone accumulates on the stapes creating a partial fixation. This limits the movement of the stapes bone, which results in hearing loss.
What Causes Otosclerosis?
There is a specific gene, which if present in the patient’s genetic make up, can result in the development of otosclerosis. Some medical studies have implicated the measles virus as a factor in causing otosclerosis. In some women with otosclerosis, pregnancy can accelerate the process.
Is There Anything The Patient Did To Cause Otosclerosis?
There is nothing a person does or does not do to cause otosclerosis. Most people do not have the gene that causes otosclerosis. Patients do not have control over their genetic make up.

Otosclerosis is a relatively common cause of hearing loss and may run in families. The disorder is a condition affecting the bones of the middle ear, and is only known to effect humans. The hearing loss caused by otosclerosis is often treatable by surgery.
The normal ear and hearing loss
The ear is divided into the external, middle and inner ear. Sound waves pass into the external ear and vibrate the eardrum. The movements of the eardrum are transmitted into the middle ear by three small bones. These are the malleus (hammer), incus (anvil) and the stapes (stirrup). The bones transform sound waves into vibrations of the inner ear fluids, turning sound waves into electrical impulses.
The external and middle ear act as conductors of sound and the inner ear is the receiver. A disorder of the external or middle ear causes a conductive hearing loss. A problem involving the inner ear causes a sensorineural or nerve hearing loss. Otosclerosis often causes a combination of these hearing losses but the dominant component is conductive.
Otosclerosis
Otosclerosis causes tiny areas of bone to harden. These hardened areas may affect the stapes bone, inner ear or both of these areas. Pure inner ear otosclerosis is rare. Otosclerosis typically begins in the teenage years or third decade of life. The condition is reported more frequently in females and usually, but not always, involves both ears. Carefully performed and accurate hearing tests determine the degree of hearing loss due to stapes or inner ear involvement.

The most common cause of conductive hearing loss in adults is otosclerosis. About 10% of the entire population has otosclerosis but only 10% of those have hearing loss as a result. The loss results from fixation of the stapes (the third bone in the middle ear) so that sounds cannot be transported to the inner ear . Otosclerosis is thought to be an inherited condition. It usually begins in early adulthood and progresses slowly, typically causing up to a 60 dB loss in both ears. What is Otosclerosis?
Otosclerosis is a disease in which there is abnormal hardening of the bone of the ear. This hardening causes the third middle ear bone, known as the stapes, to become fixed. Fixation of the stapes prevents it from moving and thus, prevents the transmission of sound of the middle ear to the inner ear.
What causes Otosclerosis?
There are many interesting studies that pertain to the abnormalities of bone in the ear. We know that otosclerosis is an inherited disorder and that many people with this disorder will report similar symptoms in their parents or siblings. Other patients report no family history of this disorder. This is not because their family members were not affected by otosclerosis, but rather because the hardening of the bone within the ear did not take place in the region of the stapes.
How is otosclerosis diagnosed?
Otosclerosis is diagnosed by a combination of the patient history, physical examination, and a hearing test that shows significant middle ear (conductive) hearing loss. This diagnosis can only be confirmed at the time of surgery when the abnormal bony growth is visualized within the middle ear.
