    Amniotic Fluid Problems / Hydramnios / Oligohydramnios
    What is amniotic fluid?
    Amniotic fluid is an important part of pregnancy and fetal development. This watery fluid is inside a casing called the amniotic membrane (or sac) and fluid surrounds the fetus throughout pregnancy. Normal amounts may vary, but, generally, women carry about 500 ml of amniotic fluid. Amniotic fluid helps protect and cushion the fetus and plays an important role in the development of many of the fetal organs including the lungs, kidneys, and gastrointestinal tract. Fluid is produced by the fetal lungs and kidneys. It is taken up with fetal swallowing and sent across the placenta to the mother's circulation. Amniotic fluid problems occur in about 7 percent of pregnancies. Too much or too little amniotic fluid is associated with abnormalities in development and pregnancy complications. Differences in the amount of fluid may be the cause or the result of the problem.

Measurement of the volume of amniotic fluid is extremely difficult and subjective assessment is still used by many. Measurement of the depth of pools of amniotic fluid gives a semiobjective method of assessment either in the form of amniotic fluid index or just as maximum pool depth. The commonest causes of oligohydramnios are premature rupture of membranes, intrauterine growth retardation renal tract abnormalities and major syndrome complexes. Fetal ultrasound usually demonstrates the renal tracts with some accuracy although oligohydramnios does prevent high quality demonstration of the kidneys. It will usually exclude obstructive renal disorder. Renal agenesis can be diagnosed but the level of certainty of the diagnosis is hampered by the lack of amniotic fluid.

Renal agenesis If there is a major problem with either the production of liquor (amniotic fluid), or the outflow from the bladder to the amniotic cavity then there will be little or no liquor around the fetus. This has devastating consequences for the development of the fetal lungs. If there is no liquor production the normal development of the fetal lungs cannot occur. When there is little or no liquor around a fetus the quality of the ultrasound images obtained is often poor. If the bladder is seen it is most likely that the lack of liquor is due to either rupture of membranes or severe placental failure, both of which can give a similar pattern of reduced liquor volume. In some cases, it may be difficult to be certain whether there is any renal tissue present or not. The bladder is more easily seen on ultrasound than the kidneys. After 24 weeks gestation is much less likely to be due to renal anomalies, and the other causes, namely rupture of membranes and placental failure need to be considered. Although occasionally a cause of major fetal problems, these diagnoses will usually have a good outcome.

Oligohydramnios What is oligohydramnios Oligohydramnios refers to a condition where there is significantly less amniotic fluid around the foetus. This disorder indicates some abnormality in the pregnancy. An abnormality that affects the production of urine in the foetus may be linked to the development of oligohydramnios.
How is this condition diagnosed and treated The size of the abdomen could indicate the existence of this condition. A suspiciously small abdomen could indicate oligohydramnios. The diagnosis can be confirmed by doing an ultrasound. If a woman has oligohydramnios, depending on the woman’s health and pregnancy records, the doctor may recommend one or all of the following:
  • Change in the mother’s diet (increase in fluids, nutritive foods, etc); Complete bed rest (if intrauterine growth retardation is suspected); Additional tests to determine if there are any genetic deformities.

A patient is 32 wks gestation with a prior hx of oligohydramnios. She presents to the MD office with decreased fetal movement. Are there any studies available that suggest immediate ultasound and possible C-section would increase the survival rate of the child. I would truly appreciate this information for a research paper I am doing. Thank you very much. Please respond asap.

I cannot say there are no research papers that address the scenario you describe, but I would think you need more specific criteria that that you have stated. Determining the need for an immediate C-section at 32 weeks is very complex. By the way, would you please indentify yourself with your professional credentials?

      Too little amniotic fluid: Oligohydramnios
      by Peg Plumbo, CNM (see more from this expert) My wife has been diagnosed with oligohydramnios, with fluid levels at five and a half centimeters at seven months. With hospital bed rest and IV fluids, the level rose above seven. Ultrasounds did not show any obvious disorder. The doctor said that the level of fluid was mostly controlled by the baby, not my wife's fluid intake. If so, why did the IVs raise the fluid levels? Why does bed rest help with the levels?
      Oligohydramnios, or low amounts of amniotic fluid, is defined in terms of "pockets." If it is between two and five, this is considered low. But there are changes in interpretation in the literature every day, it seems. Recently, I read that a two centimeter vertical pocket is considered normal, a one to two centimeter pocket is marginal and less than one centimeter is decreased. With low levels, there is an increased risk of fetal heart decelerations in labor. Your provider may induce labor at term to keep you from going overdue, when the risks would be even higher.

Feb. 7, 2003 (HealthScoutNews) A low level of amniotic fluid in the last trimester of a pregnancy, often thought to be sufficient cause to induce delivery, is not reason enough to do so. Johns Hopkins researchers who studied the health of more than 250 babies born at 37 weeks of gestation say they found that babies whose mothers had low levels of amniotic fluid were of normal size. And the babies had no greater risk for health problems than babies whose mothers had normal levels of amniotic fluid. "This study indicates that we don't want to intervene because of a Amniotic Fluid Index (AFI) of less than five if everything else is normal," says study author Dr. Ernest M. Graham, an assistant professor of gynecology and obstetrics at Johns Hopkins University.

Hydramnios and Oligohydramnios
Hydramnios refers to an excessive amount of amniotic fluid, whereas oligohydramnios refers to a lack of amniotic fluid. The two conditions are diagnosed by a larger or smaller abdomen size, respectively. Hydramnios can be a problem for the mother because it may interfere with breathing. It can be treated by performing amniocentesis to remove the excess fluid. Oligohydramnios can be dangerous for the fetus because it may impede the development of lungs, arms and legs. Twisting or compression of the ambilical cord may result, thus impairing the transport of oxygen and nutrients to the fetus.
