Objectives: The objective of this review was to assess the effects of prophylactic amnioinfusion for oligohydramnios compared with therapeutic amnioinfusion only if fetal heart rate decelerations or thick meconium-staining of the liquor occur. Search strategy: The Cochrane Pregnancy and Childbirth Group trials register and Cochrane Controlled Trials Register were searched. Date of last search: October 2001. Selection criteria: Randomised trials comparing prophylactic with therapeutic amnioinfusion in women with oligohydramnios but not fetal heart rate deceleration in labour. Objectives: The objective of this review was to assess the effects of maternal hydration on amniotic fluid volume and measures of pregnancy outcome. What is amniotic fluid?
Amniotic fluid is an important part of pregnancy and fetal development. This watery fluid is inside a casing called the amniotic membrane (or sac) and fluid surrounds the fetus throughout pregnancy. Normal amounts may vary, but, generally, women carry about 500 ml of amniotic fluid. Amniotic fluid helps protect and cushion the fetus and plays an important role in the development of many of the fetal organs including the lungs, kidneys, and gastrointestinal tract. Fluid is produced by the fetal lungs and kidneys. It is taken up with fetal swallowing and sent across the placenta to the mother's circulation. Amniotic fluid problems occur in about 7 percent of pregnancies. Too much or too little amniotic fluid is associated with abnormalities in development and pregnancy complications. Differences in the amount of fluid may be the cause or the result of the problem.

Family physicians providing maternity care often face a scenario in which an otherwise low-risk, term patient is incidentally noted to have a low amniotic fluid index (AFI). Common reasons for obtaining an AFI in a woman with a low-risk pregnancy include evaluation of decreased fetal movement, spontaneous variable decelerations during monitoring to evaluate for labor, or an ultrasound evaluation for fundal height measurements discordant with gestational age. How should "isolated" oligohydranmiosan AFI <5 cmbe interpreted, and should immediate induction be recommended for such patients? Oligohydramnios occurs in about 1% to 5% of pregnancies at term. (1,2) Because adverse outcomes occur in high-risk pregnancies complicated by low amniotic fluid volume, oligohydramnios commonly prompts labor induction? (1,3,4) At one university center, oligohydramnios is now the leading indication for labor induction? Many centers may even induce labor when the AFI is between 5 cm and 8 cm, the so-called borderline AFI. (3)

    Oligohydramnios
    Abnormally low volume of amniotic fluid. Volume of fluid is generally at its maximum at 36-37 weeks' gestation with 800-1000ml. In the second half of pregnancy, when the condition occurs, fetal urinary output is responsible for the production of amniotic fluid. A deficiency can result from:
    • Premature rupture of the membranes
    • Chronic amniotic leakage
    • Maternal NSAID or ACE inhibitor usage
    • Placental insufficiency
    • Congenital abnormalities of the urinary tract
    • Twin-twin transfusion in the donor twin.
    The most serious consequence of oligohydramnios is the underdevelopment of fetal lung tissue ( pulmonary hypoplasia ). This occurs because of compression of the fetal abdomen restricting movement of the diaphragm and preventing the lungs exercising. It is also associated with meconium staining , heart conduction anomalies and fetal acidosis.

      The effect of maternal hydration with IV isotonic fluid, IV hypotonic fluid and oral water on the amniotic fluid index in women with oligohydramnios were reported.
      Eighty-four patients with low AFI gestational age older than 35 weeks and without maternal complications were assigned randomly to 1 of the 3 treatment groups or a control group. The mean increase in AFI after hydration was significantly greater in the IV hypotonic and oral water groups compared with control group. Hydration with IV hypotonic or oral water increases amniotic fluid index in women with oligohydramnios. Maternal osomotic change had a greater direct effect on increasing amniotic fluid volume with short-term acute hydration than maternal volume expansion.
      The approach offers the possibility of avoiding labour induction solely on grounds of a reduced AFI at or past term through use of oral water administration.

      183 women under-going labour induction for isolated oligohydramnios at between 37 and 42 weeks gestation were studied. It was found that isolated oligohydramnios in otherwise normal term pregnancies does not necessarily indicate fetal compromise. Thus in most women with isolated oligohydramnios, labour induction may not be warranted.

    General Discussion Amniotic bands is an abnormal condition of fetal development in which fibrous bands of tissue that originate from the amniotic sac encircle and constrict certain fetal areas, disrupting fetal growth. The amniotic sac is the thin, membranous sac that contains amniotic fluid and the developing fetus during pregnancy. Amniotic band formation may be associated with early rupture of the amniotic membrane, chronic leakage and abnormally decreased levels of the fluid surrounding the developing fetus (oligohydramnios), and abnormally decreased fetal movements. Associated structural malformations are extremely variable and may depend upon the time of amniotic band formation during fetal development. Such deformities may include abnormalities of the arms and legs (limbs), fingers and toes (digits), spine, lungs, facial area, skull, and/or abdominal and chest region.
