Absent amniotic fluid due to renal agenesis
Discussion
Profound oligohydramnios may result from fetal urinary tract anomalies that include bilateral renal agenesis, posterior urethral valves, and bilateral multicystic dysplastic kidneys. Nonrenal etiologies include intrauterine growth retardation, post-term pregnancy, fetal demise, and ruptured membranes. Due to the lack of amniotic fluid, the lungs are hypoplastic and neonates who survive delivery die of respiratory failure. Severe and prolonged oligohydramnios leads to a constellation of findings that include Potter's facies (flattened nose, recessed chin, and low-set ears) and positional abnormalities (clubbed feet, dislocated hips, and contractures). Bilateral renal agenesis is a result of absence or failure of development of the ureteric bud and occurs in approximately 1 in 4,000 births. The inability to produce urine results in severe oligohydromnios and nonvisualization of the bladder. Absence of the kidney may be difficult to diagnose due to severe oligohydramnios. A normal bladder excludes the diagnosis. Bilateral renal agenesis is incompatible with life.

By Laurie Dove When Koni Howard was 25 and pregnant with her second child, she sensed something wasn't quite right. The Oak Ridge, Tenn., mother wasn't gaining much weight, and at 36 weeks into the pregnancy, was continuing too measure small for the baby's due date. "Unlike my first baby, [my second child] hardly ever moved," she says. "I had a feeling there might be a problem." She just didn't know what the problem might be. Despite numerous blood tests and non-stress tests, which tracked the number of movements made by the baby, Howard's obstetrician didn't find anything to cause major concern. However, at 36 weeks into the pregnancy, an ultrasound showed low amniotic fluid levels. Howard was diagnosed with oligohydramnios, a condition in which the amniotic fluid surrounding the baby is below normal levels. "I was extremely surprised," Howard says, of both the diagnosis and what happened next. She had already returned home after the ultrasound and expected to hear the results at her next appointment. "When the phone rang, I never suspected it would be my doctor," she says. "I knew it was after 5 p.m. and her offices were closed. So when she told me, 'I think we're ready to have this baby,' I knew something was up."

What is amniotic fluid?
Amniotic fluid is an important part of pregnancy and fetal development. This watery fluid is inside a casing called the amniotic membrane (or sac) and fluid surrounds the fetus throughout pregnancy. Normal amounts may vary, but, generally, women carry about 500 ml of amniotic fluid. Amniotic fluid helps protect and cushion the fetus and plays an important role in the development of many of the fetal organs including the lungs, kidneys, and gastrointestinal tract. Fluid is produced by the fetal lungs and kidneys. It is taken up with fetal swallowing and sent across the placenta to the mother's circulation. Amniotic fluid problems occur in about 7 percent of pregnancies. Too much or too little amniotic fluid is associated with abnormalities in development and pregnancy complications. Differences in the amount of fluid may be the cause or the result of the problem.

Oligohydramnios What you need to know:
Oligohydramnios is the term for too little amniotic fluid. This condition affects about 8 percent of pregnancies. It can develop at any time in pregnancy, although it is most common in the last trimester. About 12 percent of women whose pregnancies last two weeks beyond their due date develop oligohydramnios as amniotic fluid levels naturally decline. Oligohydramnios is diagnosed with ultrasound
The most important known cause of oligohydramnios early in pregnancy is birth defects in the baby (often involving the kidneys or other parts of the urinary tract ) and ruptured membranes. The effect of oligohydramnios on the baby depends on the cause, the stage of pregnancy in which the problem occurs, and how little fluid there is.
  • In the first half of pregnancy, too little amniotic fluid is associated with birth defects of the lungs and limbs and increases the risk of miscarriage preterm birth and stillbirth
  • When oligohydramnios occurs in the second half of pregnancy, it is associated with poor fetal growth.

Amniotic Fluid Abnormalities
The amniotic fluid that surrounds a developing baby plays a crucial role in normal development. This clear-colored liquid cushions and protects the baby and provides it with fluids. By the second trimester, the baby is able to breathe the fluid into his lungs and to swallow it, promoting normal growth and development of the lungs and gastrointestinal system. Amniotic fluid also allows the baby to move around, which aids in normal development of muscle and bone.
The amniotic sac that contains the embryo forms about 12 days after conception. Amniotic fluid immediately begins to fill the sac. In the early weeks of pregnancy, amniotic fluid consists mainly of water supplied by the mother. After about 12 weeks, fetal urine makes up most of the fluid.
The amount of amniotic fluid increases until about 28 to 32 weeks of pregnancy, when it measures a little less than 1 quart. After that time, the level of fluid generally stays about the same until the baby is full term (about 37 to 40 weeks), when the level begins to decline.

Objectives: The objective of this review was to assess the effects of prophylactic amnioinfusion for oligohydramnios compared with therapeutic amnioinfusion only if fetal heart rate decelerations or thick meconium-staining of the liquor occur. Search strategy: The Cochrane Pregnancy and Childbirth Group trials register and Cochrane Controlled Trials Register were searched. Date of last search: October 2001. Selection criteria: Randomised trials comparing prophylactic with therapeutic amnioinfusion in women with oligohydramnios but not fetal heart rate deceleration in labour. What is amniotic fluid? Amniotic fluid is the liquid that surrounds your baby as he grows in the amniotic sac in your uterus during pregnancy. It cushions your baby against trauma (if you fall down, for example), keeps the umbilical cord from being compressed (which can reduce your baby's oxygen supply), and protects both your baby and your uterus from infection. Your baby will produce this fluid in his lungs and swallow it to help his breathing and digestive systems develop properly.
