Peer Review Status: Internally Peer Reviewed INTRODUCTION DIAGNOSIS AND CLINICAL PRESENTATION PATHOPHYSIOLOGY The true pathophysiology of OCD is unknown at this time, but many theories have surfaced which are based on what we know has been effective to treat this disease and our knowledge of how these medications work. The role of serotonin (5HT) has been highly questioned and investigated. Since medications that are serotonin reuptake inhibitors have been shown to be effective in OCD, it is believed that changes in 5HT function are critical for the treatment of this disease, and perhaps also play a role in the pathophysiology of the disease. The role of serotonin in OCD has been demonstrated in studies using serotonin agonists and antagonists. M-CPP is a 5-HT 1C receptor agonist that also bind to 5-HT2, 5-HT1A, and 5-HT1B receptors. Administering this compound causes acute exacerbations of OCD symptoms, while fenfluramine, which is a 5-HT presynaptic agonist, does not exacerbate symptoms (Chouinard 1992). PHARMACOTHERAPY SEROTONIN REUPTAKE INHIBITORS CLOMIPRAMINE CMI has been studied in at least 21 different controlled trials for the treatment of OCD. In 8 of these trials CMI was tested against placebo alone while in the remainder CMI was tested against other medications such as the SRIs, the TCAs, clonazepam and diphenhydramine. The majority of these studies are summarized in Table One.

A CASE HISTORY Bob saw his psychiatrist for treatment of depression for six months before he finally had the courage to bring up his other 'secret' problem. Since childhood he had a compulsion to count things. He had to count the letters in words and in people's names. If the letters added up to any number except 9 he felt a sense of release and could stop counting. He knew it was silly but nevertheless he had a fear that if he did not do this something bad could happen to his mom or dad. He seemed unable to stop doing this. He did poorly in school because he was distracted by his secret compulsion to count letters when he should have been paying attention to the teacher's lessons. He was later bothered as a teenager by upsetting sacrilegious mental images when he was in church. Having these sacrilegious images made him feel that he lost his soul for eternity.

Overriding obsession
Thought control All of us can think of an occasion when an idea or mental image has popped into our head without warning. These might be completely senseless, or they might be things we know we shouldn't do. But most of us are never bothered by these thoughts and we can easily forget them. Obsessive-compulsives are tortured by distressing mental images Torment Intrusive thoughts, or obsessions as psychologists call them, affect everyone. But some people can't get rid of them as easily as the rest of us. People who suffer from obsessive-compulsive disorder (OCD) are plagued by intrusive thoughts that they can't banish from their mind, no matter how hard they try. Obsessive-compulsives are tortured by these thoughts, which they find profoundly disgusting and distressing. The obsessions experienced by OCD sufferers can be grouped along several common themes. I work part time as a consultant and NLP therapist at a private mental health facility in North Carollina. I received your EFT training materials about three weeks ago and have begun to utilize the techniques. The Director of the clinic received a call concerning a four-year-old who had developed an OCD or obsessive compulsive disorder that manifests itself as repeated hand washings. The youngster was washing his hands over thirty times an evening and would rage and cry if restrained from the ritual washings. The parents were distraught and were on the verge of panic. I was asked if I would see the youngster and I subsequently scheduled an appointment. I talked with the mother beforehand and discussed with her the importance of immediately allowing the behaviors to occur without the parents attaching any unusual notice to the washings, to act as if everything was normal. This reduced the traumatic effects to the youngster and in effect desensitized the behavior.
